The Home Farm Trust is registered to provide accommodation and personal care for up to 8 persons with a learning disability, in the age range of 18 to 65 years. Roslyn House is a large, three storey and detached building close to the centre of Wadebridge Town. The home is close to community facilities, shops and public transport. The service users bedroom accommodation is provided on the ground and first floors. There is an enclosed, small garden to the front of the building. There is parking to the rear of the building, accessed through a private secure area, which also contains the separate resource centre building owned by the Home Farm Trust. Fees range from £513.75 to £1677.81 per week.

Annual Service Review Page 2 of 4 Service update since the last key inspection or annual service review: What did we do for this annual service review? We looked at all the information that we have received, or asked for, since the last key inspection or annual service review. This included: the annual quality assurance assessment (AQAA) that was sent to us by this service (the AQAA is a self-assessment that focuses on how well outcomes are being met for people using the service, and it also gave us some numerical information about the service); Information we have about how the service has managed any complaints; What the service has told us about things that have happened in the service, these are called notifications and are a legal requirement; The previous key inspection and random inspection. What has this told us about the service? The AQAA told us the home does the following to incorporate the views of people that use the service: We hold annual contract review meetings. We hold fortnightly house meetings, where individuals needs and goals are reviewed. QA checks are completed monthly when the views of the people we support are sought - records are kept in house and available for inspection. We are working on updating PCP and end of life plans to identify new goals. We invite parents/carers to annual meetings and record their contribution. The Annual Quality Assurance Assessment also told us the home made the following changes as a result of listening to people that use the service: Individual time has been set aside to work towards goals, which has enabled people who use our service to access badminton, horse riding, dance, and movement psychotherapy, and a paid paper round. Rota changes have enabled people we support more flexibility in times they access the house or resource centre. What are we going to do as a result of this annual service review? We are not going to change our inspection plan, and will do a key inspection by 22nd October 2010. However we can inspect the service at any time if we have concerns about the quality of the service or the safety of the people using the service.
