There is a car parking area at the front and the side of the building. The building stands on a slight hill, so has three floor levels, which can be accessed with a shaft lift. The home has some shared bedrooms, but the present philosophy of the home is to operate at less than full capacity, thus ensuring that current service users have a single room. Communal space is provided by way of a large lounge, a dining room and a conservatory area. There are also two patio areas, with some furniture.

What the care home does well We reported at the last key inspection undertaken on 02 December 2009 that, `People tell us that they are happy living at the home and that their needs are met. The care staff spend time chatting with people on an individual basis, provide people with activities and are friendly and supportive toward the people that they care for`. What the care home could do better: The inspection undertaken on 02 December 2009 identified three further improvement areas in addition to those addressed within this report: These were to do with the lack of assessments prior to someone moving into the home, inadequate recruitment checks on staff and the unsafe storage of hazardous materials. The manager submitted an improvement plan on 15 April 2010 detailing the action taken to address these issues. Compliance with the remaining requirements made following thekey inspection in December 2009 will be assessed at a later date. Care Homes for Older People Page 3 of 10 What we found: We visited the home on 27 April 2010 to to undertake an unannounced random inspection to assess compliance with three Statutory Requirement Notices issued on 16 February 2010. These were issued following our findings at the last key inspection of the home on 02 December 2009. The visit commenced at 09:45 and lasted for two hours and was undertaken by two inspectors. The registered manager was present throughout the visit. The three statutory requirement notices each had a compliance date of 28 February 2010. The first statutory requirement notice related to care plans and required the home to; (i) Have a system in place to ensure that all the service users care needs are clearly recorded in their care plans. (ii) Have a system in place to ensure that the care plans are sufficiently detailed with clear action plans in place to ensure the identified needs of all the service users are fully met. (iii) Have a system in place to ensure that the care plans provide up to date and accurate information, that is cross referenced and corresponds to information provided in assessments. (iv) Have a system in place to ensure that the care plans are reviewed at least monthly, or when a service users circumstances or needs change and revised where appropriate. During the visit we looked at the care plans and associated files of four people. Since we were last at the home the manager had archived a lot of older material. This made the care plans more accessible and less confusing than when we previously visited. The manager stated that the home had received support from Hampshire Social Services quality improvement officer. The manager had implemented the recommendations of the quality improvement officer in that the care plans had been divided into a current file with current care plans, current risk assessments and daily records. Older information had been stored separately. Care plans appeared to contain relevant information and were in an activities of daily living format, covering all aspects of a persons daily life. The care plans viewed contained individual detail about how peoples needs should be met and how they preferred them to be met. All four care plans looked at were current with evidence that they had either recently been reviewed or re written. Wherever possible people had signed to say they were in agreement with their plan. The plans looked at had all been re written and appeared more personal to the person than at the previous inspection. Peoples names were used throughout the care plan and there was a lot of detail regarding peoples personal care support needs as well as information regarding their likes and dislikes, diet and social information. Care Homes for Older People Page 4 of 10 We were unable to speak to everyone whose care plan that we examined. However we were able to observe that one person was sat on a low pressure cushion as stated in their care plan and they appeared comfortable and cared for. Another persons care plan identified that they were at risk of falls and through observation we noted that measures were in place as described in the care plan to reduce the risk. In discussion with the person they told us that they did not have any complaints and that they were very happy with the care that they received. The only issue that we identified from cross referencing care notes with daily recordings were that staff were not always recording bowel movements as specified in some peoples care plans. This was brought to the attention of the manager who said that he would be discussing this with staff. The evidence viewed and discussions with the registered manager, registered provider, and service users would indicate that the statutory requirement notice in relation to care plans has been complied with. The second Statutory Requirement Notice was in relation to how the home manages medication and required by 28 February 2010 that the home; (i) Have a system in place to ensure that all medication is administered as prescribed. (ii) Have a system in place to ensure that medication administration records are accurately maintained, including a method to indicate the reason for any non administration. (iii) Have a system in place to ensure that all handwritten entries on medication record sheets are checked for accuracy by a second person qualified to do so. (iv) Ensure that the register of controlled drugs meets the Royal Pharmaceutical Society Guidelines, Handling of Medicines in Social Care 2007. We looked at the records of medicines received into the home and given to people, the supplies of medicines, the facilities for the storage and recording of Controlled Drugs and talked to the manager. We found that clear records were kept of all medicines coming into the home. When medicines were given to people by the care staff a record was kept to show that the medicines had been given and if it wasnt given why not. Most of the medication records were printed by the supplying pharmacy. However on one occasion when a new medicine had been prescribed this had been added by the care staff. These were correctly written. From the records and medicines supplies we were able to see that people had received their medicines as prescribed. A new register to record and audit (track) the use of Controlled Drugs which meets to Royal Pharmaceutical Society guidelines, handling of Medicines in Social care 2007, had been obtained. The evidence viewed and discussions with the registered manager would indicate that the Statutory Requirement Notice in respect of medication has been complied with. Care Homes for Older People Page 5 of 10 The third Statutory Requirement Notice related to Regulation 26 visits and reports. Regulation 26 visits should be completed at least monthly by the registered provider and are a quality review of the service provided at the home with a report being completed for the registered manager. The Statutory Requirement notice required that by the 28 February 2010 that there would be a system in place to ensure that the written reports following visits to the home by the provider or their representative under the provision of regulation 26 are prepared and supplied to the registered manager and the Commission. The responsible individual was present during the inspection and spoke with the inspectors. The registered provider discussed how they undertake regulation 26 reports and assess the quality of the service provided at the home. The registered provider lives in an apartment in the home and has daily contact with the home, staff and people who live there. The registered provider clearly knew the people who lived at the home and they her. The registered provider was aware of the service users care needs. The registered provider stated that they dictate daily comments about the home which are passed to the registered manager. The registered manager then collates these into a report which is read back to the registered provider and she then signs to confirm that this is a true reflection of her findings. The Registered Manager separately described the same procedure to the inspectors. We viewed the completed Regulation 26 reports for previous months. The regulation 26 reports had been completed on forms suggested by the Commission and covered all the areas necessary for the registered provider to make an assessment of the service provided. The registered provider had signed the Regulation 26 reports. We noted that the registered provider had been doing these reports much more frequently than is actually required. In discussion she was aware of this and said that she preferred to do them more regularly. The evidence viewed and discussions with the Registered Manager and registered provider would indicate that the statutory requirement in relation to Regulation 26 has been complied with.
