Care Homes for Older People Page 3 of 8 What we found: The reason for this inspection was to assess compliance with the statutory requirement notice dated 27 October 2009. You were required by 10 November to 1. Have a medication policy and procedure in place that conforms to the Royal Pharmaceutical Society guidance - the handling of medicines in social care. The policy and procedure must include the process for receiving medication into the Home and the recording of this. 2. Have a system in place to record the date and quantity of all medication received into the Home, with the record signed and dated by the person responsible for checking in the new medication. 3. Ensure all medication is stored securely including those that require refrigeration. There must be a designated fridge which is lockable and kept locked at all times. 4. Have a system in place to prevent a build up of medication. You must record the quantities of medication carried forward, that also includes a monitoring of these levels, rotation of new and old stock to ensure medication is in date. 5. Ensure you provide clear written guidance to staff for each service users who is prescribed as required (PRN) medication, as to when to administer the medication and any triggers or indicators known that shows the medication is needed. You must maintain records of - date, time, dose, service user name and what medication was administered. 6. Have a system in place that accurately records all medication that is administered, including: medication name, the dosage, the date, the times, refusal or unable to take medication and the signature of the person administering the medication. 7. Have a system in place to ensure the safe disposal of unused or out of date medicines. At this visit we looked at records of medicine receipts, disposals and given to people, medicine supplies and storage, policies and procedures, care plans for giving people their medicines and talked to the manager and provider. The medication policy and procedure had been reviewed. It now includes details of the process for receiving medicines into the home and how to record this plus the process to be followed when medicines are no longer needed and how to safely dispose of them. All staff who handled medicines in the home have read the new procedures and signed to confirm this. Records were being kept of medicines received into the home and of any balance brought forward from one medication cycle to the next. These were signed and dated by the person completing the record. All medicines were stored securely for the protection of people who use the service. This Care Homes for Older People Page 4 of 8 included the refrigerator used to store any items requiring cool storage, which is now fitted with a lock. At the last inspection on 23 September 2009 there was an excess of some medicines. These had been disposed of appropriately and a record of their disposal was seen. When medicines were prescribed to be given only when needed written guidance had been produced for staff describing when the medicines were to be given. In addition clear records were being kept of when these medicines were given to people. Clear records were kept of all medicines given to people. These included the medication name, the dosage, the date and the times that they were given. If any medicines were not given the reason why was clearly documented on the medication administration record chart. There were no gaps or ambiguous entries in these records. We found that the statutory requirement had been met.
