Sunrise Operations Bassett Ltd Assisted Living is a registered nursing home able to provide nursing care and accommodation for up to 75 older people. Care Homes for Older People Page 3 of 8 What we found: The pharmacist inspector visited this service as we had information of a few medicine errors and the Adult safeguarding team had raised concerns. We visited the home to check that medicine management systems are safe and the health and welfare of all the people in the home is protected. The service has good facility to manage medicines well. There is adequate storage space which is kept at the required temperature. Medicines that require more secure storage and additional records by law known as Controlled Drugs are also managed correctly. There are four medicines trolleys which are used during administration rounds. We watched the end of a medicine round and saw that safe practices are used. The records relating to medicine were checked. We saw records of a daily audit for medicines given. This record showed that any concerns picked up by the audit are dealt with and a record kept of the action taken. This means there is a check to ensure medicines are given as prescribed. We saw that when medicines instructions are hand written there is a check by a second person to ensure the accuracy of the directions before commencing the medicine. On one occasion additions to the medicine administration record [MAR] chart was not initialled, dated or checked. This particular record was very confusing and the manager could not explain what had happened. With regards to information in care plans on the use of medicine to be taken only when needed we saw guidance for staff but some of these guidance lacked sufficient information to give clear guidance to staff to be able to make a judgement when to use that medicine. Clear guidance would mean that there is consistency in the use of this medicine. This was pointed out to the lead wellness nurse who said that she would take this on board. This person is new in post. Similarly the self administration documentation seen did not incorporate a risk assessment and procedure of the exact method to manage the self administration. Training records of staff authorised to give medicines were available. We asked what the staff thought could be the reason for the errors noted. We were informed that there is a high turnover of staff and communication could be the root cause for the errors. Evidence seen suggests this to be the case.
