The Gables Nursing Home is an older building with character and retains some of its original features. The home is situated on a main road close to local amenities. The home provides nursing care for twenty-two service users. The staff team consists of qualified nurses, carers and ancillary staff with a manager, who is a suitably qualified nurse. The home has access to a large well-kept garden, which is situated approximately five minutes away at Mandeville Grange, which is also owned by the proprietors. What the care home does well Residents told us that they were happy in the home and that staff were kind to them. The activities coordinator helps people to engage in everyday activities. On the day of the unannounced visit residents were enjoying a chat in the lounge, knitting, reading and joining in a quiz. The home has good relationships with the local general practitioner who visits weekly. There was evidence in people`s care plans that they saw other healthcare professionals regularly, including a chiropodist and optician. Everyone had a care plan which described their main care needs and which had been updated regularly. A copy of the complaints procedure is displayed in the hallway. Residents told us that their concerns were listened to. Complaints records are kept. These showed that the home had received one complaint in the last six months which had been addressed by reviewing the resident`s care with the care manager and making the changes needed. All staff with the exception of two had had training in safeguarding procedures, although some staff training needed to be updated. We are not aware of any complaints or safeguarding referrals made to the local authority which is the lead agency in these matters. There is a stable management and staff team. The manager is an experienced nurse who has registered with us. There is an ongoing maintenance programme and the annual quality assurance assessment provided by the proprietor in December 2009 showed that maintenance of essential services was up to date. The Environmental Health Officer visited in January 2010 and the home was awarded 4 stars for food hygiene. A fire safety enforcement notice issued in April 2009 was addressed and a subsequent inspection in June 2009 found all matters to be satisfactory. A requirement, made at the last inspection, that people did not share hoist slings or sliding sheets to reduce the risk of cross infection has been met. A requirement was also made that the proprietor undertakes a regular quality assurance visits to the home as is required and leaves a report of her visit . This has now been met. What the care home could do better: There is a need to improve the management of medication. The medication administration records were checked and a number had gaps in the records, some handwritten entries were not signed by two people and not all had photographs of the resident. There has not been a recent internal audit of medication as part of the qualityassurance procedures or pharmacy audit. This must be addressed to ensure residents receive their medication regularly and safely. The quality assurance procedures should be developed. There are no regular audits of care plans, medication or other aspects of the service offered to ensure that the service is systematically monitored and improved. An internal quality monitoring system was in place until November 2009. The manager said that this was to be replaced but that it had not been decided how and what with as yet. This should be agreed and implemented. Care Homes for Older People Page 3 of 9 What we found: The focus of this unannounced inspection was to ensure that the home was being well managed, that peoples health and personal care needs were being met and that the home had met the requirements of the previous inspection. We found that there was a stable management and staff team and that the home was run in the interests of the residents. In general peoples health care needs were met although some improvements were needed in the management of medication. The requirements made following the last inspection had been met.
