We visited this home to check whether the unsafe practice of secondary dispensing for medicines had stopped and we found that this practice was no longer being used. We looked at storage arrangements and the records for controlled drugs which are medicines that need extra controls as they could be misused. We noted that two recommendations made after the last inspection had not been acted upon. Firstly, we noted that temazepam [a controlled drug] records are not kept in a controlled drugs register style but steps had been taken to receive this medicine in a format [MDS] so missing medicine would be seen at a glance and monthly audits are maintained on the Medicine administartion record [MAR] charts on stock levels in the home. Secondly, we noted several examples where hand written additions to the MAR chart had not been initialed and dated and we saw hand written charts where there was no evidence that a second person had checked the accuracy before comencing the use of the charts. This is needed so that there is accountability of actions taken by staff and any transcribing error is checked for before medicine is commenced to reduce potential harm to residents. Training in the home is cascaded by the senior person who receives external training. Policies and procedures are available which are local and individual care plans are available for any self administration and for any medicine to be taken only when needed. These care plans did not cover the safety of medicines while in the care of the residents both to themselves and other residents in the home.
