The Tobias Centre is a care home providing personal care and accommodation for up to eleven people, aged over 18, who have autistic spectrum disorders and may have additional sensory impairments. The home presently caters for six people with complex needs who require high staffing levels. The home is owned by Modus Care Limited, which owns other care homes and hospitals in Devon and Surrey. Fee levels vary depending on the individual needs of service users but presently range from #1876 to #4317 per week. The Tobias Centre has been a care home for several years and is comprised of a large detached three-storey house in a residential area of Torquay. The home is within walking distance of local shops, the centre of Torquay is accessible by public transport, and the home has its own vehicles.Annual Service Review All the bedrooms are single and are located on each floor. Three of the bedrooms have en suite toilets with showers and one bedroom has an en suite toilet. There are also showers, baths and toilets on each floor, close to bedrooms and communal rooms. The basement has been converted into two one-bedroom flats, each of which has its own bathroom, and they each have an enclosed part of the garden for the personal use of the people in the flats. There is a lounge and dining/activity room on the ground floor as well as quiet rooms used by specified people who live in the home. The home has an enclosed garden at the front and side of the premises. The home has its own car park and on-street parking is also available nearby. Annual Service Review Page 3 of 6 Service update since the last key inspection or annual service review: What did we do for this annual service review? We looked at all the information we have received or asked for since the last key inspection. This included: the Annual Quality Assurance assessment (AQAA) that was sent to us by the service. The AQAA is a self assessment tool that focuses on how well outcomes are being met for people using the service. It also gives us some statistical and numerical information about the service. Surveys were returned to us by people using the service and from other people with an interest in the service; Information we have received about how the service has managed any complaints and what the service has told us about things that have happened, called notifications which are a legal requirement, which had been received during the past year were also examined; We looked at the previous key inspection and the results of any other visits that we have made to the service in the last 12 months, and any other communication we have had from them. What has this told us about the service? The home sent us the Annual quality assurance document which gave us information about how the home is being run and what improvements they have made since their last inspection. They told us that they have made some improvements following consultation with people living at the home, including decoration; increased one persons home visits and plan to provide en suite facilities in the future to meet peoples specific needs. The home tell us they have also had increased input from speech and language therapy services and improved the care planning transition process when people move into the home; ensured people have more choices in activities; some staff have had training in visual and hearing impairment; improved recording systems to ensure people have choices and encouraged people to have more of a say in the furnishing of the home; two individual services have been provided to meet specific needs; improved the percentage of staff with National Vocational Qualifications; provided a full time training and development officer within the company; and put a suggestion box in place for people to make comments on the service. There is an annual development plan in place, developed from audits and a new daily structure is being developed to allow people more choices. They have also told us they plan to Improve the format of information available to people about the home; plan to ensure all staff have access to training in visual and hearing impairment and communication skills; continue with the redecoration and refurbishment of the premises; develop a staff appraisal system; and try to improve communication within the unit. Annual Service Review Page 4 of 6 In the last year the home have told us about two significant incidents where there have been problems with medication administration or supply. The home undertook an investigation in both instances and have implemented changes to minimise any further risk of problems. They sent us a copy of their new policy regarding medication which was seen by the pharmacist Inspector employed by CQC. He made suggestions to improve the policy and ensure it was compliant with best practice and legislation. The home have told us they have received two complaints in the last 12 months, which were resolved within 28 days following being upheld. We received three questionnaires from people who work at the home, one from a healthcare professional who supports the service and two questionnaires which the home developed themselves for people who live at the service to complete. People said the home Handled challenging and challenged clients well, Deals well with physical behaviours and anxieties of the service users and used a variety of communication methods within the unit to support people. People also said that there were choices available for people to do what they wished. One person living at the home confirmed this for the day but felt they didnt have much choice in the evening. Some staff felt that communication could be improved at times, and that maintenance issues were still taking a long time to resolve. The home keeps in contact with us lets us know of changes and developments they are making. What are we going to do as a result of this annual service review? We will plan to carry out a key Inspection before 15th July 2010. However we can visit the service at any time if we have concerns about the people living there.
