The Watlington & District Care Home is a purpose-built 60-bed care facility, providing nursing care to older people who are physically or mentally frail. The ground floor has 30 single rooms, a dining room and a sitting room overlooking the front of the home, and a quiet sitting room and a sun lounge to the rear of the home with access to anAnnual Service Review enclosed courtyard garden with a large raised centre flower/herb bed and paved pathways to a further large garden area. The first floor, with a further 30 single rooms, includes a designated area for the care of a maximum of ten service users requiring intensive short-term rehabilitative intermediate care. The local primary care trust works in partnership with the home to provide physiotherapy and occupational therapy for service users receiving intermediate care. Service users admitted for intermediate care may be in a younger age range (from age 18 years and over). There is a large sitting room/activities lounge and dining room. A third floor is used for staff accommodation. There are passenger lifts and stairs accessing residents` accommodation. The home has a range of assisted bath and shower facilities and two treatment rooms. All residents` rooms have an en-suite toilet and washbasin. The home`s kitchen and laundry facilities are in refurbished and refitted former outbuildings across the approach drive to the home`s main front entrance. Annual Service Review Page 3 of 7 Service update since the last key inspection or annual service review: What did we do for this annual service review? We looked at all the information we have received, or asked for, since the last key inspection. This included the annual quality assurance assessment (AQAA) that was sent to us by the service. The AQAA is a self assessment that focuses on how well people who live here feel their care and support needs are met. It also gave us some numerical information about the service, for example, the number of staff trained to the nationally recognised standard - National Vocational Qualification in Care or Health and Social Care. We have looked at what the service has told us about things that have happened in the home in the past 12 months - these are called notifications that the home has to report to us by law. The previous report and AQAA for the home were reviewed, and AQAA record of any complaints they have received and how these were dealt with. We took into account the comments we received in the sample of our questionnaires (surveys) that we sent to residents, visiting health and social care professionals and the homes staff in September 2009. We received 15 surveys from residents, 10 from relatives, carers and advocates, 9 from staff members, and 4 from health and social care professionals. We phoned the manager to let her know the kinds of things people had put in their surveys, and discussed with her how the home and provider organisation, Sanctuary Care Ltd, gets peoples views and acts on them. What has this told us about the service? The home sent us their AQAA when we asked for it. It was clearly written and gave us a satisfactory amount of detailed information we required. It showed us how the home continues to develop and meet the individual needs of people who live here. The AQAA shows that the managers have identified ways in which some things could be improved and how they plan to do this in the year ahead. Survey comments received were largely very positive about the home, especially the staff, environment, meals, and social life. Some of the comments made were - They really care about people. I feel when I leave my husband, he is well looked after. Helpful, friendly and caring. Good food, well prepared. Everything very clean and tidy. it is a pleasure to visit such a lovely caring home. The care and thoughtfulness with respect for dignity of the person. The carers are very warm and seem to build up a good relationship with their patients. Staff and visiting health and social care professionals survey comments reflected good Annual Service Review Page 4 of 7 team working, a cheerful and helpful staff team, a good staff training programme and supportive managers. Some staff survey responses indicated that the home could be better by improving the way in which staff communicate with each other (more staff meetings); providing more activities for residents (and involve families and volunteers in this); and enlarging the space available for residents who are more physically active (in the dementia unit). The manager and AQAA tell us that there are regular staff meetings, a low staff turnover and an increasing proportion of care staff who have achieved the NVQ Level 2 qualification, or above. The home aims to increase the number of care staff with NVQ Level 3 in the coming year. The company has its own established system for quality assurance and getting the views of the people who live here. The information available to the Commission indicates that the service continues to provide good outcomes for residents and their advocates and that the home is run in the best interest of the people living here. The AQAA information (June 2009) lists a range of improvements that have been made, and are planned, as a result of listening to people who live here. These include re-fitting the kitchen area in the Intermediate care unit to help assess people during their rehabilitation, and improving the front garden to provide better access for people who use wheelchairs. The large greenhouse provides more opportunities for people to do outdoor activities and gardening. New digital TVs with built-in DVD players have been bought for the home to improve the range of programmes and film entertainment available to residents. The home plans to make further changes in improving the care of people living with dementia through piloting a computer interactive reminiscence programme to stimulate and support communication between care givers and residents. The home continues to develop its person-centred approach to care, supported by care plans and records that are user-friendly so that residents and their families can be actively involved in updating them where possible. Previous inspections and AQAAs indicate that residents and relatives have confidence in the homes complaints procedure. The home stated in its AQAA that since our last inspection it had addressed two complaints, neither if which was upheld. The Commission has not received any complaints about the service. The Commission is not aware of any current Local Authority safeguarding adults referrals. The provider organisation (SanctuaryCare Ltd) for the home told us that the current manager is in the process of applying to the Commission become the registered manager for the service. All registered services must have a registered manager and so it is important that the manager is assessed, approved and registered by us in order to meet the legal requirements as soon as possible. What are we going to do as a result of this annual service review? We are not going to change our inspection plan and will do a key inspection by 13.11.2010. However, we can inspect the service at any time if we have concerns about the quality of the service of the safety of the people using the service.
