What the care home does well

Cherry Tree House provides a pleasant, homely environment for the people who live there. `I have all I need`, `I enjoy the garden` and `the owner is kind and helpful` were some of the comments people made about the service. People receive individualised care and support to meet their needs. Health needs are recorded and people see the GP and other health professionals as required. People do the things they want during the day. Visitors are welcome.

What has improved since the last inspection?

Records are kept of hot water temperatures, showing that people are protected from harm. The portable electrical appliances have been tested, again protecting people who use the service and visitors from harm. The owner and manager have completed training in lifting and handling, protection of vulnerable adults, fire safety, food hygiene and infection control.

What the care home could do better:

Ensure care plans are up to date so peoples needs can be fully met. Repair or replace the lock on one medication cabinet to ensure medication is securely stored.

The home is situated in a quiet residential area, with easy access to shops, medical centre and public transport links to larger local shopping areas. Accommodation is provided over two floors, with single bedrooms, a laundry area, lounge and kitchen/ dining area. A well-maintained garden is available to the rear and front of the property. Fees for the service are from £410 per week. Information about the CSCI is available at the service. This means people who use this service experience good quality outcomes. This unannounced inspection took place over two and a half hours on the 23rd July and two and a half hours on the 31st July 2008. One regulation inspector visited, looked at records, spoke with people who use the service, the owner and manager. The manager completed an Annual Quality Assurance Assessment AQAA in the given timescales, which included good information about the service. No service user moves into the home without having had his/her needs assessed and been assured that these will be met. Service users and their representatives know that the home they enter will meet their needs. Prospective service users and their relatives and friends have an opportunity to visit and assess the quality, facilities and suitability of the home. Service users assessed and referred solely for intermediate care are helped to maximise their independence and return home. JUDGEMENT – we looked at outcomes for the following standard(s): 1, 3 and 6 People who use this service receive good quality in this outcome area. This judgement has been made using available evidence including a visit to this service. The home understands the importance of having sufficient information when choosing a care home. A Statement of Purpose and Service Users Guide have been developed to help people make the decision to move in. Assessments are completed before admission and during the ‘taster day’, to ensure that the home is the best place for an individual. Intermediate care is not provided. EVIDENCE: A Statement of Purpose and Service Users Guide have been developed to tell people about the services provided, facilities available and the owner and manager. The manager said she would do an assessment before someone moved in. They would also have a ‘taster day’, when the person is invited to spend time at the home, meeting people who live there and the owner and manager. The service user’s health, personal and social care needs are set out in an individual plan of care. Service users’ health care needs are fully met. Service users, where appropriate, are responsible for their own medication, and are protected by the home’s policies and procedures for dealing with medicines. Service users feel they are treated with respect and their right to privacy is upheld. Service users are assured that at the time of their death, staff will treat them and their family with care, sensitivity and respect. The Commission considers Standards 7, 8, 9 and 10 the key standards to be inspected. JUDGEMENT – we looked at outcomes for the following standard(s): 7, 8, 9 and 10 People who use this service receive good quality in this outcome area. This judgement has been made using available evidence including a visit to this service. Personal support is responsive to individuals needs and preferences. People’s privacy and dignity are respected. People have access to healthcare services in the home. Medication is well managed with records signed and almost up to date. EVIDENCE: The manager said they ensure that people who use the service are happy and healthy by giving them one to one care in a private and individual setting. People who use the service confirmed that they receive the care and support they need, that their privacy and dignity is maintained. We saw staff offer people support and assistance in a caring, discreet and appropriate manner to meet individual’s needs. We saw care plans have been developed from assessments with people who use the service. The manager said they are developing a review process to ensure that all people who use the service have at least an annual review of the care and support they receive. We saw information in one case file need updating to include recent changes to the persons medication. The manager said that this would be updated. The manager said that there are only three people involved in this persons care and all are aware of the changes due to the regular discussions and handovers on a daily basis. A new system for daily recording has been put in place, which includes detailing the care and support provided as well as health appointments, visitors and activities. People’s health care needs are recorded and any issues are referred to the GP or other relevant health professional. The GP visits when required and carries out regular medication reviews. One person said their health needs are well met. Appropriate medication policies and procedures are in place. Medication is securely stored, although the lock on one cabinet did not close easily and should be repaired or replaced. Medication Administration Record (MAR) Sheets were up to date and signed with one exception. The manager said this was because a new recording system was being developed. This was updated with a new MAR Sheet in place on our second visit. Service users maintain contact with family/ friends/ representatives and the local community as they wish. Service users are helped to exercise choice and control over their lives. Service users receive a wholesome appealing balanced diet in pleasing surroundings at times convenient to them. The Commission considers all of the above key standards to be inspected. JUDGEMENT – we looked at outcomes for the following standard(s): 12, 13, 14 and 15 People who use this service receive good quality in this outcome area. This judgement has been made using available evidence including a visit to this service. People who use the service can take part in a few activities and outings, depending on their choice and wishes. People have the opportunity to maintain important family and personal relationships. The menu is varied and meets people’s cultural, religious and medical needs. EVIDENCE: We saw people spending time in their room and in the lounge, talking with the owner and manager, reading the paper, looking at magazines, watching television and sitting in the garden during our visits. People told us that they do the things they want to. The manager said they welcome visitors and encourage family and friends of people who use the service to be involved in daily life at the home. We saw visitors made welcome and people who use the service enjoy their company. One person told us that their visitors are very important to them. We saw one mealtime to be an unhurried social occasion with people sitting at the dining table for main course and desert and having a cup of tea at the end of the meal. People who use the service made positive comments about the quantity and quality of food they receive. This judgement has been made using available evidence including a visit to this service. The service has a complaints procedure that is clearly written and easy to understand. Information about how to make a complaint is available to people who use the service and their representatives. People are aware of how to make a complaint. There are appropriate policies and procedures for safeguarding adults. The owner and manager have completed training in protection. EVIDENCE: The manager said people who use the service get a copy of the complaints procedure when they first move in. One person was aware of how to make a complaint although was clear that they had no issues or worries. The service has not received any complaints since the last inspection in December 2007. We have not received any complaints or concerns since the last inspection. The manager and owner have completed training in safeguarding and are aware of the issues that need reporting to the local social services department. The manager said that they do not hold any finances for people who use the service. JUDGEMENT – we looked at outcomes for the following standard(s): 19, 20, 21, 24 and 26 People who use this service receive good quality in this outcome area. This judgement has been made using available evidence including a visit to this service. The home is appropriate to meet the needs of the people who live there. The well maintained, safe environment provides aids and adaptations to meet people’s needs. Bedrooms have been personalised to the individuals taste. The home is well lit, clean and smells fresh. EVIDENCE: The lounge/dining room has appropriate comfortable chairs and table and chairs for mealtimes and has doors to the garden. We saw people enjoying the view of the garden and sitting outside. Bedrooms are single, we saw that people have personalised their rooms with photographs, pictures and belongings. One person said they like their room and enjoy sitting in it watching television. All areas are maintained and decorated to a high standard. All areas of the home were clean and smelt fresh. This judgement has been made using available evidence including a visit to this service. People have confidence in the staff who care for them. Staffing levels were seen to be sufficient to meet the needs of people who use the service. The owner and manager provide most care and support with no staff employed. The owner and manager have attended appropriate training courses. EVIDENCE: The owner and manager provide most care and support with a close friend helping to cover a few hours during the week. Staff are not employed at the home. The manager said they have developed recruitment policies although are not in the process of recruiting staff at this time. People who use the service made positive comments about the owner and manager, saying they are ‘kind’ and ‘helpful’. The manager said she and the owner have regular daily discussions about individuals needs, although a formal staff handover does not take place. This remains appropriate due to the small size of the home. The manager and owner have experience in working in similar services and are aware of the standards to be achieved to provide and maintain a good service. People who use the service are involved in the day to running of the home and their needs and wishes are taken into consideration for planning for the future of the service. Clear health and safety policies and practices are in place to protect people who use the service from harm.
