We visited Eldonian House to review how residents needs are being met in the continued absence of the registered manager, who is unwell. Nobody knew that we were coming and the acting manager was on leave when we arrived. We looked at all of the shared areas of the home, including both lounges and the dining room. We talked to all of the care staff who were on duty and some of the residents. We checked a sample of medication records. The home was clean and fresh, with no trace of unpleasant odours. Members of care staff were talking to residents and planning some activities, in particular a trip to church. Staff told us about recent activities such as a visit to a tea dance, and plans for future activities such as basket making for Easter. We were unable to look at some care plans as the acting manager had taken them home to work on. Care plans and other essential details should always be available on the premises. We were told that there is no programme of one to one supervision sessions for staff. Medication was in order and a prescribed food supplement had been put on the Medication Administration Record (MAR) sheets as required by a recent Social Services investigation. However, there was no entry by some doses so it was unclear from the record if the supplement had been given on that day. Each medication episode needs to have an entry to explain if it has been administered or if not, why not. The senior carer on duty had not received any special training in the administration of medication though she had been shown what to do by senior staff. All staff who administer medication should be given appropriate medication training. A medicine that had been prescribed to be give as required (PRN) had no written explanation as to the circumstances in which it was to be give. The member of staff did know this information but it should be recorded.
