The home is a large detached property situated in a residential area and directly opposite an attractive park. The home is close to local shops, bus stops and local amenities. Chatham and Gillingham town centres are approximately one mile away. Accommodation is provided on three floors, the upper floors are accessible by chairlifts. All bedrooms have a call bell point. The home has a large garden, which is accessible for all residents. This Random Inspection process culminated in a site visit to the service on the 8th June 2010. The site visit commenced at 14:00 hours and concluded at 17:00 hours. Prior to the site visit a range of information was reviewed including the Annual Quality Assurance Assessment completed by the service, notifications of any untoward occurrences that the home has informed us about, details of any concerns, complaints or safeguarding alerts raised in relation to the service and any other information that we have received from the home or other people with an interest in the service. This random inspection focused on outstanding requirements made at the previous key inspection to ascertain compliance. During the course of the site visit discussions were held with the manager, staff on duty and service users. A short tour of the premises was conducted. A range of documentation was examined including information relating to complaints and concerns, the management of adult protection and abuse awareness, staff training and supervision, recruitment, quality assurance and health and safety issues. What the care home does well:
The service has taken steps to address all of the requirements made at the previous key inspection conducted on 14th July 2009. Service users told us that they enjoy living in the home, that they have a comfortable environment and friendly atmosphere. They said that the care staff are respectful of their needs and preferences and that they are treated with dignity. One person went on to say, there always seems to be something going on, we had a theatre company visit us this week. It was very entertaining. The manager has reviewed the complaints process. A copy of the policy is on display in the home and complaint forms are readily available in the hallway of the service. The manager stated that there have been no complaints raised since the last key inspection. He was able to demonstrate that if any complaints are made the issues would be investigated, outcomes recorded and feedback given to the complainant. All care staff have now received training covering Adult Protection and abuse awareness issues. This means that staff are able to recognise and take appropriate action if they are concerned that any issues of abuse may have occurred. Polices and procedures are in place relating to issues of abuse and staff are asked to read through and sign this policy as part of the induction process. The home has improved recruitment procedures ensuring that all of the required checks are completed prior to a member of care staff commencing work in the home. Three staff personnel files were examined for the most recent employees of the home and in all cases the required information was on file including CRB/ISA checks, proof of identity and two written references. In each case there was a full employment history. Care Homes for Older People Page 4 of 9 Progress has been made with regard to staff training. The manager has now produced an up to date training matrix that demonstrates the vast majority of staff have received suitable levels of mandatory training. The manager stated that he had previously used training videos to comply with this standard, but has recognised that these videos are suitable for induction purposes only. Care staff have received up to date training through external providers and where gaps remain in the required training there is evidence that courses have been booked and planned to address this. Care staff now receive supervision from a senior member of staff on a regular basis. Some staff files were randomly selected showing that completed supervision forms were on file. Care staff have also received annual appraisals clarifying strengths and weaknesses and setting objectives for the future. Staff spoken to stated that they feel well supported in their roles. A system of Quality Assurance has been introduced. The manager is also the owner of the home and therefore is in day-to-day control of the service. This means that he is able to monitor all aspects of the home and ensure on-going improvement and development. He has also introduced annual surveys for service users and relatives/visitors. Examples of these were examined and in the main provided very positive feedback about the service. A range of records were examined relating to health and safety issues. Records of fire safety were examined and have been kept up to date. the manager has appointed a responsible person to undertake this task and they have received the appropriate training to perform this role. Regular fire drills now take place and it is planned that all staff will take part in these drills throughout the year. Staff have received fire safety training. Other records examined included service safety certificates such as gas safety, electrical wiring, equipment checks and other related information. All of this information was seen to be up to date and in good working order. What they could do better:
The service has taken appropriate steps to address the outstanding requirements and no further requirements under the Care Standards Act 2000 were made. The manager is advised to provide suitable instruction to staff covering the Mental Capacity Act and the Deprivation of Liberty Safeguards to ensure that the key principles underpinning the Acts are applied within the service on a consistent basis. Significant progress has been made in respect of staff training needs, but the manager should continue to provide structured and suitable training covering all mandatory topics for staff. To ensure that all new employees have completed a full employment history the manager was advised to include this as part of the interview question pro-forma. It is recommended that quality assurance systems are further to developed to include surveys of staff and key professionals. The manager should also collate the results of all surveys to formulate an annual report summarising the perceived strengths and weaknesses of the service and demonstrating what action has been taken to address any issues raised.
