Conditions of registration: Date of last inspection 28/06/05 Brief Description of the Service: Curlew Close is a detached property on a housing estate on the outskirts of Tewksbury. The accommodation is arranged over two floors, with one bedroom being on the ground floor. This bedroom has an en-suite bathroom. The home provides care and support for four adults with Learning Disabilities and is owned and staffed by the national Mencap Society. 1 Curlew Close DS0000016309.V272981.R01.S.doc Version 5.0 Page 5 SUMMARY
This is an overview of what the inspector found during the inspection. This unannounced inspection took place over 2 hours on 22nd December 2005. The home was previously visited on 28th June 2005 when a fuller inspection was completed. The inspector met with one staff member and one service user and examined records relating to medication, care planning, Health and Safety and staff and service user meetings. An inspection of the environment was also carried out. The inspector wishes to thank the staff member and service user for their cooperation and open approach to the inspection. Prospective users’ individual aspirations and needs are assessed. Prospective service users know that the home that they will choose will meet their needs and aspirations. Prospective service users have an opportunity to visit and to “test drive” the home. Each service user has an individual written contract or statement of terms and conditions with the home. The Commission consider Standard 2 the key standard to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): None of these standard were inspected during this visit. The intended outcomes for Standards 6 – 10 are: 6. 7. 8. 9. 10. Service users know their assessed and changing needs and personal goals are reflected in their individual Plan. Service users make decisions about their lives with assistance as needed. Service users are consulted on, and participate in, all aspects of life in the home. Service users are supported to take risks as part of an independent lifestyle. Service users know that information about them is handled appropriately, and that their confidences are kept. The Commission considers Standards 6, 7 and 9 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 6,7,9 & 10 The care planning is person centred and identifies individual needs and goals. The staff are committed to ensuring service users can make informed and independent decisions about their lives. EVIDENCE: All service users have care plans in place, which are reviewed appropriately with the involvement of the service users. Service users also have Individual Personal Planning meetings, which are recorded and identify clear goals and objectives. The staff team have greatly improved this area of work over the previous 12 months. The sample examined evidenced clear recording of information and of staff working with, and supporting, service users. Two of the service users have advocate involvement, which has been organised by their key-workers. This is to ensure that their choices and preferences over certain important issues are identified and that people are supported to make their own decisions.

The process is being used positively to support increased independence for one person and ensure that skills are developed which will minimise any risks. Service users are able to take part in age, peer and culturally appropriate activities. Service users are part of the local community. Service users engage in appropriate leisure activities. Service users have appropriate personal, family and sexual relationships. Service users’ rights are respected and responsibilities recognised in their daily lives. Service users are offered a healthy diet and enjoy their meals and mealtimes. The Commission considers Standards 12, 13, 15, 16 and 17 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 11,12 & 15 The home supports the service users to lead lifestyles that are based on their assessed needs and identified wishes and aspirations. EVIDENCE: The home has continued to develop and support one person’s wish to live more independently. This is being approached in a structured and planned way and it is evident that this has resulted in positive outcomes for the service user’s confidence and lifestyle. They are now able to attend a number of local activities independently and also visit the local town. Employment experience is now being investigated as a possible next step. Records show that people are supported to attend a variety activities in the evenings and at the weekends. There have been two holidays provided this year, as well as trips to local pubs, social clubs and a Barn Dance. Two theatre trips are booked for the Christmas period. The home also has organised a monthly music session that takes place in the house and is attended by all the service users.
1 Curlew Close DS0000016309.V272981.R01.S.doc Version 5.0 Page 12 The staff member on duty described how they were considering some alterations to the furnishings and furniture in one person’s room to see if this increased their use of the bedroom. Service users are supported to maintain personal relationships and are provided with opportunities for developing friendships. Service users take responsibility for domestic tasks as far as their abilities allow. On the day of the inspection one service user was being supported to prepare their meals and had also completed some other chores around the home. If requested service users can have keys to their rooms, and one person presently has a key to their bedroom and to the home. 1 Curlew Close DS0000016309.V272981.R01.S.doc Version 5.0 Page 13 Personal and Healthcare Support
The intended outcomes for Standards 18 - 21 are: 18. 19. 20. 21. Service users receive personal support in the way they prefer and require. Service users’ physical and emotional health needs are met. Service users retain, administer and control their own medication where appropriate, and are protected by the home’s policies and procedures for dealing with medicines. The ageing, illness and death of a service user are handled with respect and as the individual would wish. The Commission considers Standards 18, 19, and 20 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 18,19 & 20 The home has effective systems in place for monitoring of health needs and involving outside professionals as and when required. EVIDENCE: The care plans detail the personal care that is required and how this should be provided. The home has arranged for some new bathroom equipment to be provided in the en-suite bathroom of the downstairs bedroom. This will help to meet the needs of the individual service user, and help ensure that the home can continue to meet these needs for the foreseeable future. The files showed detailed recording of health appointments and check ups for all service users. There was evidence of staff responding to concerns identified about health needs and arranging contact with the appropriate professionals. On the morning of the inspection one person had been taken for some tests following observations by the staff. This visit had been written up with clear information about the outcome and what the next steps should be.

The medication storage and administration were examined and found to be in order apart from one minor error. One person is currently having medication that cannot be supplied in the MDS format and this has been provided in packets. Service users’ bedrooms promote their independence. Service users’ toilets and bathrooms provide sufficient privacy and meet their individual needs. Shared spaces complement and supplement service users’ individual rooms. Service users have the specialist equipment they require to maximise their independence. The home is clean and hygienic. The Commission considers Standards 24, and 30 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 24,25,26,27,28,29 & 30 The home provides comfortable and homely accommodation that is well maintained and decorated to a good standard throughout. EVIDENCE: All of the communal areas have recently been decorated and the home appeared clean, hygienic and well maintained throughout. Service users were involved in choosing the colours. A letter rack has been fitted in the hallway with individual sections for each service user. Three of the four bedrooms were seen and these were personalised and met the individual needs of the service users. A staff member explained how they are investigating making alterations to one bedroom to see whether the person can be supported to make more use of their personal space. Paper towels have now been provided in the bathroom to improve the measures preventing cross infection.

The hot tap in the upstairs bathroom was not working but it was explained that it had only been mended that week and the maintenance had been contacted to return and make further repairs. 1 Curlew Close DS0000016309.V272981.R01.S.doc Version 5.0 Page 18 Staffing

JUDGEMENT – we looked at outcomes for the following standard(s): These standards were not inspected in detail during this visit. EVIDENCE: The home has been having regular staff meetings and these show that a variety of issues are discussed and information is distributed through the staff team. The staff member who supported the inspection was able to demonstrate a professional knowledge of the service users needs and the various actions and issues that are currently relevant within the home. They stated that the staff worked well as a team and were very well supported by the manager. JUDGEMENT – we looked at outcomes for the following standard(s): 42 The home has good systems in place for monitoring health and safety that help promote and maintain a safe environment. EVIDENCE: The home has registered its manager since the previous inspection but they were not on duty at the time this unannounced visit. Whilst these standards were not inspected during this visit, the home appears to be well organised, with the staff team having a clear sense of direction. All health and safety monitoring records were up to date. All fire safety checks have been completed and equipment has been tested and serviced.
