The inspector made no statutory requirements on the home as a result of this inspection and there were no outstanding actions from the previous inspection report.

What the care home does well

The home provides care to service users in pleasant surroundings and care and support is given to enable service users to live fulfilling and meaningful lives. Service users are given choice in their day-to-day lives with appropriate support provided by staff at the home. There is an effective care planning system in place and each service users has a key worker who assists individual service users to be actively involved in this process. Service users are supported to access the local community and to undertake leisure pursuits of their choice and all service users attend the local day service, which is run by Hampshire Autistic Society.

What has improved since the last inspection?

Policies and procedures have been updated and requirements and recommendations made at the last inspection have been addressed. 2 service users rooms have been decorated and some decoration has taken place in the communal areas and service users have been involved in the choice of colours.

What the care home could do better:

The inspection report will make 4 requirements the home, which will help improve the service provided for residents. It was noted that care plans are formally reviewed at six monthly intervals but there was no evidence of regular monthly reviews of care plans taking place and it is a requirement that clear records must be kept to provide evidence that monthly reviews have taken place. The Gas Safety certificate expired in January 2005 and it is arequirement that gas appliances at the home must be inspected by an appropriate gas engineer and a safety certificate obtained. There was no record of private electrical equipment at the home being tested and it is a requirement that all private electrical equipment must be tested by a person qualified to do so and a record of these tests be kept at the home. The home`s staffing rota showed that the deputy manager was not being allocated sufficient management time and it is a requirement that managers at the service are allocated sufficient time away from hands on work to carry out management duties. Hampshire Autistic Society is currently reviewing and addressing the need for clear information for service users on the charges and facilities which are provided in the service users fees. It was not possible to assess if the requirement that information and documents required to be kept at the home regarding those staff employed had been addressed as this standard was not inspected on this occasion, but will be looked at when the next inspection takes place.

All service users must be at least 18 years of age. Date of last inspection Brief Description of the Service: 102B Brockhurst road is a semi-detached property situated up a short drive and is close to local shops and amenities, the home is close to a local bus route, which runs into the town centre of Gosport and also into Fareham town centre. The home is run by the Hampshire Autistic Society (HAS) and is registered with the Commission for Social Care Inspection (CSCI) to provide support and accommodation for four younger adults who have a Learning Disability. The home was purpose built approximately 2 years ago and meets all of the national minimum standards with regard to the physical environment. 102B Brockhurst Road H54 S29248 102B Brockhurst Road V239010 190705.doc Version 1.40 Page 5 SUMMARY This is an overview of what the inspector found during the inspection. An unannounced inspection was carried out on the 19 July 2005. The homes manager was not available at the time of the inspection therefore the inspector was assisted by care staff at the home and also the deputy manager. The inspection was carried out over 3 hours and during the inspection it was possible to tour the premises and to view records. At the time of the inspection there were only 2 service users living at the home and the inspector had the opportunity to speak with one of the service users and it was also possible to speak with 2 members of staff who were on duty and also the deputy manager. Each service user has an individual written contract or statement of terms and conditions with the home. The Commission consider Standard 2 the key standard to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 4 The home invites prospective service users to visit the home on an introductory basis and to “test drive” the home before making a decision to move there. EVIDENCE: One service users spoken to stated that he had visited the home on a number of occasions, staying overnight, having a meal and meeting the homes existing service users before making a decision to move in. Service users know that information about them is handled appropriately, and that their confidences are kept The Commission considers Standards 6, 7 and 9 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 6, 7 & 9 Service users assessed needs and personal goals are reflected in their individual plan of care, however it was a requirement that care plans must be reviewed monthly and a records of monthly reviews kept. Staff respect service users rights to make decisions and staff if needed provides assistance. Service users are supported to take responsible risks and have good information on which to base decisions. EVIDENCE: Care plans were seen for 2 service users and these were comprehensive documents and were clear and easy to follow, they contained all relevant information and formal reviews were carried out at six monthly intervals and review notes were kept on file, however there was no record of regular monthly reviews taking place. There was evidence that service users are supported to make decisions and are consulted on a one to one basis at their 1-1 sessions and also meetings are held for service users on a regular basis with minutes taken. Service user spoken to stated that staff respected decisions made and offered support and assistance in the decision making process. Staff was observed interacting with service users and service users were seen to be consulted and supported to make informed decisions. Service users have opportunities for personal development. Service users are able to take part in age, peer and culturally appropriate activities. Service users are part of the local community. Service users engage in appropriate leisure activities. Service users have appropriate personal, family and sexual relationships. Service users’ rights are respected and responsibilities recognised in their daily lives. Service users are offered a healthy diet and enjoy their meals and mealtimes. The Commission considers Standards 12, 13, 15, 16 and 17 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 11, 12, 15 & 17 Staff enable service users to have opportunities to maintain and develop social, emotional, communication and independent living skills and staff help service users to find and keep appropriate jobs, continue their education or training and take part in valued and fulfilling activities. Service users have appropriate personal, family relationships and service users are offered a healthy diet and meal times are unhurried. EVIDENCE: Service users have one to one sessions with their key worker and care plan files contained a life skills programme, which service users help compile. One service user has accessed a counselling service and current service users regularly attend day service where social skills are developed and this is continued in the home. Service users are supported to continue their education and one service user expressed a wish to gain employment and staff at the home have supported the service user to obtain work experience for him with a local maintenance company. One service user spoken to stated that he was not keen on some activities at day service as they were not challenging 102B Brockhurst Road H54 S29248 102B Brockhurst Road V239010 190705.doc Version 1.40 Page 12 enough and staff have supported him to undertake a furniture restoration course at a local college, although this has now ended but will continue next term. Visitors to the home are welcome at any time and the home has a visiting policy, which gives clear information. There are appropriate policies and procedures in place with regards to sexuality/sexual relationships. Records inspected showed that service users are offered a healthy diet and menus are made up weekly with input from service users. Service users spoken to said that they enjoyed their meals at the home and liked to be involved in the planning and preparation of meals 102B Brockhurst Road H54 S29248 102B Brockhurst Road V239010 190705.doc Version 1.40 Page 13 Personal and Healthcare Support The intended outcomes for Standards 18 - 21 are: 18. 19. 20. 21. Service users receive personal support in the way they prefer and require. Service users’ physical and emotional health needs are met. Service users retain, administer and control their own medication where appropriate, and are protected by the home’s policies and procedures for dealing with medicines. The ageing, illness and death of a service user are handled with respect and as the individual would wish. The Commission considers Standards 18, 19, and 20 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 18 Service users do consistently receive personal care and support in the way they prefer and require EVIDENCE: Staff support service users with personal care in private and the support that is required is documented in individual care plans and the service aims to offer same gender care where possible. The home has a policy on cross gender care and service users are able to choose when to get up and go to bed, when to have a bath, what activities they do and when they do them and they can choose their own clothes and hairstyle with support given as and when required. Service users feel their views are listened to and acted on. Service users are protected from abuse, neglect and self-harm. The Commission considers Standards 22, and 23 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 22 & 23 Service users views are listened to and acted upon and service users are protected from abuse, neglect and self-harm. EVIDENCE: The home has a comprehensive corporate complaints procedure, which includes details of who would investigate complaints together with timescales. The complaints procedure also gave details of how to contact the Commission for Social Care Inspection. There is also a simple to follow “in house” complaints procedure for service users, which has pictures and symbols and this is used in conjunction with the corporate policy. Records kept showed that Service users had made complaints in the past and staff had supported them with this process. One service user spoken to stated that he would speak to a member of staff if he had a complaint and was happy that staff would support him to make a complaint. The home has a copy of the Hampshire Adult Protection procedure and has a whistle blowing policy and a copy of the department of health guideline “No Secrets” staff also receive training with regard to adult protection and POVA as part of their induction. Staff members spoken to were aware of their responsibilities in this area. There was a requirement made at the last inspection for the organisation to put clear terms and conditions in place which gives a breakdown of fees charged so service users can be clear of what is included in the service user’s fees and what is considered as extras that should be paid by individual service users. JUDGEMENT – we looked at outcomes for standard(s) None of these standards were inspected on this occasion. Service users are supported and protected by the home’s recruitment policy and practices. Service users’ individual and joint needs are met by appropriately trained staff. Service users benefit from well supported and supervised staff. The Commission considers Standards 35 the key standard to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 33 There is an effective staff team in sufficient numbers to meet the needs of service users, however one recommendation was made to increase the number of management hours for the deputy manager at the home. EVIDENCE: The home currently has only 2 service users and the staff rota showed that there are sufficient staff on duty to support them, there are 2 staff members on duty between 0730 – 2100 and at night time the home has one staff member who sleeps in and is backed up by an on call system that is able to provide advice and if necessary attend the service to give additional support. The home rota showed that the deputy manager at the home was only allocated 12 hours management time over a four week period consisting of 156 hours, this is not sufficient to enable the deputy to get to know her role, develop her skills and carryout management tasks. It is recommended that sufficient management time be allocated to enable the deputy manager to carry out her role effectively. It was not possible to view recruitment records at this inspection as these were kept in locked cabinets and the care staff did not have access to these files, as they were confidential. It was therefore not possible to establish if the requirement made at the last inspection with regard to staff records being kept at the home has been addressed and this issue will be followed up at the next 102B Brockhurst Road H54 S29248 102B Brockhurst Road V239010 190705.doc Version 1.40 Page 18 inspection. JUDGEMENT – we looked at outcomes for standard(s) 42 The home is aware of its responsibilities with regard to the health, safety and welfare of staff and service users, and these are generally promoted and protected. However requirements were made regarding the Gas Safety Certificate for the home and also for the testing of private electrical equipment. EVIDENCE: The inspector looked at fire records and these were accurate and up to date with regard to training, there were also records of fire alarms test, emergency lighting and fire fighting equipment tests. The Gas Safety Certificate for the home expired in January 2005 and there was no record that Private Electrical equipment in the home had been tested.
