The inspector found there to be outstanding requirements from the previous inspection report but made no statutory requirements on the home.

What the care home does well

The home has very good care plans to make sure that staff can help residents, in the right way. Residents get lots of opportunities to do activities during the daytime. The families and friends of residents are made welcome and residents are helped to keep in close touch with them. Residents have very good food and are encouraged to help to make it.

What has improved since the last inspection?

Staff are now supervised regularly to make sure that they are doing their best for the residents. Residents now have their own bank accounts and can see how much they have got in the bank.

What the care home could do better:

The home could give the residents a contract, which tells them all about the home and what they are paying for. The money in resident`s bank accounts should be properly recorded so that they know what it has been spent on and when it was spent. The home could make sure that there are enough staff to do all the necessary jobs to look after the residents. The home could find a way of showing that it is giving good care to the residents. The home could show that it is keeping the residents safe and all the staff know how to.

This was an unannounced inspection which took place between 3.30 pm and 6pm on the 7th September 2005. The inspector spent time with two long-term staff members and two residents. Each service user has an individual written contract or statement of terms and conditions with the home. The Commission consider Standard 2 the key standard to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 2 and 5 There have been no new admissions to the home since it opened. Service users do not have a contract/statement of terms and conditions. Service users know their assessed and changing needs and personal goals are reflected in their individual Plan. Service users make decisions about their lives with assistance as needed. Service users are consulted on, and participate, in all aspects of life in the home. Service users are supported to take risks as part of an independent lifestyle. Service users know that information about them is handled appropriately, and that their confidences are kept The Commission considers Standards 6, 7 and 9 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 6.7 and 9 Service users have a comprehensive care plan, which is reviewed regularly. Service users are encouraged to make appropriate decisions about their daily lives and have robust risk assessments to support this process. EVIDENCE: The home has a care planning system called E.L.PS (Essential Life Plans) these describe all aspects of the service users care and includes goals and development. These are reviewed regularly and service users are part of the review process. All the care plans were seen to be up-to-date and review dates are set for the rest of this year. Service users attend house meetings with staff to discuss any developments or changes being made and are able to articulate their views. They decide when they participate in activities are involved in the care planning process and make decisions in their daily lives. All necessary areas of care are risk assessed to ensure that service users are encouraged to be as independent as possible. Service users have opportunities for personal development. Service users are able to take part in age, peer and culturally appropriate activities. Service users are part of the local community. Service users engage in appropriate leisure activities. Service users have appropriate personal, family and sexual relationships. Service users’ rights are respected and responsibilities recognised in their daily lives. Service users are offered a healthy diet and enjoy their meals and mealtimes. The Commission considers Standards 12, 13, 15, 16 and 17 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 12,13,15 and 17 Service users have good opportunities to participate in appropriate activity programmes and have good access to the local community. Families and friends are encouraged to be as involved as possible in the lives of service users and their rights and responsibilities are respected. Menus are varied and nutritious and offer wholesome home cooked food. EVIDENCE: Service users attend organised day programmes for all five, week days. The programmes are designed around the individual and include work experience, sports activity and community activities. On two evenings per week service users go to local clubs and at weekends they attend church, visit families and participate in various community activities. Two of the three service users have an annual holiday but one chooses not to take advantage of this opportunity. All three service users have contact with family and friends and are visited by or visit them. The menus seen were very varied and food freshly prepared. Service users receive personal support in the way they prefer and require. Service users’ physical and emotional health needs are met. Service users retain, administer and control their own medication where appropriate, and are protected by the home’s policies and procedures for dealing with medicines. The ageing, illness and death of a service user are handled with respect and as the individual would wish. The Commission considers Standards 18, 19, and 20 the key standards to be inspected at least once during a 12 month period. Service users feel their views are listened to and acted on. Service users are protected from abuse, neglect and self-harm. The Commission considers Standards 22, and 23 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 22 and 23 The home has a robust complaints policy and all complaints are recorded in the Complaints book. Service users are, generally, protected from abuse but their financial safety is less clear. EVIDENCE: The complaints procedure is produced in service user-friendly format and the home has a complaints book where all concerns and complaints are entered. The home has not had any, recorded, complaints since 2003. One service user confirmed that he would know who to talk to if he wasn’t happy. The home has a vulnerable Adults and Whistle blowing policy and individual service users have behavioural guidelines, as necessary. The home does not have the need to use any physical interventions with their service users. The home has made progress in ensuring the service users have their own bank accounts and some financial information but there are still recording issues around service users expenditure. One financial record examined (savings account) showed a large expenditure over a three-month period and the records and receipts to correspond with this could not be located. The manager was not available and the staff member felt that she could probably explain the expenditure but it was not clear from the records, which could be accessed. Service users have the specialist equipment they require to maximise their independence. The home is clean and hygienic. The Commission considers Standards 24, and 30 the key standards to be inspected at least once during a 12 month period. Service users benefit from well supported and supervised staff. The Commission considers Standards 35 the key standard to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 33,35 and 36 The home has a very limited, although effective staff team and individual needs of the service users appear to be met. Staff are supervised regularly. EVIDENCE: The home has a very limited staff team, which consists of the manager and two full time staff, one of the staff members is currently on extended leave. The home has only one staff member on duty at any time, except for an occasional few hours during the week. There are two staff in the home during the day, on occasion and this means that there are some handover times and supervision times but these are limited. Staff are able to pursue activities with service users, at weekends because two of the service users visit families frequently, if this was not the case this could be a real issue as two people need one to one support in the community. There appear to be slightly more staff available than at the last inspection but the manager was not available to discuss or evidence whether a formal review of the staffing establishment had been completed. The staff member said that she was now supervised regularly but she had not had an annual appraisal, as yet. JUDGEMENT – we looked at outcomes for standard(s) 39 and 42 The home does not have an effective quality assurance system and does not have evidence, available of its’ compliance with Health and Safety requirements. EVIDENCE: The home does not have a quality assurance system and the commission does not receive regular regulation 26 reports, none were available in the home on the day of inspection and the staff were not sure whether they had taken place or not. There were no records of the Mandatory Health and Safety training and risk assessments for the radiators. Safe working practice risk assessments were limited and there was no evidence that the fire officers’ recommendations had been complied with.
