What the care home does well

The home provides good quality care to individuals. The care is provided within a person centred approach, which promotes the rights, independence, choice and inclusion of the client. The domestic environment is well maintained and suited to promoting the concept of self-care. The registered provider is a clear communicator and has a pro-active approach to improving the already above average service.

What has improved since the last inspection?

New railings have been fitted to the front steps leading to a new porch. All members of the managers family have had enhanced checks made through the Criminal Records Bureaux. The Provider and another family member have attended training on the protection of vulnerable adults.

The home is owned and run by Sarah Vooght who is a registered nurse and has the Registered Managers Award and NVQ 4 in care. The Provider offers 1:1 care. Dedicated time is spent with the clients between 10.00 am and 18.00pm, five days a week. The Provider is on call at all other times. Clients are encouraged to perform their own house keeping duties and chose weekly menus, the manager assists with shopping and food preparation. Clients are encouraged to go on outings with the manager, sometimes with her family. For long term stays a weekly plan is devised, which could include a work placement. Routines and plans are flexible and designed to meet individual needs. The home is situated in the quiet residential area of Highweek, a short car distance from Newton Abbott. There are bus routes into the town centre. The accommodation is provided as a self contained one bedroom flat which is adjoined to the Providers private dwelling. The flat has a kitchen, lounge/dining room, bedroom with ensuite WC and shower. The inspection took place over 2 and half hours and was announced. Information was received from the registered provider before the inspection as was written feedback from 1 visiting professional and the client living at the home. A full tour of the client’s accommodation took place and personal records of the client and staff were inspected. The inspector spoke to the client as well the registered owner/manager. Prospective service users have the information they need to make an informed choice about where to live. Prospective users’ individual aspirations and needs are assessed. Prospective service users’ know that the home that they will choose will meet their needs and aspirations. Prospective service users have an opportunity to visit and to “test drive” the home. Each service user has an individual written contract or statement of terms and conditions with the home. The Commission consider Standard 2 the key standard to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 2,3,4, The level of information received about clients before their admission is good this allows the registered owner/manager to make informed decisions about if/how the persons needs will be met. EVIDENCE: The Provider confirmed that all service users admitted to the home have their placement arranged under care management processes. The Present Service User has been living at the home for over two years therefore there have been no other admissions since. Despite this the Manager provided evidence that detailed information about the clients previous history had been obtained from the clients care manager. The existing client confirmed that they visited 16 Hill Park before making a decision to stay. The Provider stated that prospective clients are invited to visit the home and spend time with the management team and are encouraged to stay for a meal and spend time within the environment. 16 Hill Park Road D54-D07 S3625 16 Hill Park Road V214538 310505 Stage 4.doc Version 1.20 Page 9 Individual Needs and Choices The intended outcomes for Standards 6 – 10 are: 6. 7. 8. 9. 10. JUDGEMENT – we looked at outcomes for standard(s) 6,7,8,9, Clients living in the home are able to make decisions about their life and are consulted about all aspects of life within the home and maximise their independence. EVIDENCE: The client was aware and is constantly involved in planning their goals, which are documented in their individual plan. The client is able to make decisions about their life and is consulted about all aspects of life within the home, clients are encouraged to maximise their independence. An annual review is undertaken this is documented, agreed and signed by the client, the provider and the person contracting the care. Care plans set out the goals clearly with plans on how to meet them and these are reviewed and agreed on a monthly basis. Service users’ rights are respected and responsibilities recognised in their daily lives. Service users are offered a healthy diet and enjoy their meals and mealtimes. The Commission considers Standards 12, 13, 15, 16 and 17 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 11,12,13,14,15,,16,17 The home offers a broad range of activities to meet the client’s needs. Clients can make choices about all aspects of their lives and are supported to do so. The food in the home is of good quality and suits the needs of the client. EVIDENCE: The client told the inspector that they attend college on a weekly basis, and is working towards an advanced IT certificate. The client showed the inspector certificates of qualifications already gained and of the work that they are currently undertaking. The client told the inspector about what they like to do in their leisure time this included trips to town to do their own shopping and visit the library to use the internet, going to the cinema and just ‘chilling out’ at home watching TV (especially Manchester United football matches) and playing computer games. The client told the inspector that they enjoy regular trips home to their family and was looking forward to a 6-day break with them to celebrate their birthday. As well as frequent trips home the client is in almost daily contact with their family by phone. 16 Hill Park Road D54-D07 S3625 16 Hill Park Road V214538 310505 Stage 4.doc Version 1.20 Page 11 The client’s accommodation has its own front door and the client is able to choose who is able to visit. The client told the inspector that the food available in the home suits their preference, the client is encouraged to make their own breakfast, at least 1 cooked meal is provided and a choice of snacks were available in the clients kitchen. Service users receive personal support in the way they prefer and require. Service users’ physical and emotional health needs are met. Service users retain, administer and control their own medication where appropriate, and are protected by the home’s policies and procedures for dealing with medicines. The ageing, illness and death of a service user are handled with respect and as the individual would wish. The Commission considers Standards 18, 19, and 20 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 18,19,20 and 21 The home has robust arrangements to ensure the health care needs of the client are identified and met. EVIDENCE: The client is independent with personal care however they did tell the inspector that sometimes they needed reminding about certain aspects of their self- care and indicated that this was done in an appropriate manner. The client told the inspector that they had recently attended an out patients appointment with a hospital specialist and is able to get an appointment with their GP when needed. Records confirmed that an optician and a dentist have recently seen the client. The client self administers medication and chooses not to lock it in a medicines box, but as they live independently in their own accommodation this does not pose a risk to others and is clearly documented in the care plan. EVIDENCE: The client told the inspector that they feel comfortable to discuss any concerns or issues with the registered provider and that things get sorted as long as they are known about. The registered provider and another family member have recently attended training on the protection of vulnerable adults. Service users have the specialist equipment they require to maximise their independence. The home is clean and hygienic. JUDGEMENT – we looked at outcomes for standard(s) 24,25,26,27,29,30 The environment meets the needs of the client. EVIDENCE: The accommodation is provided as a self contained one bedroom flat which is adjoined to the Providers private dwelling and contains a lot of the clients own belongings. The flat has a kitchen, lounge/dining room, bedroom with ensuite WC and shower. The flat is accessed by a steep driveway which means that Service Users have to be independently mobile. The client told the inspector that they would like their bedroom to be decorated in a different colour and the provider agreed this at the time of the inspection. Part of the clients care plan includes encouraging domestic independence and to that end a cleaning schedule is available for them, with encouragement and prompting the client cleans their own accommodation which was clean and tidy at the time of this inspection. 16 Hill Park Road D54-D07 S3625 16 Hill Park Road V214538 310505 Stage 4.doc Version 1.20 Page 16 Staffing The intended outcomes for Standards 31 – 36 are: 31. 32. 33. 34. 35. 36. JUDGEMENT – we looked at outcomes for standard(s) 37,38,39 and 42 The management of this home is effective, clear and transparent. EVIDENCE: The client told the manager that they like the registered provider and want to continue living at the home because they feel safe and comfortable. Completed Service satisfaction questionnaires where inspected dated January 2005 – feedback from a visiting care manager included the following comments – the provider ‘acts on suggestions and reports back’, ‘communicates effectively with the family and maintains positive relationships’, ‘more service users could benefit from this type of support and assessment’. The clients next of kin commented ‘ that their relative ‘is happy with all that is provided for him’ and the provider ‘is easy to approach’.
