All accommodation is provided in single bedroom. There is sitting room and a kitchen/diner on the ground floor. Two of the bedrooms are situated on the first floor and one on the ground floor. The home has a walk in shower on the ground floor and bathroom on the first floor. Toilets are located on the ground and first floor. To the rear of the property there is a small patio garden, which is easily accessible to the residents. The home is staffed twenty four hours a day. A seven day holiday is included in the weekly fees and staff support the residents to access community facilities and maintain and develop new interests/hobbies. 16 Watford Road F52_F02_s25139_Watford_v227157_200505_Stage 4.doc Version 1.30 Page 6 SUMMARY
This is an overview of what the inspector found during the inspection. This was an unannounced inspection, which took place over three hours. At the time of the inspection two residents, two staff members and one visitor was present. I inspected the residents and staffing records, spoke to residents, staff and visitor to get their views of the services offered at Watford. A tour of the building was also conducted to assess the quality of the environment to ensure the health and safety of the residents. The intended outcomes for Standards 1 – 6 are: 1. 2. 3. 4. 5. 6. Prospective service users have the information they need to make an informed choice about where to live. Each service user has a written contract/ statement of terms and conditions with the home. No service user moves into the home without having had his/her needs assessed and been assured that these will be met. Service users and their representatives know that the home they enter will meet their needs. Prospective service users and their relatives and friends have an opportunity to visit and assess the quality, facilities and suitability of the home. Service users assessed and referred solely for intermediate care are helped to maximise their independence and return home. The Commission considers Standards 3 and 6 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 1,3,5 The home has developed a detailed Statement of Purpose, which gives prospective residents details of the services and facilities offered at the home. Included in the Statement of Purpose is an outline of the home’s admission procedure, so that prospective residents can be confident that the home would be able to meet their needs. EVIDENCE: The home’s Statement of Purpose is regularly reviewed to show any changes in the services offered at Watford Road. The Statement of Purpose is detailed enough to give a prospective resident the initial information to make a decision about whether the services offered at Watford Road would meet their needs Currently the home had one vacancy. The home has a detailed admission assessment procedure. The home provides only personal care, and the District Nurse would provide any nursing care that the resident requires. Discussion with the staff confirmed the admission process into the home. Prospective residents were visiting the home over a period of time to ensure their compatibility with the existing residents and for management and staff to make their own assessment of the prospective resident needs within the home

The intended outcomes for Standards 7 – 11 are: 7. 8. 9. 10. 11. The service user’s health, personal and social care needs are set out in an individual plan of care. Service users’ health care needs are fully met. Service users, where appropriate, are responsible for their own medication, and are protected by the home’s policies and procedures for dealing with medicines. Service users feel they are treated with respect and their right to privacy is upheld. Service users are assured that at the time of their death, staff will treat them and their family with care, sensitivity and respect. The Commission considers Standards 7, 8, 9 and 10 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 7,8,9,10, The staff at the care home, psychiatrist, Community psychiatric nurse and other heath professional review the health needs of the residents regularly to ensure that the staff at the care home have the necessary information and support to meet the needs of the resident. The home maintains accurate and detailed care plans showing how the resident’s are met and medication records. EVIDENCE: Each resident has a detailed service user plan and risk assessments showing how the staff would meet their health, personal and social needs and how any identified risk would be minimised. The key worker reviews the resident’s care plans weekly and the management reviews them monthly with the staff team. All residents in the home can access their NHS entitlement e.g. GP visits, optician, dental, chiropodist care, medication review etc. and where necessary staff would accompany residents to their hospital appointments. Inspection of the residents’ medication records showed the quantity of service user medication received into the home and the date and when medication was administered. The residents are able to administer their own medication once an assessment is made by the staff and other relevant health professionals

All staff responsible for the administration of medication has received training in the administration of medication. Observation of personal support provided by the staff to the residents’ is offered in a way to promote and protect the resident’s privacy, dignity and independence. This was evidence by the care staff being polite and courteous to the residents, knocking on service users bedroom doors before entering etc. The visitor spoken to praise the care staff for the way they carried out their work, spoke highly of care provided at Watford Road and commented on the friendly and homely atmosphere of the home. 16 Watford Road F52_F02_s25139_Watford_v227157_200505_Stage 4.doc Version 1.30 Page 12 Daily Life and Social Activities
The intended outcomes for Standards 12 - 15 are: 12. 13. 14. 15. Service users find the lifestyle experienced in the home matches their expectations and preferences, and satisfies their social, cultural, religious and recreational interests and needs. Service users maintain contact with family/ friends/ representatives and the local community as they wish. Service users are helped to exercise choice and control over their lives. Service users receive a wholesome appealing balanced diet in pleasing surroundings at times convenient to them. The Commission considers all of the above key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 12,13,14,15 The resident’s social, religious and recreational needs are being met. The staff supports the residents to maintain their independence and exercise control over all aspects of their daily lives. EVIDENCE: The residents engage in social activities as a group and individually to meet their interests. The staffing rota is devised to reflect outings and specific planned activities and a record is kept of activities residents have joined in. Observation and discussion with the visitor and staff said that they could visit at any time and the resident could choose where to see them. Prospective residents are encouraged to bring personal belongings into the care home to make their bedroom more homely and personal with things that they are familiar with and have sentimental value. Mealtime at Watford Road is very relaxed and friendly. The residents on the day choose what they wish they eat and can have a snack throughout the day. The residents are involved in the menu planning and accompany staff to do the weekly food shopping, so that they could also see for themselves what is available and make changes to the shopping list. The visitor said that there was always a wide choice of food and snacks available the home for the

Service users’ legal rights are protected. Service users are protected from abuse. The Commission considers Standards 16 and 18 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 16,17,18 The home has detailed policies and procedures in place, which is adhered to protect the residents. In addition staff receive training on Protection of Vulnerable Adults, which is regularly updated to maintain their knowledge. EVIDENCE: The home has a complaints procedure, which outlines clearly how complaints would be dealt with and timescales. The home has not had any complaints in the last twelve months. Discussion with staff and inspection of staff training records show that staff are given training in the Protection of Vulnerable Adults to ensure that they recognise all forms of abuse and are aware of what action they need to take to protect the residents and themselves. 16 Watford Road F52_F02_s25139_Watford_v227157_200505_Stage 4.doc Version 1.30 Page 15 Environment

Service users have access to safe and comfortable indoor and outdoor communal facilities. Service users have sufficient and suitable lavatories and washing facilities. Service users have the specialist equipment they require to maximise their independence. Service users’ own rooms suit their needs. Service users live in safe, comfortable bedrooms with their own possessions around them. Service users live in safe, comfortable surroundings. The home is clean, pleasant and hygienic. The Commission considers Standards 19 and 26 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 19,20,21,22,23,24,25,26 The standard of furnishing, decorating and maintenance of the property is of a good standard. EVIDENCE: The building is maintained by the housing association and has a planned programme of decoration and maintenance. All parts of the home are accessible to the residents. The home have one lounge and kitchen/diner, and residents bedrooms, all of which are decorated and furnished with “homely” furniture and fixtures and fittings. The home has a “walk in” shower and a separate toilet on the ground floor and a bathroom with toilet on the first floor. The home does not have any aids except handrails on the stairs to assist residents with their mobility. However, the staff said that aids would be looked at once it is require by a resident. The quality and décor of the home contributes towards the “homely” feel of the home.

The intended outcomes for Standards 27 – 30 are: 27. 28. 29. 30. Service users needs are met by the numbers and skill mix of staff. Service users are in safe hands at all times. Service users are supported and protected by the home’s recruitment policy and practices. Staff are trained and competent to do their jobs. The Commission considers Standards 27, 29, and 30 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 27, 29, 30 The home has a stringent recruitment policy which is followed and staff are required to attend training as identified in their supervision and in accordance with the organisation’s policy. EVIDENCE: The home has recently appointed a new manager who had not yet taken up their post. The acting manager has worked at the care home for many years and is knowledgeable and experienced in meeting the needs of the residents. There is low staff turnover and sickness level at the home, which allows residents to benefit from continuity of care. The staffing level in the home meet the needs of the residents and the staffing level is changed according to the needs of the residents. All staff have provided the employer with two written references, a Criminal Record Bureau check is obtained prior to an employee commencing employment to ensure as far a possible that the residents are protected from abuse. All new staff are inducted into their role by working alongside a more experienced worker and receives regular supervision during the induction period. All staff are expected to complete the TOPPS induction programme, which includes training in moving and handling, food hygiene, fire awareness and first aid. Service users live in a home which is run and managed by a person who is fit to be in charge, of good character and able to discharge his or her responsibilities fully. Service users benefit from the ethos, leadership and management approach of the home. The home is run in the best interests of service users. Service users are safeguarded by the accounting and financial procedures of the home. Service users’ financial interests are safeguarded. Staff are appropriately supervised. Service users’ rights and best interests are safeguarded by the home’s record keeping, policies and procedures. The health, safety and welfare of service users and staff are promoted and protected. The Commission considers Standards 33, 35 and 38 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 33,35,38 The records in the home are well kept and the well being of the residents are protected and safeguarded through staff training, maintenance of the building and equipment through regular checks with records and documentation to support this. EVIDENCE: The home has an annual development plan, which takes into account the changing needs of the residents. Residents’ views are obtained through the monthly residents’ meetings. Written records are kept of resident’s monies or valuables given to staff for safekeeping and internal audit is carried weekly to safeguard residents’ interests. The health and safety of the residents are protected through maintenance of fire equipment, regular checks of emergency lighting, hot water temperatures
