Conditions of registration: 1. Age Range 60 upwards Date of last inspection 8th December 2005 Brief Description of the Service: Warwick care home is owned and managed by Mrs Celina Jowett and provides nursing care for up to 17 residents. Previously an Edwardian private dwelling, circa 1908, it has been converted and extended for its present use. It is built with a mix of stone facing and white washed pebbledash and is on two floors including ground with a four person vertical lift providing level access to the first floor. It is located in a quiet residential area close to open parkland and within reasonable walking distance of Harrogate town centre. This is an overview of what the inspector found during the inspection. The unannounced inspection took place over two hour five minutes, following a half days preparation. A tour of the building was undertaken which included communal areas, bedrooms, kitchen, treatment room and office. The nurse in charge was available to commence the inspection. The proprietor manager also assisted with this process. Service users and one visitor was spoken with. The homes physiotherapist, keep fit “chair aerobics” facilitator, care staff and visiting National Vocational Qualification Assessor were also spoken with. Each service user has a written contract/ statement of terms and conditions with the home. No service user moves into the home without having had his/her needs assessed and been assured that these will be met. Service users and their representatives know that the home they enter will meet their needs. Prospective service users and their relatives and friends have an opportunity to visit and assess the quality, facilities and suitability of the home. Service users assessed and referred solely for intermediate care are helped to maximise their independence and return home. The Commission considers Standards 3 and 6 the key standards to be inspected at least once during a 12 month period. The intended outcomes for Standards 7 – 11 are: 7. 8. 9. 10. 11. The service user’s health, personal and social care needs are set out in an individual plan of care. Service users’ health care needs are fully met. Service users, where appropriate, are responsible for their own medication, and are protected by the home’s policies and procedures for dealing with medicines. Service users feel they are treated with respect and their right to privacy is upheld. Service users are assured that at the time of their death, staff will treat them and their family with care, sensitivity and respect. The Commission considers Standards 7, 8, 9 and 10 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): Standards 8 9 Service users health care needs are met, however storage facilities relating to medications must be reviewed. EVIDENCE: Standard 8 was inspected in relation to the shortfall highlighted during the last inspection. The proprietor stated that a suction machine was going to be purchased for the home. Medication received within the home was recorded correctly. Staff were seen to be administering medications in a safe and professional manner. The Boots Medication Dosage System was in operation and works well within the home. The proprietor confirmed that a supplier was contracted to dispose of medications utilizing the correct disposal systems. Service users at present do not self medicate although the home could provide this facility if service users were assessed to be capable and wished to do so. Storage of medication occurred within two areas of the home. One area used for storage contained a staff toilet. It was noted that available storage space, which could be utilized within the home for medications, was extremely limited

The proprietor must ensure that both storage areas comply with current law and legislation in regards to the Medicines Act 1968, the Royal Pharmaceutical Society and Misuse of Drugs Act 1971. Service users find the lifestyle experienced in the home matches their expectations and preferences, and satisfies their social, cultural, religious and recreational interests and needs. Service users maintain contact with family/ friends/ representatives and the local community as they wish. Service users are helped to exercise choice and control over their lives. Service users receive a wholesome appealing balanced diet in pleasing surroundings at times convenient to them. The Commission considers all of the above key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): Standards 13 14 Service users have control over their lives; receiving visitors at any time and could follow their chosen community activities if they wished. EVIDENCE: Service users could receive visitors at any time. Information regarding visiting was available within the service user guide and was made available to all new service users, their family and friends at the time of admission to the home. Service users were seen to exercise choice and control over their lives, and were assisted appropriately by the care staff. Service user had detailed care plans in place regarding their spiritual needs, Emotional and psychological wellbeing and Family Involvement. Service users could state whom they wished to see or not to see. Service users had continued to keep contacts with the local community as they wished. Service users live in safe, comfortable bedrooms with their own possessions around them. Service users live in safe, comfortable surroundings. The home is clean, pleasant and hygienic. The Commission considers Standards 19 and 26 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): Standard 26 Service users live within a home that was clean, however commode-sluicing facilities must be reviewed. EVIDENCE: The domestic staff were seen cleaning the home at the time of the inspection. There was no malodour present within any area of the home. There was no sluice disinfector machine provided within the home. Sluicing of commode pans was undertaken by utilizing a cleaning sanitizing product each time a commode pan was emptied. The staff also bleach all the commode pans twice a week. The bleach was stored within the sluice in a lockable cupboard. At the time of the inspection the cupboard was unlocked, the proprietor stated that this was not usually left unlocked and rectified this immediately. Service users are supported and protected by the home’s recruitment policy and practices. Staff are trained and competent to do their jobs. The Commission consider all the above are key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): Standard 28 Service users were in safe hands at all times. EVIDENCE: Service users were looked after by staff who were encouraged to undertake the National Vocational Qualification in Care at level 2 or 3. At present 3 care staff hold this qualification, with a further 2 undertaking it. One carer was seen with the NVQ assessor within the home, and was progressing well with the qualification. Once the two care staff have completed the course the home would have nearly achieved a 50 ratio of staff that hold this qualification. Service users live in a home which is run and managed by a person who is fit to be in charge, of good character and able to discharge his or her responsibilities fully. Service users benefit from the ethos, leadership and management approach of the home. The home is run in the best interests of service users. Service users are safeguarded by the accounting and financial procedures of the home. Service users’ financial interests are safeguarded. Staff are appropriately supervised. Service users’ rights and best interests are safeguarded by the home’s record keeping, policies and procedures. The health, safety and welfare of service users and staff are promoted and protected. The Commission considers Standards 31, 33, 35 and 38 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): Standard 33 38 The home is managed in such a way that the health and safety of service users and staff is considered. EVIDENCE: The proprietor has commenced the implementation of a Quality Assurance system, and will continue to develop this by means of questionnaires issued to service users, relatives and visitors. Also minutes of resident and relatives meetings will be taken and displayed within the home. Outcomes of one to one discussions with service users may also be recorded to evidence the quality assurance taking place within the home on a day-to-day basis. Standard 38 was not fully inspected, however the area of shortfall relating to the last inspection was examined. Staff now read and sign a document to say that they have understood the homes policies and procedures. The proprietor stated that this has been helpful to ensure that all staff have a full understanding of the homes policies and procedures.

Yes STATUTORY REQUIREMENTS This section sets out the actions, which must be taken so that the registered person/s meets the Care Standards Act 2000, Care Homes Regulations 2001 and the National Minimum Standards. OP26 13(3) 16(2) A medical suction machine must be purchased. (Previous time –scale of 01/01/06 not met) The registered provider must 14/04/06 ensure that all medications are stored correctly in regards to The Medications Act 1968, The Royal Pharmaceutical Society guidelines, and the Misuse of Drugs Act 1971. Suitable sluicing facility and a 01/05/06 sluicing disinfector must be provided. The proprietor must provide a written account of how the commode pans are sluiced and disinfected at present.
