Conditions of registration: Date of last inspection 18th March 2005 Brief Description of the Service: 3 Prince of Wales Road is a care home registered to provide nursing care and accommodation for 8 learning disabled people who may also have physical disabilities and emotional and behavioural needs. The home is operated by Dorset Residential Homes, a registered charitable trust that operates a number of care homes in Dorset. The home is located in Dorchester within easy reach of the town centre. It is a large detached property that has been carefully extended and adapted to meet the needs of service users; the home does not stand out from neighbouring properties. There are 4 single bedrooms and 2 shared bedrooms for service users; on the ground floor there is a lounge, kitchen and dining room. The home has level access throughout the ground and 1st floor and there is also a passenger lift. There are bathrooms and toilets on both floors; the home also provides a range of equipment to meet the needs of people with disabilities. The home is staffed by a team of registered learning disability nurses and care staff who provide 24-hour care and support. This is an overview of what the inspector found during the inspection. This inspection has been undertaken as part of the statutory inspection process in accordance with the Care Standards Act, 2000. 3 Prince of Wales Road was assessed according to the Care Home for Adults (18-65) National Minimum Standards. The overall time spent to complete the inspection process was a total of six hours, two of which were spent at the Home. The Registered Manager, one qualified nurse and one support worker were available throughout the inspection. One resident was at home but did not join in the inspection process. A tour of the premises and grounds took place and records/documentation were all readily available for inspection. This was a positive inspection of a service that continues to develop and where staff work enthusiastically side by side with residents with varying abilities and needs. The inspection process was assisted by the openness of the staff and the inspector was grateful for their time and commitment to the inspection. What the service does well:
All the residents have varying abilities, needs, likes and dislikes and all these details are extremely were recorded in each resident’s Life & Support Plans. Good use of photographs, graphics and some text make the Plans accessible to the residents. All the staff met on the day of the inspection had a very clear understanding of the care provided to the residents. The staff /service have a very positive attitude in promoting active and meaningful lifestyles for all the residents and independence is encouraged at all levels. 3 Prince of Wales Road provides a comfortable and homely environment with a staff team who know and understand each person living there. Prospective service users have an opportunity to visit and to “test drive” the home. Each service user has an individual written contract or statement of terms and conditions with the home. The Commission consider Standard 2 the key standard to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): Since the last inspection there have been no admissions. The intended outcomes for Standards 6 – 10 are: 6. 7. 8. 9. 10. Service users know their assessed and changing needs and personal goals are reflected in their individual Plan. Service users make decisions about their lives with assistance as needed. Service users are consulted on, and participate in, all aspects of life in the home. Service users are supported to take risks as part of an independent lifestyle. Service users know that information about them is handled appropriately, and that their confidences are kept. The Commission considers Standards 6, 7 and 9 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 6, 8, & 9 • Each resident has an Individual Life & Support Plan, which identifies how his or her needs are to be met whilst living at the home. • Residents are encouraged to make decisions about their lives in the context of a risk assessment framework, which encourages and promotes independence and the active involvement in the day-to-day running of the home both formally and informally. EVIDENCE: A sample of three resident’s files were read and each contained very comprehensive information which provided a real picture of the resident including both specific needs and their hopes for the future. Each file was well presented and made up of a mixture of graphics, photographs and some text. The files all contained a pen profile and picture of the resident and this described their likes and dislikes. A section on Lifestyle Planning was very detailed and included assessments /plans based on the Five Accomplishments (Community Presence, Choice & Control, Rights, Security and Status, Individuality & Dignity, and Relationships). A further section written with the resident in the first person described “My Hopes and How I will achieve this and who will help me”. Details of the specific activities were then detailed and broken down into realistic and practical steps.

A final section was headed “ My Health & Safety and Well Being” this included risk assessments on safety within the home, personal hygiene, bathing, dressing, eating & drinking and diet. Information from this section cross-referenced with other details in the records i.e. reference to diet/eating and monitoring of weight. Please note it was not possible to discuss any of the Life Style Plans with the individual residents during this inspection but it is hoped this will be possible at the next visit and the views of the residents can then be included. Service users are offered a healthy diet and enjoy their meals and mealtimes. The Commission considers Standards 12, 13, 15, 16 and 17 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): None of these standards were assessed, as it was not possible to obtain the views of any of the residents living at the home due to the time of this inspection. The next inspection will be arranged to ensure residents are offered an opportunity to give their personal views on their life styles whilst living at the home and to discuss the relevant records reflecting their various activities and attendance at College and day services. EVIDENCE: The Registered Manager and staff advised that residents attend Day Services at Bridport and Sherborne and also access Weymouth College for Adult Education courses. Each resident has “ one day off a week” to firstly take their turn in household chores and then time “just to enjoy themselves”. Service users retain, administer and control their own medication where appropriate, and are protected by the home’s policies and procedures for dealing with medicines. The ageing, illness and death of a service user are handled with respect and as the individual would wish. The Commission considers Standards 18, 19, and 20 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 18 & 19 • Staff provide personal care in a sensitive and flexible way to all residents and the resident’s specific preferences were recorded clearly for all staff to read in their individual Life & Support Plans. • The healthcare needs of all the residents are met and residents access local NHS resources and services in addition to any specialist services identified and required. EVIDENCE: Discussions with staff confirmed that personal care is provided according to the wishes of each resident and according to their identified personal and healthcare needs. The plans were specific in the way personal care was to be provided and the information formed part of the resident’s Life and Support Plans. Each resident has a separate file containing medical information, a list of clinics they attend, outpatient appointments and outcomes of any consultations. These files make an easy reference point for any staff to check the medical details of each person as and when required. The Commission considers Standards 24, and 30 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 24 ,27, & 30 • • • Residents live in a comfortable home. There are sufficient bathrooms and toilets for all the residents use and the communal areas provide all the residents sufficient space to enjoy the shared facilities or find a quiet spot if they choose. On the day of this inspection the home was found to be clean and hygienic with no unpleasant odours. EVIDENCE: 3 Prince of Wales Road is a domestic style family home and a tour of the premises showed that the house was well maintained and in good repair. The communal rooms, which comprise a lounge, dining room, and quiet room are comfortably furnished and attractively decorated. In the shared bathrooms each resident has their own bathroom cabinet, which is an excellent idea. The ground floor bathroom is fully adapted. Some bedrooms are shared however the staff have been very careful to establish this is the choice of the residents many of whom have known each other for several years. JUDGEMENT – we looked at outcomes for the following standard(s): 32, 33, & 35 • Residents benefit from the continuity and stability of the staff team who work at 3 Prince of Wales Road. • The staff team comprises both qualified and experienced people who seem competent and knowledgeable in providing appropriate support to the residents. • Dorset Residential Homes / 3 Prince of Wales Road has a commitment to staff training ensuring all staff are kept up to date in both mandatory and professional training which contribute to their expertise and experience when supporting residents. EVIDENCE: At the time of this inspection the Registered Manager, one registered nurse and one support worker were all available and confirmed the evidence read from their staff training records re: the on going training available and achieved by them all. The Registered Manager said the staff team was “ well established and work very well both as a team and individually”. Each member of staff has their training log, which provided the evidence to demonstrate that regular mandatory was completed i.e. manual handling, food hygiene and fire training. In addition specialist training is available and a variety of staff have attended the following: stress management, specialist feeding techniques, and record keeping. Two support workers have just completed the LDAF foundation training and are now registering to complete the NVQ level 2. Four support workers are already studying for this qualification and a further two

JUDGEMENT – we looked at outcomes for the following standard(s): 42 • The Registered Manager and staff team ensure safe working practises to safeguard and protect the residents as far as practical in all aspects of daily living. EVIDENCE: Reports from other agencies, regular servicing and maintenance of equipment demonstrated the home’s commitment to ensure the health and safety and welfare of everyone living and working at 3 Prince of Wales Road. Fire Prevention and training is the responsibility of a qualified member of staff who involves all staff and residents in regular drills and refresher training. These records are kept separately from the staff training records and for the purposes of auditing the fire prevention training it might be easier if all the records were kept in one file as the individual training records showed staff to be out of date however when these details were checked in the fire training records all staff were within the required timescales.
