What the care home does well

The Registered Manager and staff team manage change extremely well with the constant transition of different service users staying at Encombe week by week for short respite stays. Detailed assessments and checklists are purposefully used to ensure the service users needs are well met during their stay and their normal patterns and routines are carefully maintained. The Manger described their level of contact with families prior to a service user staying at Encombe and in their words "it is much better to invest time at that stage to make sure we (staff) get it right for the service user". There is a reliable and knowledgeable staff team that provides a high level of care to many service users with varying needs and abilities. Encombe offers a homely comfortable place for service users to enjoy their respite stay. The Registered Manager with the support of staff maintain a relaxed and homely atmosphere whilst ensuring the National Minimum Standards are achieved.

What has improved since the last inspection?

Since the last inspection new equipment has been purchased including two new beds and mattresses. The Registered Manger continues to research for the best equipment and most suitable aids and adaptations for service users i.e. alternative equipment suitable for bed rails. Staff meetings are booked in every month and at the present time all service users` records are being reviewed. All respite stays have been booked for the next six months. The booking service is well and fairly maintained.

A maximum of 4 service users who have physical disabilities may be accommodated at any one time. 27th October 2005 Date of last inspection Brief Description of the Service: ‘Encombe’ is situated in Prince of Wales Road, close to the town centre of Dorchester and provides a ‘short stay’ residential care facility for learning disabled adults who otherwise will generally be living in their family home. Around thirty adults use the service and the home is able to accommodate up to eight service users at any one time, with facilities for up to 4 service users who have additional significant physical disability. The length of stay at ‘Encombe’ can be from a few hours to a number of weeks and is arranged to meet the needs of service users and their supporters; most stays are arranged in advance, but the home will respond to a crisis situation where possible. The property had previously been used as a care home and is now owned by a housing association; full refurbishment and alterations have been undertaken to provide a high standard of accommodation, which meets the National Minimum Standards for younger adults. The home provides staff throughout the 24-hour day, including awake and sleep-in staff. This inspection has been undertaken as part of the statutory inspection process in accordance with the Care Standards Act, 2000. Encombe was assessed according to the Care Home for Adults (18-65) National Minimum Standards. The overall time spent to complete the inspection process was a total of seven hours, three of which were spent at the home with the Registered manager, Liaison Officer and staff. During the inspection records related to the specific standards assessed were checked. This was a positive inspection of a service where staff work side by side with service users and their families ensuring their respite stay at Encombe works positively for both the service user and their families. Feedback Comment cards were received from relatives and professionals and all stated how much they valued the excellent service. Specific comments included “the staff are very caring, an excellent home, excellent liaison, all the staff have put a great deal of effort ensuring successful respite stays, my daughter is well looked after, very relaxed and happy home,, happy and well cared or it gives us a much needed break”. Service users know their assessed and changing needs and personal goals are reflected in their individual Plan. Service users make decisions about their lives with assistance as needed. Service users are consulted on, and participate in, all aspects of life in the home. Service users are supported to take risks as part of an independent lifestyle. Service users know that information about them is handled appropriately, and that their confidences are kept. The Commission considers Standards 6, 7 and 9 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 9 • Service users staying at Encombe are consulted and encouraged in maintaining their normal routines within their everyday lifestyles. Independence is encouraged where possible and comprehensive risk assessments are completed to ensure the safety and welfare of each service user. EVIDENCE: Families/Carers provide the home with information regarding the personal care needs of each individual prior to the service user’s respite stay. Further information is gathered from multi-agency assessments and these include detailed risk assessment specifically related to personal vulnerability, health related issues, adverse reactions, response to change and awareness of risk. Part of the pre-admission assessment completed with the service user or their representative involves completing the home’s own risk checklist. The initial risks identified are then followed up with comprehensive risk assessments /profiles on each specific area identified where the service user is deemed vulnerable and potentially at risk. A sample of risk assessments/profiles were read and provided detailed information of the potential hazards and risks and how these were relevant to the service user. One of the daily tasks for staff are to thereby read and update where necessary the assessments/plans for each service user staying at Encombe ensuring the service user’s needs and abilities are constantly under review during their respite stay. Service users have opportunities for personal development. Service users are able to take part in age, peer and culturally appropriate activities. Service users are part of the local community. Service users engage in appropriate leisure activities. Service users have appropriate personal, family and sexual relationships. Service users’ rights are respected and responsibilities recognised in their daily lives. Service users are offered a healthy diet and enjoy their meals and mealtimes. The Commission considers Standards 12, 13, 15, 16 and 17 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 16 & 17 • Service user’s rights are respected and independence, choice and freedom of movement are promoted in the home. • Service users are offered a healthy diet and any special dietary needs are catered for. EVIDENCE: Each service user staying at Encombe is encouraged to maintain their normal pattern and routines and to this end staff follow the service user’s existing weekly plan. These may include attendance at day centres, community opportunities or positive rest and relaxation within the safe environment of Encombe. All service user’s staying at Encombe are offered a variety of choices and are encouraged to maintain a realistic level of independence. However, staff equally respect the decisions service users may make to “opt out”. The Registered Manager provided excellent examples of how the staff have become very observant ensuring the normal routines are followed for each service user and thus causing as little disruption in their lives and routines as possible e.g. There is one service user who will refuses to eat unless the presentation of meals mirrors how their family normally does it. Prince of Wales Road (5) DS0000059023.V279239.R01.S.doc Version 5.1 Page 12 The Registered Manager explained that staff do their utmost to mirror the “style of support service user’s prefer”. Menus vary according to the group of service users staying at Encombe and a choice is always provided at every mealtime. Service users are encouraged where possible to help prepare and clear away after their own meals. Several service users require special diets and these are always recorded and catered for by staff at Encombe. Service users receive personal support in the way they prefer and require. Service users’ physical and emotional health needs are met. Service users retain, administer and control their own medication where appropriate, and are protected by the home’s policies and procedures for dealing with medicines. The ageing, illness and death of a service user are handled with respect and as the individual would wish. The Commission considers Standards 18, 19, and 20 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 20 • The record of medication and administration of medicines by staff was carefully documented and complete. EVIDENCE: Medication is booked in and out with each service user at the start and end of their stay at Encombe. All service users staying at Encombe have a completed medication profile, with their photograph attached. A comprehensive but easy to use form has been created which clearly shows the type, and quantity of any medication coming into the home and the number of any being returned at the end of the stay. Prior to the service user’s stay the carers/family are asked to complete all details of current medication. This information then gets transferred onto the home’s own medication sheets. Only senior staff administer medication and this is only sanctioned after their competency has been recorded and signed off by the Registered Manager. All staff as part of their induction complete basic training in the safe handling and storage of medication. The Registered manager regularly reviews the medication protocol with the staff team. As there were no service users currently staying at Encombe the week of this inspection visit previous records were checked and found complete. Service users feel their views are listened to and acted on. Service users are protected from abuse, neglect and self-harm. The Commission considers Standards 22, and 23 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 22 & 23 • Staff are aware of the action to be taken should a complaint be made. • The home has appropriate systems to protect service users. There are procedures and evidence of good practice in place to help safe guard service users from potential abuse and harm. EVIDENCE: DRH has comprehensive policies and procedures for dealing with any complaints or allegations of abuse and the Registered Manager is confident all staff would be very clear on how to implement the correct strategies for dealing with situations which may include allegations of abuse. The home has not received any complaints since the last inspection. Not all the service users use language to communicate. However, the staff are very familiar with the different ways service users’ indicate their likes/dislikes and feel through individual behaviour and or gestures all service users trying to indicate a complaint or making an allegation would be listened to and concerns acted on very promptly. Each service user and or their family/representative receive an Information Pack prior to their stay at Encombe. Included in this information are details about making complaints. An admission form is completed each time a service user stays at Encombe and this includes a section for feedback/comments. DRH issue all new staff their own copy of the multi agency document No Secrets, which clearly sets out the strategies, which must be followed, should an incident ever occur. Service users live in a homely, comfortable and safe environment. Service users’ bedrooms suit their needs and lifestyles. Service users’ bedrooms promote their independence. Service users’ toilets and bathrooms provide sufficient privacy and meet their individual needs. Shared spaces complement and supplement service users’ individual rooms. Service users have the specialist equipment they require to maximise their independence. The home is clean and hygienic. The Commission considers Standards 24, and 30 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 24 & 30 • Encombe provides a homely comfortable environment for service users, which on the day of the inspection was safe and clean throughout. EVIDENCE: At the time of this inspection visit there were no service users staying so the inspector with the Registered Manager undertook a full tour of the premises including all the bedrooms. Bedrooms and communal areas are extremely comfortable and well furnished. Wheelchair users can be readily accommodated either be using ground floor bedrooms or accessing the first floor via the passenger lift. The rooms for wheelchair users have wide access and one room has the provision of a wet room. Communal facilities within the home include a large lounge, which can be divided into two separate areas with sensory equipment at one end. There is a good size dining room and large well-equipped kitchen. Environmental adaptations have been made and there are specially adapted bathrooms and rooms with fully fitted overhead hoists. The booking system takes into account the maximum number of wheelchair users the home can cater for and which adapted rooms suit the needs of the service users. Service users benefit from well supported and supervised staff. The Commission considers Standards 32, 34 and 35 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 33 , 34 & 35 • Encombe provides sufficient staff to ensure service users will have their needs appropriately met by the number and skill mix of staff on duty during their short stay. • Residents are safeguarded by DRH/ the home’s recruitment policies and procedures. • Service users and or their representative will be well supported by a well lead, well organised and well trained staff. EVIDENCE: The Registered Manager and other staff spoken with have a very good understanding of their roles and responsibilities and of Encombe’s aims and values. Both the Registered Manager and Deputy are qualified nurses. The Registered Manager has NVQ level 4 in care management and the deputy is about to commence studying for this qualification. There are 4 senior staff and 7 care staff. A total of six staff have achieved levels 2 /3 in NVQ training and 5 staff are registered to start their training in these qualifications. The rota is adjusted according to the numbers and needs of the service users booked at Encombe week by week. The staff files of two recently recruited staff were checked and both contained all the required information, references and statutory checks. Service users benefit from competent and accountable management of the service. The Commission considers Standards 37, 39, and 42 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 37, 39. • Service users benefit from a home that is well managed. • Service users’ rights and best interests are safeguarded by the homes policies and procedures, which are carefully implemented at Encombe. EVIDENCE: Encombe provides respite stays for many service users with very different needs and abilities. The Registered Manager said the team positively like the stimulation and challenge of meeting the needs of different groups of people each week. The Registered Manager is very experienced and clearly values the staff team describing the importance of keeping staff enthusiastic and ensuring that each member of staff is valued for their contribution. The Manager encourages key workers to attend and represent Encombe at service user’s multi agency Individual Service Plan meetings. The Registered Manager attends monthly management meetings with other managers from the network of DRH homes and monthly staff meetings are held at Encombe. These reports are comprehensive and extremely useful in providing up to date information. Quarterly health & safety audits are completed, six monthly financial audits and all staff are asked to monitor their own work by completing their annual workbook. Additional monitoring is carried out through regular supervision sessions, staff meetings and the feed back section of the “Respite admission form” completed by the service users’ representative/family carers prior to each stay.
