CSCI found this care home to be providing an Poor service.

The inspector found there to be outstanding requirements from the previous inspection report. These are things the inspector asked to be changed, but found they had not done. The inspector also made 1 statutory requirements (actions the home must comply with) as a result of this inspection.

What the care home does well

Two requirements were made following the key inspection in October. Both of these have now been met.

What the care home could do better:

Clear records of any medicines remaining in stock at the end of each month and kept for future use should be kept, so that a complete audit of medicine use can be made. This would enable the home to show that all medicines signed as given have been given to people who use the service.

Date of last inspection Brief description of the care home Porthcawl Green provides accommodation and support for three service users with Autistic Spectrum Disorders. The house is situated in a quiet residential area of Milton Keynes, close to the Westcroft centre where there are several shops and supermarkets. Local bus networks provide regular access to central Milton Keynes and Bletchley. There are main line rail stations in Milton Keynes and Bletchley giving access to London, the Midlands and the North. The home has three bedrooms, one en-suite, a staff sleep-in room, office and a bathroom on the first floor. On the ground floor there Care Homes for Adults (18-65 years) Page 2 of 8 is a large lounge, dining room, space to use the computer, laundry and kitchen. There is a small enclosed back garden and parking at the front of the home. Care Homes for Adults (18-65 years) Page 3 of 8 What we found: This inspection was a specialist pharmacist inspection to look at the handling of medicines in the home, following concerns raised at the last key inspection. People who use this service have their medicines given to them by designated care staff. Written procedures are available to staff to provide guidance and enable medicines to be handled consistently by all staff. No one was looking after their own medicines when we visited. The medication administration records, supplies and care plan were looked at for the one person prescribed medicines. Clear records were kept of medicines prescribed for people, received into the home and given to people. When we looked at the medication supplies we noted that Clozaril remained in the blister packs on the mornings of 23rd, 28th and 29th December and the evenings of 29th December and 6th January. The service user confirmed to the manager that he had had all of his Clozaril. The carer who had been on duty on the mornings in question confirmed that they had given the medicine from a box of Clozaril that had been left over from the previous months supply, before the home moved to the blister system. The carer who had been on duty on the evenings in question confirmed that they had given the medicine. As no record had been made of the stock that had remained when they changed over to the blister system it was not possible to confirm beyond doubt that the medicine had been given. A detailed care plan was available describing how one persons anxiety was to be managed. It described how to calm them without the use of medication and circumstances under which to give them medication. When people had been away over Christmas separate supplies of medicines had been provided from the pharmacy to cover the time away. All medicines were stored securely so as to protect the people who use the service. No Controlled Drugs were currently prescribed, nor had any been prescribed in the last year. This is to ensure there is proper financial accounting. 2 22 22 A complaints procedure which is suitable for use at the home is to be put in place and made available in formats appropriate to the needs of people using the service. This is to make sure that they have the necessary information to hand to voice their concerns. 01/12/2008 3 22 17 A record of complaints and how they have been responded to is to be kept within the service.
