What the care home does well

The home provides the service users with an attractive and homely place to live. At the time of the inspection, the home was staffed by appropriately trained staff who had good understanding of the service users` support needs. Service Users spoken to felt the staff have built good relationships with them. Links with the community are good, the home has its own vehicle and encourages social and educational opportunities. The inspector witnessed the use of a good behaviour management system, which included clear boundaries and consistent and positive use of agreed sanctions and rewards.

What has improved since the last inspection?

The home has just completed their well equipped sensory room, in which the service users enjoy spending quality one to one time with staff.

What the care home could do better:

At the time of the inspection the home had not supplied CSCI with the monthly regulation 26 visit reports since December 2004, the original reports were sited at the home. The home to look at the skill mix of the service users living in the home to ensure compatibility.

The home was opened in August 2004 and is set in a residential area on the outskirts of Horndean, on the A3 between Guildford and Portsmouth. The home is one of many owned and run by Robinia Care, a national company based in the South of england. 6 Durlands Road H54 S60861 6 Durlands Road V221682 180405.doc Version 1.20 Page 5 SUMMARY This is an overview of what the inspector found during the inspection. The unannounced inspection took place over three hours, the inspector was supported during the inspection by two senior staff from the home. Three service users were at home all of whom had scheduled appointments and activities to attend during the day, one service user was at the Grove day centre. The home was warm, clean and homely, a service user helped a staff member show the inspector around their home, including three bedrooms and all communal areas. The inspector spent time with one service user as they looked at their care plan. The inspector looked at records and asked staff and service users for their views. The general comments made indicated that the majority of people in the home feel more settled now than a few months ago. On the day of the inspection there was some agitation between some of the service users, the staff supported the service users well, in line with individuals care plans and risk assessments. Service users are consulted on, and participate, in all aspects of life in the home. Service users are supported to take risks as part of an independent lifestyle. JUDGEMENT – we looked at outcomes for standard(s) 6,7,9. Service users are involved in completing their care plans and risk assessments. Service users are supported and encouraged to be as independent as possible. EVIDENCE: One service user explored their care plan with the inspector, which demonstrated the amount of involvement the service user has in completing all aspects of their care plan and risk assessments. The care plans and risk assessments are reviewed on a monthly basis, unless needs change sooner. Each service user living in the home has a record of their week’s activities, which includes educational and recreational activities. One service user said “I enjoy visiting the local shop by myself, staff support me to go to town by minibus on Saturdays if I want to.”” 6 Durlands Road H54 S60861 6 Durlands Road V221682 180405.doc Version 1.20 Page 10 Lifestyle The intended outcomes for Standards 11 - 17 are: 11. 12. 13. 14. 15. 16. 17. Service users have opportunities for personal development. Service users are able to take part in age, peer and culturally appropriate activities. Service users are part of the local community. Service users engage in appropriate leisure activities. Service users have appropriate personal, family and sexual relationships. Service users’ rights are respected and responsibilities recognised in their daily lives. Service users are offered a healthy diet and enjoy their meals and mealtimes. The Commission considers Standards 12, 13, 15, 16 and 17 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 13, 17 Links with the community are good, the home has its own vehicle and encourages social and educational opportunities. The meals in this home are good, offering both choice and variety to cater for special dietary needs. The varied needs of the service users often cause conflict during mealtimes. EVIDENCE: Service users are encouraged, in line with their care plan and risk assessments, to participate in social activities, both within the home and the community. Menus were looked at and found to be varied and balanced, one service user said he enjoys going to supermarkets to buy food on a weekly basis, the inspector saw fresh fruit and a well stocked fridge, which included fresh produce. 6 Durlands Road H54 S60861 6 Durlands Road V221682 180405.doc Version 1.20 Page 11 One service user said “we all eat in the dining room together, because we want to” The inspector asked if they can take food to their rooms if they wish, “yes” was the reply. The kitchen door is kept locked, this has been agreed with Service users, staff supervision is required if a service user wishes to enter the kitchen. Risk assessments and behaviour management agreements are in place to ensure the health and safety of all service users and staff. One service user said “I often get frustrated during mealtimes because I have to eat a healthy diet, and the others don`t.” 6 Durlands Road H54 S60861 6 Durlands Road V221682 180405.doc Version 1.20 Page 12 Personal and Healthcare Support The intended outcomes for Standards 18 - 21 are: 18. 19. 20. 21. Service users receive personal support in the way they prefer and require. Service users’ physical and emotional health needs are met. Service users retain, administer and control their own medication where appropriate, and are protected by the home’s policies and procedures for dealing with medicines. The ageing, illness and death of a service user are handled with respect and as the individual would wish. The Commission considers Standards 18, 19, and 20 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 19, 20 Comprehensive care plans and risk assessments are in place for all service users and reviewed on a regular basis. The manager needs to ensure the records made by staff for each service user are age appropriate and include enough information to produce a relevant detailed report. The systems for the administration of medication are good with clear and comprehensive arrangements being in place to ensure service users medication needs are met. EVIDENCE: Daily records comment on individuals daily activities and behaviour, the inspector advised staff to ensure records are age appropriate and detailed, as one entry stated “X behaved themself today” another record showed “X displayed no behaviour today”.. Staff administer all prescribed medication (in conjunction with the homes policies and procedures and individual’s risk assessments) using the Monitored Dosage system blister pack method. Only one months medication is stored, ensuring maximum stock rotation and minimal stock build up, the staff ordering the next months medication completes a stock check. 6 Durlands Road H54 S60861 6 Durlands Road V221682 180405.doc Version 1.20 Page 13 Concerns, Complaints and Protection The intended outcomes for Standards 22 – 23 are: 22. 23. Service users feel their views are listened to and acted on. Service users are protected from abuse, neglect and self-harm. The Commission considers Standards 22, and 23 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 22,23 Service users` views are sought from time to time but there was little evidence to show them being acted upon. Staff follow a comprehensive adult protection procedure. The home to look at the skill mix of the service users living in the home to ensure compatibility. EVIDENCE: One service user stated “I like the colour of my walls and curtains, but I did not choose them” another said “I like my keyworker, I would prefer to choose, rather than be given” The inspector asked staff if service users were given the choice, they said “no”. Service users spoken to during the inspection were aware of the complaints procedure. A copy of this document was found in each service users guide, held in their care plan. One service user stated they felt able to talk to staff if they had any concerns. The home has a complaints procedure which must be added to the statement of purpose. Staff at the home informed the inspector of the homes adult protection / abuse procedures, and of the importance of informing The Commission for social care inspection and social services of any concerns or allegations. Service users have the specialist equipment they require to maximise their independence. The home is clean and hygienic. JUDGEMENT – we looked at outcomes for standard(s) 24,30 The environment within and surrounding this home is good and provides service users with an attractive homely place to live. Robust Risk assessments are in place to ensure the areas within the home are safe and meet the needs of the service users. EVIDENCE: One service user showed the inspector around the home which was modern with well used communal areas. Three out of the four service users bedrooms were viewed, all found to be large, en-suit(shower over bath) and personalised with equipment, photos and pictures. JUDGEMENT – we looked at outcomes for standard(s) 42 Not all staff follow the clear guidelines that are in place to ensure consistency of managing behaviour that challenges the health, safety and welfare of the service users and staff. EVIDENCE: During this inspection there was conflict between two service users, the staff on duty followed the guidelines and procedures and ensured the boundaries and agreements were adhered to. Incentives were used to uphold these boundaries, an example of this is one service user enjoys one to one time with staff in the sensory room.
