Conditions of registration: Date of last inspection 9th September 2005 Brief Description of the Service: 63 West Park Road is a private domestic home, providing care and accommodation for one service user. The home is close to Corsham town centre and there are good transport links near by. No staff are employed as Mr and Mrs Cottle are the sole carers. The home has a garden to the front and the rear of the property and the service user has their own bedroom on the first floor. There is a ground floor shower and cloakroom and a first floor bathroom, as well as a sitting room and a large kitchen with table and chairs. This is an overview of what the inspector found during the inspection. This inspection took place on 2nd February and lasted for one hour. Mr and Mrs Cottle were present. The service user was on holiday with family. The care plan and accompanying records were looked at. What the service does well: What has improved since the last inspection? What they could do better:
The care plan needs to include some more detail about how exactly the registered person supports the service user in managing their finances. There is one new record, which is helpful, but there are still improvements needed. The registered person should find suitable training in epilepsy and an understanding of learning disabilities, so as to be better able to meet the needs of the service user. Service users are offered a healthy diet and enjoy their meals and mealtimes. JUDGEMENT – we looked at outcomes for the following standard(s): 16 The service user is supported in maintaining rights and responsibilities. Standards 12, 13, 15, 17 were all assessed as met at the last inspection. EVIDENCE: The care plan contains some brief details of how the registered person helps the service user manage their mail. This is because some important appointments had been missed in the past. This system has worked well since introduced at the last inspection. The service user has unrestricted access to all parts of the home. He has his own front door key and access to the rear of the home if required. The service user is supported in managing some household tasks. These vary, depending on how the service user is feeling. Service users retain, administer and control their own medication where appropriate, and are protected by the home’s policies and procedures for dealing with medicines. The ageing, illness and death of a service user are handled with respect and as the individual would wish. The Commission considers Standards 18, 19, and 20 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 19 The service user’s physical and emotional needs are supported by the registered person and other healthcare professionals. Standards 18 and 20 were assessed as met at the last inspection. EVIDENCE: Since the last inspection, there has been some additional involvement with other health care professionals to assess the service user’s needs and to find additional ways to support him in the current placement. The person completing the assessment has provided a helpful and detailed report to all parties. There are separate records showing episodes when the service user has been unwell and this is then shared with healthcare professionals. JUDGEMENT – we looked at outcomes for the following standard(s): Standards 32, 34 and 35 are not applicable. EVIDENCE: The registered person and her husband are the sole staff team for the service user. This is their own home and one of them is available to the service user at all times. Service users benefit from the ethos, leadership and management approach of the home. Service users are confident their views underpin all self-monitoring, review and development by the home. Service users’ rights and best interests are safeguarded by the home’s policies and procedures. Service users benefit from competent and accountable management of the service. The Commission considers Standards 37, 39, and 42 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 37, 39 The service user would benefit from the registered person having more training in understanding how to meet his needs. Standard 39 does not apply. Standard 42 was assessed at the last inspection. EVIDENCE: The registered person has completed some training as part of her other post in a care home. This includes moving and handling, basic food hygiene and NVQ level 2 training. The registered person has more understanding of the service user’s needs following the assessment by a healthcare professional. She has not been able to find training that is suitable to enable more understanding of the service user’s needs. The registered person and the inspector discussed the quality assurance standard. This is the registered person’s own home and there have been discussions in the past year about registering with the Adult Placement Scheme, as these standards seem to be better suited to the role provided by the registered person, providing a supportive background for her. West Park Road (63) DS0000028176.V274333.R01.S.doc Version 5.1 Page 16 However, the registered person decided not to register and the arrangements for the future care of the service user has been under discussion with the care manager for at least two years now. It appears as though the service user may move on to alternative accommodation in the future. The service user’s family do not have any direct contact with the home, other than phone calls. Health care professionals have had some contact, but mostly in the community, rather than in the home. The quality of accommodation is very high and there seems to be no useful purpose in asking the registered person to complete a quality assurance survey. At the last inspection, the service user made his positive views very clear about the home and the care provided. He repeated these views recently to a healthcare professional and they have been included in a recent assessment.
