What the care home does well

Those being supported have their rights as citizens protected and are included in the life of the local community. Those being supported have had a lot of continuity in their lives in terms of who they live with, where they live and who looks after them. All those being supported have a health action plan. The home provides a service which is tailored to individual needs. The service promotes and protects the health and safety of those being supported. The records are up to date and accurate.

What has improved since the last inspection?

Twice-daily checks and records are kept of hot water temperatures. The fire service has been consulted and their advice taken.

What the care home could do better:

Increase the number of permanent staff employed. Find more suitable accommodation for those being supported.

It is close to shops and other amenities. It provides residential care for up to three adults with learning disabilies, both under and over the age of 65. All those being supported are admitted on a permanent basis. Their physical independece is decreasing as they become older and the home cannot be fully adapted to meet these needs. Therefore, the home cannot guarantee being a home for life. This is being addressed and those being supported are to be given the choice of alternative, more suitable accommodation. All the places at the home are purchased by Social and Health care. The home is owned and managed by New support Options Ltd., a charitable organization with experience in providing services for people with learning disabilities. 7 Wychwood Close H57-H08 S13220 Wychwood Close V226428 090505 Stage 4.doc Version 1.30 Page 5 SUMMARY This is an overview of what the inspector found during the inspection. This unannounced inspection visit took place between 5.45pm and 9.00pm on Monday, 9 May 2005. The purpose of this visit was to see how the home is meeting the National Minimum Standards for Care Homes for Younger Adults. The inspector spent most of the time with the three women being supported at this time and the one member of staff on duty. The inspector listened to their views and discussed their experiences. The inspector examined the fire log, the records of those being supported, medication records and the minutes of staff meetings and observed those being supported participating in a range of activities. The home has a caring, relaxed and friendly atmosphere and those being supported are encouraged to socialise with each other. The three women being supported at the time of this visit were very relaxed with the staff member on duty. The inspector observed kind, caring and supportive interactions between the staff member and those being supported. Those being supported told the inspector that they are happy with the food provided. A staff member described the week’s induction training she received as “Comprehensive” and stated that she was very impressed. The inspector would like to thank those being supported and the staff member on duty at this time for their assistance, hospitality and courtesy during this inspection. JUDGEMENT – we looked at outcomes for standard(s) 3 Those being supported enjoy continuity in terms of who they live with, their accommodation and those looking after them. EVIDENCE: The three woman being supported at this time have lived together for a long time. They have discussed with staff their individual wishes about where they live when they leave 7 Wychwood Close. Service users know their assessed and changing needs and personal goals are reflected in their individual Plan. Service users make decisions about their lives with assistance as needed. Service users are consulted on, and participate, in all aspects of life in the home. Service users are supported to take risks as part of an independent lifestyle. Service users know that information about them is handled appropriately, and that their confidences are kept The Commission considers Standards 6, 7 and 9 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 6,7 and 8. The home enables those being supported to make decisions in their lives and they are encouraged to give their views and opinions. EVIDENCE: Those being supported told the inspector about a range of activities they enjoy. The home has a key worker system where the care of each person being supported is the responsibility of a named member of staff. Those being supported have person-centred plans which identify their life styles and choices and reflect their individual preferences. Service users have opportunities for personal development. Service users are able to take part in age, peer and culturally appropriate activities. Service users are part of the local community. Service users engage in appropriate leisure activities. Service users have appropriate personal, family and sexual relationships. Service users’ rights are respected and responsibilities recognised in their daily lives. Service users are offered a healthy diet and enjoy their meals and mealtimes. The Commission considers Standards 12, 13, 15, 16 and 17 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 12,13,14 and 16. Those being supported are encouraged to take part in their chosen activities within the local community. EVIDENCE: Those being supported at this time were all of retirement age. They are treated as individuals and one told the inspector that her wish to be alone at times is respected. All those being supported are registered to vote and one did so on the day of this inspection, which was election day. Those being supported are encouraged to change their bedding and compile the weekly shopping list. Those being supported told the inspector about the different activities they take part in such as attending a day centre, reading newspapers and magazines, watching TV, sewing, going shopping and to the bank and going out for drives. During this visit those being supported and the staff member joked together and talked about past experiences and future plans. Those who are physically able to do so are encouraged to prepare their own drinks and breakfasts. 7 Wychwood Close H57-H08 S13220 Wychwood Close V226428 090505 Stage 4.doc Version 1.30 Page 11 Personal and Healthcare Support The intended outcomes for Standards 18 - 21 are: 18. 19. 20. 21. Service users receive personal support in the way they prefer and require. Service users’ physical and emotional health needs are met. Service users retain, administer and control their own medication where appropriate, and are protected by the home’s policies and procedures for dealing with medicines. The ageing, illness and death of a service user are handled with respect and as the individual would wish. The Commission considers Standards 18, 19, and 20 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 18,19 and 20. The home was meeting the physical and emotional needs of those being supported. Medications are stored and administered safely and recorded carefully. EVIDENCE: Those being supported are not all physically independent. Their health care needs were being met by the community health services. Those being supported are encouraged to manage as much of their own personal care as they can. All those being supported need assistance with their medication. The inspector examined the well-kept medication records and saw that staff are well-informed of the possible side-effects of the medications given. 7 Wychwood Close H57-H08 S13220 Wychwood Close V226428 090505 Stage 4.doc Version 1.30 Page 12 Concerns, Complaints and Protection The intended outcomes for Standards 22 – 23 are: 22. 23. Service users feel their views are listened to and acted on. Service users are protected from abuse, neglect and self-harm. The Commission considers Standards 22, and 23 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 22 and 23. The views of those being supported are listened to and acted upon and staff are trained to protect them from abuse. EVIDENCE: Those being supported reported numerous examples of how their views and choices are acted upon and respected. They are given the contact details of CAPOLD, a citizen advocacy scheme. None of those being supported raised any issues with the inspector when she spoke to them at this time. Service users live in a homely, comfortable and safe environment. Service users’ bedrooms suit their needs and lifestyles. Service users’ bedrooms promote their independence. Service users’ toilets and bathrooms provide sufficient privacy and meet their individual needs. Shared spaces complement and supplement service users’ individual rooms. Service users have the specialist equipment they require to maximise their independence. The home is clean and hygienic. The Commission considers Standards 24, and 30 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 24,25,28 and 30. The home is kept in a good state of repair and a programme of maintenance and renewal is in place. Those being supported were comfortable and relaxed. The home was bright, clean, tidy and fresh-smelling at this time. EVIDENCE: The inspector spent most time in the lounge and in the bedroom of one of those being supported. This is a “homely” home which clearly reflects the interests of those being supported. One of those being supported confirmed to the inspector that she was happy in her room and that it contains all the items she wants and needs. Service users are supported and protected by the home’s recruitment policy and practices. Service users’ individual and joint needs are met by appropriately trained staff. Service users benefit from well supported and supervised staff. The Commission considers Standards 35 the key standard to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 32,33 and 35. The home provides good continuity of care in terms of an experienced staff team who are trained appropriately to meet the needs of those being supported. EVIDENCE: The manager has worked with those being supported for a long time, both since they came to live at this home and previously. The one member of staff on duty at this time told the inspector that she is in her six-month probationary period and that she had completed one week’s induction training provided by New Support Options Ltd. It was reported that the home has vacancies for one full-time senior support staff and one part-time support worker. It was reported that two permanent staff have been recruited recently and that staff recruitment is ongoing. The staff member on duty at this time reported that she is doing Learning Disability Award Framework training and that once she completes this and her probationary period, she is going on to do NVQ Level 2 training. 7 Wychwood Close H57-H08 S13220 Wychwood Close V226428 090505 Stage 4.doc Version 1.30 Page 15 Conduct and Management of the Home The intended outcomes for Standards 37 – 43 are: 37. 38. 39. 40. 41. 42. 43. JUDGEMENT – we looked at outcomes for standard(s) 37, 38,41 and 42. Management is safeguarding the safety of those being supported by maintaining accurate and up to date records and this is a well-run home. Staff respect the independence, rights, choices, individuality and privacy of those being supported. EVIDENCE: The various records examined were noted to be of a high standard.
