Conditions of registration: 1. The service is registered to provide accommodation to a maximum of 34 service users in the category DE(E) (Dementia over the age of 65 years) 2. Within the maximum numbers accommodated, 3 service users may be accommodated in the category DE (Dementia under the age of 65 years) 3. The registered provider must, at all times, employ a suitably qualified and experienced manager who is registered with the Commission for Social Care Inspection. 4. The registered manager must satisfactorily complete NVQ level 4 in management by December 2004. Date of last inspection 20th January 2005 Brief Description of the Service: The Elizabethan Suite within Aaron Court nursing home is registered to accommodate 34 service users over 65 years of age diagnosed with dementia. The mental health facilities in the home comprises of two ground floor units of thirty single bedrooms, five of which have en suite facilities and two double bedrooms, one of which has an en suite facility. The registered facilities are close to the centre of Ellesmere Port with local shopping facilities available to service users. The service is located on a main road and has access to public transport, being on a main bus route to ellesmere port. The service is located on the ground floor of a two storey building, the first floor being given over to a separately registered service for older people in need of general nursing care. Also located on the ground floor are central facilities such as kitchen, laundry and administration. This service is provided by a private company that operate a number of registered care homes around the country. There are registered nurses on duty at all times. This is an overview of what the inspector found during the inspection. This inspection was carried out by one inspector over a period of two hours on the 10th June 200. At the same time another inspector carried out an unannounced inspection of the second registered service. The inspector was asked to assist with this inspection and therefore only spent limited time on the unit and assessed a much smaller number of standards and their outcomes for residents than had been originally planed. The inspection was carried out with the support of the manager. The inspection included the reading of three care plans, conversations with four residents and a tour of the home. At this inspection 9 of the 20 core standards were assessed together with an additional 10 of the remaining 18 standards. What the service does well: What has improved since the last inspection? The intended outcomes for Standards 1 – 6 are: 1. 2. 3. 4. 5. 6. Prospective service users have the information they need to make an informed choice about where to live. Each service user has a written contract/ statement of terms and conditions with the home. No service user moves into the home without having had his/her needs assessed and been assured that these will be met. Service users and their representatives know that the home they enter will meet their needs. Prospective service users and their relatives and friends have an opportunity to visit and assess the quality, facilities and suitability of the home. Service users assessed and referred solely for intermediate care are helped to maximise their independence and return home. The Commission considers Standards 3 and 6 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 3, 4 & 6 Assessment procedures before residents move into the home are thorough and allow family members to be part of the process of assessing needs. The home only admits those people whose needs are in keeping with the skills and knowledge of staff working within the home. Residents receive the care and support that they need from a well informed group of staff. EVIDENCE: Three residents’ care plans were examined, two of the care plans inspected were of residents who had been admitted to the home in recent months. These showed that a full medical and social worker assessment had been carried out prior to admission being agreed. These records also showed that family members had visited the home to discuss how the home could meet the needs of the residents after being admitted. The third care plan was for a resident who had been identified as being physically frail and was nursed in bed for large periods of the day. This care plan had clear and specific information regarding the type of care required from staff to properly support her within the home. The home does not provide intermediate care.

The intended outcomes for Standards 7 – 11 are: 7. 8. 9. 10. 11. The service user’s health, personal and social care needs are set out in an individual plan of care. Service users’ health care needs are fully met. Service users, where appropriate, are responsible for their own medication, and are protected by the home’s policies and procedures for dealing with medicines. Service users feel they are treated with respect and their right to privacy is upheld. Service users are assured that at the time of their death, staff will treat them and their family with care, sensitivity and respect. The Commission considers Standards 7, 8, 9 and 10 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 7 & 8 Care plans are maintained to a uniform standard, there is a good standard in how the files are maintained and the paperwork they contain. The complex and various needs of residents are identified within the care plans, with this information those needs may be properly met. EVIDENCE: Three residents’ case files were examined and contained detailed care plans that identified emotional, social and physical needs. These plans were up to date and were being regularly reviewed by senior staff. Where residents had been unable to contribute to the development and content of these plans their relatives had been invited to contribute on their behalf. Relatives had also been involved in the reviewing of these plans and had signed to confirm this. Where a significant risk had been identified this was included within the care plans and actions identified to minimise the risk or help the person were also written within the plan providing clear information for staff to follow.

Service users find the lifestyle experienced in the home matches their expectations and preferences, and satisfies their social, cultural, religious and recreational interests and needs. Service users maintain contact with family/ friends/ representatives and the local community as they wish. Service users are helped to exercise choice and control over their lives. Service users receive a wholesome appealing balanced diet in pleasing surroundings at times convenient to them. The Commission considers all of the above key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 12 & 13 Social activitiesare well managed, creating and providing daily variation and stimulation for people living within the home. Families regularly visit the home providing residents with opportunities to maintain family links. EVIDENCE: Relatives were seen visiting residents during the inspection. Staff were seen to be talking with relatives and making them welcomed into the home. There was an activities co-ordinator on duty who was seen to be working with residents in a small group. Residents were clearly comfortable with this activity and were seen to be responding to the questions being asked of them as part of a structured plan to address memory loss. Since the last inspection notice boards had been placed in the lounges of the home. Staff were writing the day, date and weather on these boards and using them to stimulate conversations with residents. Service users live in safe, comfortable bedrooms with their own possessions around them. Service users live in safe, comfortable surroundings. The home is clean, pleasant and hygienic. The Commission considers Standards 19 and 26 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 19, 20, 21, 22, 23, 24, 25 & 26 Improvements have been made to the décor and furnishings within the home. Residents bedrooms were comfortably furnished and contained various items of personal possessions. Residents live within a safe and well maintained home. EVIDENCE: This service is provided in a well-decorated and maintained home. Improvements have been made to what was a good standard of décor. The hot water temperature in the bathrooms and bedrooms was hand tested and found to be delivered at a safe temperature. All bathrooms were clean and suitable for residents, with two having mechanical hoists provided. The overall appearance of the home was good with no obvious odours or smells. Individual bedrooms are well decorated and personalised to reflect and express the character of the residents who occupy them.

JUDGEMENT – we looked at outcomes for standard(s) 27 & 28 A regular team of knowledgeable and experienced staff support residents living in this home. Ther needs of residents are being well met and stability in their mental health and wellbeing was obvious. Residents were observed as being calm and active with this staff support. EVIDENCE: On the afternoon of the inspection there were two registered nurses with specific mental health qualifications on duty. In addition there were four experienced health care assistants and an activity co-ordinator on duty till 20.00hs. Residents were observed to be mobile yet not wandering aimlessly. There was a calmness on both of the units with residents talking to each other and to staff in a very positive way. Service users live in a home which is run and managed by a person who is fit to be in charge, of good character and able to discharge his or her responsibilities fully. Service users benefit from the ethos, leadership and management approach of the home. The home is run in the best interests of service users. Service users are safeguarded by the accounting and financial procedures of the home. Service users’ financial interests are safeguarded. Staff are appropriately supervised. Service users’ rights and best interests are safeguarded by the home’s record keeping, policies and procedures. The health, safety and welfare of service users and staff are promoted and protected. The Commission considers Standards 33, 35 and 38 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 31 This is a well managed and organised service, in which the manager uses her skills, knowledge and experiences to lead and direct staff to provide good care and support to the residents. EVIDENCE: The manager was able to provide clear and current information about residents. The manager was using the skills and experiences of staff, both qualified nurses and care staff to provide care to residents. This was seen by their involvement in the records maintained in residents’ care plans. The manager had been open to comments made at the last inspection as to how the home could develop in its commitment to older people with dementia and had started to put them into place, an example of this was the use of conversation boards on a daily basis.
