What the care home does well

The home is well maintained, and decoration and furnishing are of a high standard. The home is well managed, and provides a range of training for staff. There is a good atmosphere in the home, and residents and staff get on well together.

What has improved since the last inspection?

Some bedrooms have benefited from new carpeting. Some pot plants have been placed in the conservatory and a resident has chosen to take responsibility for looking after these.

What the care home could do better:

Recruitment procedures need to be improved to better protect residents. An annual development plan to advise residents and others of improvements planned for the service should be provided. The home should be recording, reviewing and publishing views on the service offered by residents, their relatives, and others.

The home is close to the town centre, local shops and the seafront. Accommodation is provided for twelve service users in single rooms, all with en-suite toilet and some with bath and toilet. Service users are able to furnish their room to suit their taste. There is a large south-facing garden at the rear of the home laid to lawn, flowerbeds and patio. There is a large chalet in the garden that serves as an activities room. The home is joined by a corridor to the Sheltered Housing Scheme also run by Abbeyfield, but not required to be registered under the Commission for Social Care Inspection. Residents are able to take part and join in activities that take place there. The service is managed by Mrs Webster. The inspection was arranged to update assessments made at the previous inspection. The inspector sampled policies, procedures and records, interviewed two members of staff, the registered manager, and five residents, and carried out a partial tour of the premises. No service user moves into the home without having had his/her needs assessed and been assured that these will be met. Prospective service users and their relatives and friends have an opportunity to visit and assess the quality, facilities and suitability of the home. Service users assessed and referred solely for intermediate care are helped to maximise their independence and return home. JUDGEMENT – we looked at outcomes for the following standard(s): 1, 3, 6 There are good arrangements in place for residents’ needs to be assessed before admission is agreed. Outcomes for residents were seen as good. This judgement has been made from evidence gathered during the inspection, which included a visit to the service and takes into account the views and experiences of people using the service. EVIDENCE: There is an introductory pack for residents and prospective residents which includes a pocket guide, an application form, the service user guide and statement of purpose. There is a guest room which is made available for prospective residents who wish to have a trial stay. The service user’s health, personal and social care needs are set out in an individual plan of care. Service users’ health care needs are fully met. Service users, where appropriate, are responsible for their own medication, and are protected by the home’s policies and procedures for dealing with medicines. Service users feel they are treated with respect and their right to privacy is upheld. Service users are assured that at the time of their death, staff will treat them and their family with care, sensitivity and respect. The Commission considers Standards 7, 8, 9 and 10 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 7, 8, 9, 10 Residents health and care needs are being met by the home. Residents are encouraged to remain as independent as possible. Outcomes for residents were seen as good. This judgement has been made from evidence gathered during the inspection, which included a visit to the service and takes into account the views and experiences of people using the service. EVIDENCE: Three sets of care plans were seen which set out how a resident’s needs will be met in the home. Registered manager Mrs Webster said that she drafts the care plan in consultation with the resident. Care plans are being reviewed and updated as appropriate. Abbeyfield (Bognor Regis) Society Ltd DS0000014340.V293055.R01.S.doc Version 5.1 Page 11 Care records seen indicated that residents are being assisted to access a wide range of health services, and Mrs Webster attributed the longevity of residents in the home to the good atmosphere and care provided in the home. Good arrangements for the administration of medicines are in place, and for the locked storage of medicines. There is a contract with a local pharmacist to provide advice and visits to assess medicine administration systems in the home, and the report of such a visit carried out in November 2005 was seen. A number of residents have mobility, hearing or sight difficulties, and the home ensure that they have access to equipment they need. Staff also provided examples of how residents are encouraged to maintain their independence as far as possible. Examples of this given by staff included an observation that one resident “can’t make it to the bathroom, so get things ready for her, but she does for herself”, and that staff encourage residents to do buttons when they can, offering help if needed. Staff said they filled out menu choices with residents every week – some residents however fill out their own menu choices. One resident lays the tables in the dining room, and one resident has been arranging plants in the conservatory, and waters them as well. Service users find the lifestyle experienced in the home matches their expectations and preferences, and satisfies their social, cultural, religious and recreational interests and needs. Service users maintain contact with family/ friends/ representatives and the local community as they wish. Service users are helped to exercise choice and control over their lives. Service users receive a wholesome appealing balanced diet in pleasing surroundings at times convenient to them. The Commission considers all of the above key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 12, 13, 14. 15 Residents have opportunities for social contact and activities, exercise choice. Good and varied meals are provided. Outcomes for residents were seen as good. This judgement has been made from evidence gathered during the inspection, which included a visit to the service and takes into account the views and experiences of people using the service. EVIDENCE: Some activities, such as music and movement, are provided in the home, and some outings are arranged in conjunction with the residents who live in the adjacent sheltered accommodation house. There is a chalet in the garden used for arts and crafts. Some residents have chosen to take on tasks such as laying the table and watering the plants. Some residents walk into town or along the seafront, and others make use of their electric buggies to go out and about. Mrs Webster said that committee members attend residents’ meetings to hear their views. Mrs Webster said local produce is delivered twice per week. Menus were seen, and residents are being offered choice with meals, and fill out a form to say what their choice is. Two main choices were being offered at lunch on the day of the inspection. One diabetic is catered for, staff said she received desserts done with soya milk, and sugar free jellies. The lunch observed was unhurried and relaxed. Residents said the food was good. Service users and their relatives and friends are confident that their complaints will be listened to, taken seriously and acted upon. Service users’ legal rights are protected. Service users are protected from abuse. The Commission considers Standards 16 and 18 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 16, 18 Good arrangements are in place for the recording of complaints, and for staff to be trained in adult protection issues. Outcomes for residents were seen as good. This judgement has been made from evidence gathered during the inspection, which included a visit to the service and takes into account the views and experiences of people using the service. EVIDENCE: The complaints record was seen, but no complaints have been recorded since the previous inspection. Staff training certificates seen indicate that many staff have received training in adult protection. Mrs Webster said she was aware that local procedures in adult protection are changing, and that she has been advised of training in the new procedures being arranged by West Sussex County Council, and was planning for staff to attend this. The home is clean, pleasant and hygienic. The Commission considers Standards 19 and 26 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 19, 20, 21, 26 The home is in very good decorative order, is well furnished, and there are good communal facilities provided. Outcomes for residents were seen as good. This judgement has been made from evidence gathered during the inspection, which included a visit to the service and takes into account the views and experiences of people using the service. EVIDENCE: All bedrooms seen were in good decorative order, and had good quality furnishings and floorings. The main communal areas are the dining room, the sitting room and the conservatory. All were found to be comfortably furnished and homely, with good quality decoration and furniture. There is a nice garden with chairs and tables with umbrellas. The premises and grounds are being maintained well. Arrangements for clinical waste storage seen. There is a chalet in garden used for arts and crafts. All parts of the home visited were clean and hygienic. There are good communal toilet and bathroom facilities provided. All bedrooms have an en suite toilet, and four bedroom have an en suite bath. There is a visitor’s room with en suite facilities. There is a kitchen for use by residents or visitors on the first floor. Mrs Webster said each resident’s laundry is washed separately and kept in bedrooms and not in the laundry room, which helped ensure laundry didn’t get mislaid. Service users’ needs are met by the numbers and skill mix of staff. Service users are in safe hands at all times. Service users are supported and protected by the home’s recruitment policy and practices. Staff are trained and competent to do their jobs. The Commission consider all the above are key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 27, 28, 29, 30 Staff employed are receiving training, supervision and support to enable them to do their job well. Staff feel they are well managed. “PoVA First” checks need to be carried out before staff employment commences. If a CRB disclosure has not been received back for a member of staff before employment is commenced, published guidelines for ensuring the safety of residents (such as the employee not being left alone with residents) must be followed. Outcomes for residents were assessed as good. This judgement has been made from evidence gathered during the inspection, which included a visit to the service and takes into account the views and experiences of people using the service. EVIDENCE: Mrs Webster advised that 6 of the 14 care staff employed have achieved the National Vocational Qualification (NVQ) in care at level 2 or above. Abbeyfield (Bognor Regis) Society Ltd DS0000014340.V293055.R01.S.doc Version 5.1 Page 18 Two sets of recruitment records were seen for staff employed since the previous inspection, which included application forms, employment references, and CRB checks. It was noted that one member of staff has been employed without a CRB check being received, and one member of staff employed on the understanding that the CRB check from a previous job was transferable. The inspector advised that while a CRB disclosure is being awaited, the guidelines are that the employee must not be left alone with a vulnerable adult at any time. Mrs Webster said that all new staff were closely supervised for the first two weeks of their employment. Mrs Webster said that “PoVA First” checks were not carried out before staff were employed. One member of staff said that this was “the best home I’ve worked in”, and that the home is “very efficient on everything, like fire training, and everyone respects the manager”. Staff said the most recent staff meeting was 4 weeks ago, had been a useful meeting, and minutes of the meeting have been circulated. Staff said there were opportunities during the meeting to raise issues and discuss them, and that staff supervisions take place every 2 months. Staff are encouraged to participate in the running of the home, and one member of staff said “the manager gives us responsibility we can cope with, helps build up your confidence”. Service users live in a home which is run and managed by a person who is fit to be in charge, of good character and able to discharge his or her responsibilities fully. Service users benefit from the ethos, leadership and management approach of the home. The home is run in the best interests of service users. Service users are safeguarded by the accounting and financial procedures of the home. Service users’ financial interests are safeguarded. Staff are appropriately supervised. Service users’ rights and best interests are safeguarded by the home’s record keeping, policies and procedures. The health, safety and welfare of service users and staff are promoted and protected. The Commission considers Standards 31, 33, 35 and 38 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 31, 32, 33, 35, 38 The home is well managed and has a good atmosphere. An annual development plan and system for recording, reviewing and publishing views on the service should be developed. Outcomes for residents were seen as good. This judgement has been made from evidence gathered during the inspection, which included a visit to the service and takes into account the views and experiences of people using the service. EVIDENCE: Mrs Webster has advised the Commission of the most recent equipment and service checks carried out to ensure the safety of the premises and equipment. Staff interviewed said the home was well managed, and staff were given the opportunity to undertake responsibilities to assist them to develop their skills. Committee members attend residents’ meetings to hear their views, and the approach of the staff team and managers indicates that the needs of residents are being put first. Mrs Webster said there was not an annual development plan for the home, and that no system is in place to record the views of residents, their friends and family, and others on the service provided. Mrs Webster said the home does not manage the finances of the residents accommodated. However, the home does hold some money for one resident received from the person’s power of attorney, which is used to pay for personal items and services such as chiropody visits. Receipts and records for these transactions were sampled, and were in order.
