The inspector made no statutory requirements on the home as a result of this inspection and there were no outstanding actions from the previous inspection report.

What the care home does well

Looking after people well continues to be the theme at the home. It is clear that staff and residents have built up good relationships and there is a happy atmosphere in the home. The home has also worked very well in looking after a resident who they can no longer fully meet the needs of. An assessment from a specialist team is long overdue.

What has improved since the last inspection?

The home is constantly striving to improve the training of its staff team and there has been a considerable input into training since the last inspection.

What the care home could do better:

At the last inspection, the inspector was told that the home was replacing a lot of the carpets in resident`s rooms. Some of them had been replaced but due to an unforeseen problem with the water system, funds were diverted to its repair. The manager said that the remainder of the carpets were being replaced shortly and that rooms had been measured up and it was just a case of choosing patterns etc. The inspector expects that these will all have been replaced by the time of the next inspection.

The home is close to the town centre, local shops and the seafront. Accommodation is provided for twelve service users in single rooms, all with en-suite toilet and some with bath and toilet. Service users are able to furnish their room to suit their taste. There is a large south-facing garden at the rear of the home laid to lawn, flowerbeds and patio. There is a large chalet in the garden that serves as an activities room. The home is joined by a corridor to the Sheltered Housing Scheme also run by Abbeyfield, but not required to be registered under the Commission for Social Care Inspection. Residents are able to take part and join in activities that take place there. The service is managed by Mrs Webster. This inspection was announced and took place between 9-30am and 12-30pm. Abbeyfield has performed well over the past years and at the last inspection no requirements were made. During the course of this inspection the inspector spoke to residents, staff and the Registered Manager. Records and Care Plans were looked at and a tour of the home was conducted. There were ten residents in the home at the time of the inspection. Someone was visiting the home next week hoping to take up one of the two vacancies. Another resident had been waiting over five weeks for a re-assessment with a view to moving on to a home that could meet their needs more fully. The inspector would like to congratulate the home on the way that they have managed this resident who clearly needs more specialist help and feels that the particular social work team are slow to help in what has been a very difficult time for the home. The residents spoken to were very happy with the level of care that they were receiving. The inspector had sent out questionnaires to the residents and their families but at the time of the inspection, none had been returned. The inspector read a letter from a family thanking the staff for “a wonderful day” on the occasion of a 100th birthday celebration. It continued, “the kindness you show to all is marvellous”. Another resident said, “I am very happy here, where else could I be looked after better?” All records were found to be in order and up to date. The Care Plans were extremely well written and contained all of the information needed to look after the residents. They were reviewed regularly. Staff training was ongoing and it was noted that the majority of staff were either National Vocational Qualification (NVQ) trained or were on training. The building was warm, clean and welcoming. There were two care staff on duty with one domestic, a cook, a handyman and the administrator. The manager was also on duty along with her deputy. At night there was one staff awake on duty with another sleeping in on call. The rotas showed this to be the case. The home met all the requirements made by Environmental Health and the Fire Department and was considered a safe environment. No requirements were made at this inspection. Prospective service users have the information they need to make an informed choice about where to live. Each service user has a written contract/ statement of terms and conditions with the home. No service user moves into the home without having had his/her needs assessed and been assured that these will be met. Service users and their representatives know that the home they enter will meet their needs. Prospective service users and their relatives and friends have an opportunity to visit and assess the quality, facilities and suitability of the home. Service users assessed and referred solely for intermediate care are helped to maximise their independence and return home. The Commission considers Standards 3 and 6 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 1, 2, 3, 4, 5, 6 Residents and their families have the information they need to make an informed choice about where they live and are suitably assessed before placing. EVIDENCE: Each resident is given a contract when he/she enters the home. Most of the residents had family or friends who were able to act upon their behalf. The manager assesses all residents before they join the home either in their own home or in hospital. These completed assessments are kept in the Care Plan. The manager told the inspector about an assessment she was planning to do. The prospective resident was being invited to the home to have a look around before any decision would be made. The home does not take anyone in for intermediate care. The service user’s health, personal and social care needs are set out in an individual plan of care. Service users’ health care needs are fully met. Service users, where appropriate, are responsible for their own medication, and are protected by the home’s policies and procedures for dealing with medicines. Service users feel they are treated with respect and their right to privacy is upheld. Service users are assured that at the time of their death, staff will treat them and their family with care, sensitivity and respect. The Commission considers Standards 7, 8, 9 and 10 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 7, 8, 9, 10, 11 The resident’s health, personal and social care needs are well met through training and supervision of the staff. There is a policy for Death and Dying that is adhered to. EVIDENCE: Care Plans were well written and contained all the information necessary to look after the residents. They were updated regularly and reviewed at least once a month. The inspector looked at the Care plans of the residents he had spoken with and felt that the care given was accurately recorded. District nurses visit the home when requested and records are kept of all such visits. A district nurse was visiting a resident at the time of the inspection. Medication was stored safely and records were well kept in regard to the administering and disposing of them. The home had suffered the death of a long term resident during the last week. Staff were clearly upset and were supporting the family as best as they could. Residents were called by their chosen name; others preferred to be addressed as Mrs. The inspector spoke to residents in their rooms and privacy was observed. Service users find the lifestyle experienced in the home matches their expectations and preferences, and satisfies their social, cultural, religious and recreational interests and needs. Service users maintain contact with family/ friends/ representatives and the local community as they wish. Service users are helped to exercise choice and control over their lives. Service users receive a wholesome appealing balanced diet in pleasing surroundings at times convenient to them. The Commission considers all of the above key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 12, 13, 14, 15 Residents say that they are happy with their lifestyle, are able to keep in contact with their family and friends and able to have control over their daily tasks. Good home cooked food is provided. EVIDENCE: The home tries to provide as many activities as they possibly can. Often residents decide against going out on the morning of the arranged trip. Just two residents attended a recent trip to a local theatre. The weekly Music and Movement session is well attended as to is the monthly arts and craft session. There is a regular monthly church service with communion and quarterly fashion shows. The home has provided board games etc. but no residents feel the need to take up that facility. Friends and family are always welcome and one resident told me about her weekly visits from a son. Another spoke of her daughter visiting her. “They give us plenty of food” was a common theme. Residents look forward to their meals and on the day of the inspection they had a choice between Chicken and Mushroom Pie with potato, green beans and cauliflower or Lamb Curry. The food is cooked in the adjoining home and transported on a heated trolley to the dining area. No one needs any help with feeding and although special diets can be catered for, no one had any special requirements at the time of the inspection. Service users and their relatives and friends are confident that their complaints will be listened to, taken seriously and acted upon. Service users’ legal rights are protected. JUDGEMENT – we looked at outcomes for the following standard(s): 16, 17, 18 Any complaint is taken seriously and staff and residents know how to complain. Staff are well aware of the Adult Protection Procedures and a Whistle Blowing Policy is in evidence. EVIDENCE: All staff have attended the Adult Protection Training and the procedures are in the office. There have been no complaints made about the home to the CSCI. The home is clean, pleasant and hygienic. The Commission considers Standards 19 and 26 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 19, 20, 21, 22, 23, 24, 25, 26 The home is well maintained, safe and clean with no nasty smells. EVIDENCE: The residents all have single rooms with en-suite facilities. The rooms are well decorated and are generally light and airy. Several carpets need replacing and the inspector was told that this was next on the list to do! The inspector noted that on the second floor there was an Emergency Stair Climber just in case the lift was out of action for a while. There is a very comfortable lounge and dining area and conservatory for the residents to sit in. The inspector noted that the majority of the residents preferred to stay in their own rooms although they would go along to the dining room for lunch. The inspector observed the cleaner doing her rounds and every room was left clean and tidy. There were no nasty odours. Service users’ needs are met by the numbers and skill mix of staff. Service users are in safe hands at all times. Service users are supported and protected by the home’s recruitment policy and practices. Staff are trained and competent to do their jobs. The Commission consider all the above are key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 27, 28, 29, 30 There is usually sufficient staff employed to ensure that residents’ basic needs are met. The home has a sound recruitment policy and all checks are made. There has been considerable input into staff training. EVIDENCE: All staff have undergone Criminal Records Bureau (CRB) checks and copies that these were satisfactory were kept in the safe. Prospective staff are invited to complete an application form and attend an interview. The files showed that two references were obtained. There has been considerable staff training since the last inspection, staff attending Fire Lectures, Manual Handling, Infection Control, Continence, First Aid as well as several continuing with their National Vocational Qualification (NVQ) studies. Service users live in a home which is run and managed by a person who is fit to be in charge, of good character and able to discharge his or her responsibilities fully. Service users benefit from the ethos, leadership and management approach of the home. The home is run in the best interests of service users. Service users are safeguarded by the accounting and financial procedures of the home. Service users’ financial interests are safeguarded. Staff are appropriately supervised. Service users’ rights and best interests are safeguarded by the home’s record keeping, policies and procedures. The health, safety and welfare of service users and staff are promoted and protected. The Commission considers Standards 31, 33, 35 and 38 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 31, 32, 33, 34, 35, 36, 37, 38 The home is well managed by an experienced and competent person. It is apparent that the needs of the residents are paramount and that things happen for their benefit. The home has no dealings with any personal finances preferring to leave that to families or other professionals. EVIDENCE: Mrs Webster has managed the home for a number of years and has the respect of the residents and staff. One resident told the inspector that she was “very good, no wonderful”, whilst another said that she was “fair”. The home was constantly updating their practices to ensure that residents received the best care that they could. The home does not have any dealings with any money belonging to residents.
