This inspection was carried out on 19th December 2005.

The inspector made no statutory requirements on the home as a result of this inspection and there were no outstanding actions from the previous inspection report.

What the care home does well

Residents themselves said that they were happy at the home and the staff were nice. Relatives said that the care delivered by staff was very good. A friendly, lively and welcoming feel was evident in Alpine Lodge. Residents and relatives spoke positively about the amount of activities available both within the home and on trips outside the home. Numerous activities were advertised on the notice board in the home. The home was clean, with no unpleasant odours noticeable. Residents said that the home was always kept clean.

What has improved since the last inspection?

The presentation of the liquidised meals had improved. There had been an improvement in the recording systems of the fire drill records. Staff were receiving training on how to safely move and handle residents at the required frequency.

What the care home could do better:

Medication must be safely and securely stored around the home. Staff need to ensure that all residents bed linen is clean and the pillows on the beds are comfortable. Records containing information about residents must be securely stored in the home.

Within the home is a twenty-bedded unit for people with dementia. Alpine Health Care Limited owns the home. The home is located in Stocksbridge, northwest Sheffield, opposite a school, within walking distance of shops and the bus route. The home is a purpose built two-storey building. All bedrooms are single occupancy and have en-suite facilities. The home is comfortably furnished and well decorated with an appropriate number of lounge and dining areas. This was an unannounced inspection, which in the main was carried out to check requirements made at the last inspection. The inspection took place from 09:50 to 12:50. Annie Kudangirana, registered manager was present during the inspection. Opportunity was taken to make a partial tour of the premises, inspect a sample of records and policies and talk to 5 staff, 4 visiting relatives and 9 residents. The inspector wishes to thank the staff, relatives and residents for their time, friendliness and co-operation throughout the inspection process. Prospective service users have the information they need to make an informed choice about where to live. Each service user has a written contract/ statement of terms and conditions with the home. No service user moves into the home without having had his/her needs assessed and been assured that these will be met. Service users and their representatives know that the home they enter will meet their needs. Prospective service users and their relatives and friends have an opportunity to visit and assess the quality, facilities and suitability of the home. Service users assessed and referred solely for intermediate care are helped to maximise their independence and return home. The Commission considers Standards 3 and 6 the key standards to be inspected at least once during a 12 month period. The service user’s health, personal and social care needs are set out in an individual plan of care. Service users’ health care needs are fully met. Service users, where appropriate, are responsible for their own medication, and are protected by the home’s policies and procedures for dealing with medicines. Service users feel they are treated with respect and their right to privacy is upheld. Service users are assured that at the time of their death, staff will treat them and their family with care, sensitivity and respect. The Commission considers Standards 7, 8, 9 and 10 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): Standards 8,9 and 10. Residents themselves said that they were happy at the home and the staff were nice. Relatives said that the care delivered by staff was very good. Some medication practices provide a risk to the residents’ health and welfare. EVIDENCE: Residents said that they were happy and that the staff were nice. Relatives said that they thought the staff of the home were” wonderful” and that the care delivered by them was “very good”. Residents were well dressed in clean clothes and had received a good standard of personal care. Staff were observed to be assisting residents in a positive and friendly manner, doors were closed where staff were helping with personal care. Medicines were not all securely stored around the home. Antibiotics, in a liquid form, were found in the fridge in the unlocked kitchenette servery room. The antibiotics were handed over to the manager who immediately locked them away in the treatment room fridge. Service users find the lifestyle experienced in the home matches their expectations and preferences, and satisfies their social, cultural, religious and recreational interests and needs. Service users maintain contact with family/ friends/ representatives and the local community as they wish. Service users are helped to exercise choice and control over their lives. Service users receive a wholesome appealing balanced diet in pleasing surroundings at times convenient to them. The Commission considers all of the above key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): Standards 12,13,14 and 15. Residents had a choice of lifestyle within the home and they were able to maintain contact with family and friends. Meals served at the home were of a good quality and offered choice. EVIDENCE: A friendly, lively and welcoming feel was evident in Alpine Lodge. Residents and relatives spoke positively about the amount of activities available both within the home and on trips outside the home. Numerous activities were advertised on the notice board in the home. Residents said that they were looking forward to some concerts that had been arranged including a planned carol concert by visiting school children. Residents said that they were able to maintain contact with their family and friends. Relatives said they were always made to feel welcome when they visited. Residents said they chose when they got up and went to bed and had a choice of food at mealtimes. Lunch was served in a pleasant relaxed manner and the residents said that they enjoyed their lunch. The appearance of the pureed diets had improved since the last inspection .the meat and vegetables were pureed individually and served in three separate portions on a plate. Service users live in a safe, well-maintained environment. Service users have access to safe and comfortable indoor and outdoor communal facilities. Service users have sufficient and suitable lavatories and washing facilities. Service users have the specialist equipment they require to maximise their independence. Service users’ own rooms suit their needs. Service users live in safe, comfortable bedrooms with their own possessions around them. Service users live in safe, comfortable surroundings. The home is clean, pleasant and hygienic. The Commission considers Standards 19 and 26 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): Standards 19,24 and 26. The environment within the home was on the whole well maintained and clean providing a comfortable, safe environment for residents. EVIDENCE: Eight bedrooms were checked in detail and many others seen, all were comfortable and homely. Residents said their beds were comfortable. Bed linen checked was generally clean and in a good condition. However some marks were found on pillows on Ullswater unit. Also several pillows were found to be very lumpy which would make them uncomfortable for the residents. The home was clean, with no unpleasant odours noticeable. Residents said that the home was always kept clean. Relatives said that they were impressed with the cleanliness of the home. The home was warm in all areas. Window restrictors were fitted to all windows checked. This will assist in maintaining resident safety. Service users’ needs are met by the numbers and skill mix of staff. Service users are in safe hands at all times. Service users are supported and protected by the home’s recruitment policy and practices. Staff are trained and competent to do their jobs. The Commission consider all the above are key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): Standard 27. Staff were employed in sufficient numbers. EVIDENCE: The manager stated that agreed staffing levels were being maintained. The staff rota identified agreed staffing levels had been met. This will assist in making sure that residents’ needs are met. With the exception of staff sickness, staff and relatives said staffing levels were generally adequate. Service users live in a home which is run and managed by a person who is fit to be in charge, of good character and able to discharge his or her responsibilities fully. Service users benefit from the ethos, leadership and management approach of the home. The home is run in the best interests of service users. Service users are safeguarded by the accounting and financial procedures of the home. Service users’ financial interests are safeguarded. Staff are appropriately supervised. Service users’ rights and best interests are safeguarded by the home’s record keeping, policies and procedures. The health, safety and welfare of service users and staff are promoted and protected. The Commission considers Standards 31, 33, 35 and 38 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): Standards 31,32,37 and 38. There was a positive style of management in the home and staff moral was good. Residents’ confidentiality was not fully protected. The homes policies and procedures promoted the health, safety and welfare of residents and staff. EVIDENCE: Residents said that the manager was approachable and helpful. The manager said that she had completed her NVQ level 4 in management and care. However she had not yet received certification to support this. The manager should be commended for achieving this qualification. Residents’ confidentiality was not fully protected, as not all their records were securely stored. A file containing resident information was left unattended in a lounge. Records checked showed that staff had received recent moving and handling training. Practice fire drills had been conducted in the home and the records identified the length of the drill and corrective action needed after the drill .The drills were conducted at different times of the day. This will promote the safety and welfare of the service users. At the time of inspection no fire exits were blocked and window restraints were in situ at first floor windows checked to prevent falls. Hazardous products were safely stored in the home. This will promote the safety and welfare of the service users.
