This inspection was carried out on 9th May 2005.

The inspector made no statutory requirements on the home as a result of this inspection and there were no outstanding actions from the previous inspection report.

What the care home does well

The home offers a friendly environment where the residents feel safe and well cared for. The meals are of good quality and several of the residents commented on how much they enjoyed the meals and how much they looked forward to them.

What has improved since the last inspection?

Some service users have moved bedrooms to enable a laundry shoot to be constricted between the ground floor and the basement laundry. Once this is completed it should mean that the laundry can be transported into the laundry in a more hygienic manner which will also be safer for the staff than the current staircase.

What the care home could do better:

Where a complaint is made against a member of staff provided by an Agency, the home should take a more active role in ensuring that the complaint is properly investigated and the resolution passed on to the complainant within a reasonable space of time.

Date of last inspection 16 November 2004 Brief Description of the Service: Amethyst House is a care home for older people providing personal and nursing care, registered for 39 service users.The accommodation is a converted older building with a new extension.The home is situated in the village of Rossington, near Doncaster.The home has well maintained gardens that are level, save and accessible to people who use wheelchairs or have mobility problems.The home is owned by Ashbourne Homes. This was an unannounced inspection which happened to be done on the same day that the company undertook it’s own audit of the home. The inspection started at 09:00, lasted for 5 hours, and included observation of both breakfast and lunch. Three visitors, and nine residents were asked for their impressions of the care delivered by the home and all indicated their satisfaction. Because the two inspections coincided some of the residents became confused as to who people were, but it was felt that it was still able to get accurate information about how happy they were with the care they received. Prospective service users have the information they need to make an informed choice about where to live. Each service user has a written contract/ statement of terms and conditions with the home. No service user moves into the home without having had his/her needs assessed and been assured that these will be met. Service users and their representatives know that the home they enter will meet their needs. Prospective service users and their relatives and friends have an opportunity to visit and assess the quality, facilities and suitability of the home. Service users assessed and referred solely for intermediate care are helped to maximise their independence and return home. The Commission considers Standards 3 and 6 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 6 The home does not offer Intermediate care. EVIDENCE: The home is not registered to provide Intermediate Care and the facilities do not meet with the National Minimum Standards for such a service. The service user’s health, personal and social care needs are set out in an individual plan of care. Service users’ health care needs are fully met. Service users, where appropriate, are responsible for their own medication, and are protected by the home’s policies and procedures for dealing with medicines. Service users feel they are treated with respect and their right to privacy is upheld. Service users are assured that at the time of their death, staff will treat them and their family with care, sensitivity and respect. The Commission considers Standards 7, 8, 9 and 10 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 10 The staff follow the principals of ensuring the residents benefit from as much privacy as possible, whilst still attending to their care needs. The resident’s dignity was preserved by staff, and an atmosphere of mutual respect was maintained during the inspection. EVIDENCE: When residents needed personal care this was delivered away from the communal areas and other residents so that their privacy is not breached. There were examples seen where staff were giving residents plenty of time to complete what they were doing without being rushed or confused. Residents said that the staff would knock on their bedroom doors and wait to be invited in. This indicates a commitment to the resident maintaining control of that environment whilst they were in the room. Service users find the lifestyle experienced in the home matches their expectations and preferences, and satisfies their social, cultural, religious and recreational interests and needs. Service users maintain contact with family/ friends/ representatives and the local community as they wish. Service users are helped to exercise choice and control over their lives. Service users receive a wholesome appealing balanced diet in pleasing surroundings at times convenient to them. The Commission considers all of the above key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 13,15 The visiting hours of the home are not restrictive and that visitors are encouraged. The meals are well prepared and presented so that the residents can have a good choice of food and a healthy diet. EVIDENCE: One visitor confirmed that they came into the home regularly and that they were always made welcome, both by the staff and the residents. Another visitor said that the only restrictions on visiting times were at meal times and late at night, they felt that this was reasonable and that it was not a problem. The meals were well cooked and presented so that the residents had a good choice of what to eat and were encouraged to eat heartily. Residents confirmed how much they looked forward to their meals and how much they enjoyed them. Service users and their relatives and friends are confident that their complaints will be listened to, taken seriously and acted upon. Service users’ legal rights are protected. Service users are protected from abuse. The Commission considers Standards 16 and 18 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 16 There is a good complaints procedure within the company but more care needs to be taken to ensure that complaints made against agency staff are adequately investigated. EVIDENCE: An instance was noted where a complaint had been made against a member of staff provided by an Agency where there was no record in the home that the complaint had been investigated satisfactorily, nor was there any record of the outcome. This situation needs to be rectified to ensure that the company is satisfied that the residents are cared for in an appropriate manner. Service users have sufficient and suitable lavatories and washing facilities. Service users have the specialist equipment they require to maximise their independence. The home is clean, pleasant and hygienic. The Commission considers Standards 19 and 26 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 19 The environment is well maintained and there is a regular maintenance schedule for keeping the building up to standard. EVIDENCE: The grounds were well maintained and accessible to service users who have mobility problems. Several of the residents said that they enjoyed going into the gardens and that they also enjoyed watching the wildlife that was attracted by the food that was put out by the staff. There was some minor damage noted that had probably been caused by wheelchairs but the handyman had a schedule to periodically touch these areas up with paint. Service users needs are met by the numbers and skill mix of staff. Service users are in safe hands at all times. Service users are supported and protected by the home’s recruitment policy and practices. Staff are trained and competent to do their jobs. The Commission considers Standards 27, 29, and 30 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 27 There was an adequate number of staff on duty to meet the needs of the residents. The staff members had different skills and a lot of experience in dealing with the needs of the resident group. EVIDENCE: There is a commitment to National Vocational Qualification training from both the company and the staff in order that their knowledge base will be increased. Less experienced staff said how much they benefited from working with more experienced people whilst learning the job. Staff were heard discussing resident needs in a knowledgeable manner with colleagues. Service users live in a home which is run and managed by a person who is fit to be in charge, of good character and able to discharge his or her responsibilities fully. Service users benefit from the ethos, leadership and management approach of the home. The home is run in the best interests of service users. Service users are safeguarded by the accounting and financial procedures of the home. Service users’ financial interests are safeguarded. Staff are appropriately supervised. Service users’ rights and best interests are safeguarded by the home’s record keeping, policies and procedures. The health, safety and welfare of service users and staff are promoted and protected. The Commission considers Standards 33, 35 and 38 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 38 There is a continuing concern about staff safety regarding taking dirty washing into and clean washing out of the laundry because of the steepness of the staircase that has to be used. There were no other concerns identified with the safe operation of the home or the appropriate moving and handling techniques of the staff. EVIDENCE: Staff were seen taking washing into the laundry in a manner that put a lot of strain on their back possibly leading to an injury. However, there are plans to rectify this situation with the installation of a shoot. There were good techniques observed when residents needed help with getting in and out of chairs. This meant that the staff were minimising the risk of injury and the residents felt safe.
