It is a Georgian, circa 1830, stone built country house, previously a private dwelling and is situated in a rural setting on the outskirts of the market town of Settle in the Yorkshire Dales. There are two floors including ground floor and a vertical lift provides level access to the first floor. It retains many of its original features including high ceilings and chandeliers and has extensive gardens, which are well maintained. This is an overview of what the inspector found during the inspection. This report relates to an unannounced inspection that took place on Tuesday 20th December 2005 April. The manager, Mrs Felicity Ann Kirby was available to assist with the process throughout. Mrs Wanda Carr deputy manager was also present until lunchtime. The inspection lasted 4.5 hrs (10.30 am to 3pm). There were 35 residents in the home with one resident in hospital and all were in receipt of nursing care. The inspection focused on some of the key standards as well as addressing issues, which were identified at the last inspection. The manager and all staff spoken to provided information in an open and professional manner and without reservation. Conversations with the manager ranged wide and included many professional matters including, funding and assessment issues and the inspector agreed to send the manager further information, which will assist her with these. An inspection of some areas of the premises took place, including bedrooms, lounge and dining areas, bathroom and kitchen. Discussions took place with the manager, carers, the cook and residents. What the service does well:
The home provides a good quality of care in a homely and professional manner. All the staff works as a well-integrated team and the rapport between the staff and manager was observed to be very positive. Conversations with the staff confirmed this impression. There are good systems of documentation in relation to care planning, policies and procedures. This was particularly evident in the documentation of a pressure sore which is being resolved satisfactorily. All residents spoken to indicated their contentment at the quality of the care provided by the nurses and care staff and also commented favourably on the quality of the food. The input from the activities organiser is particularly appreciated and favourably commented upon by staff and residents alike. The intended outcomes for Standards 1 – 6 are: 1. 2. 3. 4. 5. 6. Prospective service users have the information they need to make an informed choice about where to live. Each service user has a written contract/ statement of terms and conditions with the home. No service user moves into the home without having had his/her needs assessed and been assured that these will be met. Service users and their representatives know that the home they enter will meet their needs. Prospective service users and their relatives and friends have an opportunity to visit and assess the quality, facilities and suitability of the home. Service users assessed and referred solely for intermediate care are helped to maximise their independence and return home. The Commission considers Standards 3 and 6 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 3. The resident’s needs are assessed which permit the care plans to be developed so that their needs can be met. EVIDENCE: The assessment documentation inspected was good and included sufficient detail so that residents needs are identified and upon which information the care plans are developed. The intended outcomes for Standards 7 – 11 are: 7. 8. 9. 10. 11. The service user’s health, personal and social care needs are set out in an individual plan of care. Service users’ health care needs are fully met. Service users, where appropriate, are responsible for their own medication, and are protected by the home’s policies and procedures for dealing with medicines. Service users feel they are treated with respect and their right to privacy is upheld. Service users are assured that at the time of their death, staff will treat them and their family with care, sensitivity and respect. The Commission considers Standards 7, 8, 9 and 10 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 7, 8 and 10. The physical and health needs of the residents are identified and appropriate care plans created to ensure they are met. EVIDENCE: The home utilise the Kardex system of documentation and individual plastic folders are available for each of the residents and into which all information relating to them is located. The documentation includes admission details, assessments, care plans, daily statements, nutritional assessments and weight charts. The Waterlow assessments for risk from pressure sores were relevant and appropriately documented. There is only one resident with a pressure sore and the particular assessment and strategy to resolve it was of a very high quality, with diagrammatic illustration and progress in resolution recorded. This was a particularly challenging case and the professional approach in meeting the problem it is a tribute to the professional approach of the nursing staff in the home. There is a key worker system whereby the care staff are allocated a small group of residents to care for.

The resident’s privacy and dignity are respected, with the staff referring to them in appropriate terms and also observed to knock on room doors prior to entering on invitation. Each of the residents rooms are provided with lockable space so that they may store their valuables or medication in safety should they so wish. The intended outcomes for Standards 12 - 15 are: 12. 13. 14. 15. Service users find the lifestyle experienced in the home matches their expectations and preferences, and satisfies their social, cultural, religious and recreational interests and needs. Service users maintain contact with family/ friends/ representatives and the local community as they wish. Service users are helped to exercise choice and control over their lives. Service users receive a wholesome appealing balanced diet in pleasing surroundings at times convenient to them. The Commission considers all of the above key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 12,13 and 14. The resident’s wishes are respected at all times and contact with relatives and friends is encouraged. EVIDENCE: The resident’s routines of daily living are tailored to their needs and choice and there is an activities organiser to assist with in social activities. The activities organiser’s work is important and particularly well appreciated both by the residents and staff spoken to during the inspection. The organiser works for 20 hrs per week and makes it her job to identify those who may require a visit in their rooms and help either with conversation or assistance with letters or other private matters. During the inspection the residents were being assisted in making their own Christmas cards. Entertainers are arranged on a regular basis and outings are also organised with the use of a mini–bus that is made available on request. The policy on visiting and encouraging contact with relatives and friends is included in the home’s Statement of Purpose. The residents personal monies is managed either by themselves or their relatives and the home has no responsibility for any residents monies at the present time. The Commission considers Standards 16 and 18 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 18. The policies and procedures and training of the staff in adult protection issues safeguard the residents from abuse. EVIDENCE: Adult Protection and Whistle Blowing procedures are available to protect the residents. A carer who had completed the NVQ Level 2 confirmed that adult abuse issues were included, as part of this course and that she had recently attended training on the subject of aggressive behaviour. There is a system whereby the staff sign to evidence they have seen and understood the home’s policies and procedures and this accords with good recommended practice. The Commission considers Standards 19 and 26 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 19, 21. The environmental standards of the home are satisfactory. EVIDENCE: The home is set in very pleasant grounds and these are kept in a good condition with easy access by wheelchair being provided. The home is well maintained and issues relating to the floor surfaces and furniture raised at the last inspection have been satisfactorily addressed. There were no malodours detected on the day of inspection. Improvement in the easy management of the residents in getting into and out of the baths would be facilitated by the provision of a power-assisted hoists rather than the ‘wind-up’ design presently provided.

The residents receive a good standard of care from staff who are appropriately screened, trained and supervised. EVIDENCE: The recruitment procedures and records kept of staff at the home meet the required standard. Appropriate Criminal Record Bureau (CRB) and Protection of Vulnerable Adults (POVA) checks are completed for all staff. There is a training programme in place for the care staff, which meets the National Training Organisation (NTO) standards. Service users’ rights and best interests are safeguarded by the home’s record keeping, policies and procedures. The health, safety and welfare of service users and staff are promoted and protected. The Commission considers Standards 31, 33, 35 and 38 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 35 and 38. The home is well managed with staff that is kept informed and consulted on developments in the home. There are systems in place to ensure that the Health & Safety of the residents and staff are protected. EVIDENCE: The residents personal monies is managed either by themselves or their relatives and the home has no responsibility for any residents monies at the present time. The staff receives training in the areas of Health & Safety, First Aid, Fire Safety and Moving and Handling at the relevant recommended intervals. Regular servicing of gas boilers, lifts, hoists and other equipment takes place and the records of these are kept in an easily accessible ring folder.
