Conditions of registration: 1. The service must at all times employ a suitably qualified and experienced manager who is registered with the Commission for Social Care Inspection. 2. A maximum of twenty eight older people (28OP) may be accommodated of whom twelve may have dementia (12DE(E)). 3. The staffing levels for the home must meet the Residential Forum Staffing Formula for Older Adults. 4. When single rooms of less than 12 sqm become vacant, they must not be used to accommodate wheelchair users and where wheelchair users are in bedrooms of less than 12 sqm they must be given the opportunity to move to a larger room when one becomes available. 5. Two service users may share a bedroom of at least 16 sqm useable floor space only if they have made a positive choice to do so, and when a shared space becomes vacant the remaining service user has the opportunity to choose to share, moving to a different room if necessary. Date of last inspection 26 January 2005 Brief Description of the Service: Applethwaite Green is a residential care home registered with the Commission for Social Care Inspection to provide care and accommodation for up to twenty-eight residents. Accommodation is provided in three living units, one of which specialises in providing care for people with dementia. The home is owned by Cumbria County Council and operated on its behalf by Cumbria Care, a business unit of the County Council. The home is a purpose built two-storey building located on the outskirts of Windermere and is close to shops, pubs, a post office and other amenities. Accommodation for residents is provided on the ground and first floor of the home and all bedrooms are for single occupancy. Three bedrooms have ensuite toilet and bathing facilities and there are suitable toilets close to all the accommodation used by residents. The home is set in an attractive location with pleasant and safe garden areas with views towards the town and a playing field. This is an overview of what the inspector found during the inspection. The inspection was unannounced carried out on Wednesday 21st September 2005. The inspection focussed on how well the services provided meet the needs of the residents living in the home. This was assessed by speaking to residents and staff, observing activity in the home and examining the records which care homes are required to hold. A number of areas of the home should be redecorated in order to provide a comfortable and pleasant environment for residents to live in. Toilets and bathrooms need to be cleaned more thoroughly and the scuffmarks on the assisted baths should be repaired. An assessment of the risk to residents when using the assisted baths should be carried out to ensure their safety. One assisted bath could not be used as the hoist was out of order, this hoist must be repaired or replaced so that residents can use the bathroom safely. Appropriate storage areas must be provided for equipment away from those areas used by residents. The intended outcomes for Standards 1 – 6 are: 1. 2. 3. 4. 5. 6. Prospective service users have the information they need to make an informed choice about where to live. Each service user has a written contract/ statement of terms and conditions with the home. No service user moves into the home without having had his/her needs assessed and been assured that these will be met. Service users and their representatives know that the home they enter will meet their needs. Prospective service users and their relatives and friends have an opportunity to visit and assess the quality, facilities and suitability of the home. Service users assessed and referred solely for intermediate care are helped to maximise their independence and return home. The Commission considers Standards 3 and 6 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 3, 4 and 6. The home has a good admissions procedure and residents know that the services provided will meet their needs. The home does not provide intermediate care. EVIDENCE: Residents’ needs are assessed before they are offered accommodation in the home to ensure that the services and facilities provided are suitable to provide the support they need. The home provides a high standard of care and residents’ needs and preferences are known to the staff who support them. Residents said “I can’t think of anything that would make this place better”. Applethwaite Green does not provide intermediate care and standard 6 does not apply to the home. Service users’ health care needs are fully met. Service users, where appropriate, are responsible for their own medication, and are protected by the home’s policies and procedures for dealing with medicines. Service users feel they are treated with respect and their right to privacy is upheld. Service users are assured that at the time of their death, staff will treat them and their family with care, sensitivity and respect. The Commission considers Standards 7, 8, 9 and 10 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 7, 8 and 10. Residents’ health and personal care needs are identified and met by the services provided. Residents are treated with respect and their privacy and dignity are protected. EVIDENCE: Records are held of residents’ health and personal care needs and staff are aware of the support which individuals need. Residents are encouraged to self-care as far as they are able so that they maintain their independence. Health care services are provided by local GP practices and specialist health services and residents’ health and wellbeing are maintained. The home focuses on treating people as individuals and their preferences and opinions are taken into account. Care staff treat residents with respect and take appropriate actions to protect their privacy and dignity. Service users find the lifestyle experienced in the home matches their expectations and preferences, and satisfies their social, cultural, religious and recreational interests and needs. Service users maintain contact with family/ friends/ representatives and the local community as they wish. Service users are helped to exercise choice and control over their lives. Service users receive a wholesome appealing balanced diet in pleasing surroundings at times convenient to them. The Commission considers all of the above key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 13 and 14. Residents are supported to make choices about their lives and the decisions they make are respected. EVIDENCE: Residents make choices about their lives in the home including how they spend their time and the visitors they receive. The choices residents make are respected by the staff in the home. Visitors are made welcome and residents are invited to attend events in the local community. Residents said “I choose what I do just like when I was at home”. JUDGEMENT – we looked at outcomes for standard(s) 16 Residents know how they can raise concerns about the services in the home and that their complaints will be handled appropriately. EVIDENCE: Residents have been given information about how to make complaints about the home and a copy of the complaints procedure is displayed in the entrance hall. Residents said they had raised concerns with the supervisors in the home and had been satisfied with the response given. The complaints procedure details agencies outside of home to whom complaints can be made including the contact details of the Commission for Social Care Inspection local office. Service users live in safe, comfortable bedrooms with their own possessions around them. Service users live in safe, comfortable surroundings. The home is clean, pleasant and hygienic. The Commission considers Standards 19 and 26 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 19, 21, 22, 23, 24 and 26. Although the home is generally well maintained there are a number of areas which require attention in order to provide clean and comfortable accommodation for residents. EVIDENCE: Applethwaite Green was purpose built as a care home for older people and the accommodation is suitable to meet the needs of the residents living there. The home has a maintenance plan which includes the planned redecorated of a number of bedrooms, hallways and stairwells. A number of areas of the home are rundown and in need of redecoration. Residents said they are “very happy” with their bedrooms and that they had been able to being their own possessions into their rooms when they moved in. The home has three assisted bathrooms, one of which could not be used safely as the ambulift hoist was out of order and was being stored in the bathroom. All the assisted baths had been scuffed by the hoist seats. The marks on the baths must be repaired and the risks to residents using the hoists must be assessed. Bathrooms and toilets are unhygienic and require thorough cleaning.

Care staff are trained and competent and provide a high standard of care to residents. EVIDENCE: Care staff have received a range of training to give them the skills and competence to provide a high standard of care. Residents receive the support they need in a manner which takes into account their personal preferences and respects their rights and dignity. Residents are comfortable around the staff and supervisors in the home and requests for assistance are answered promptly. Service users’ rights and best interests are safeguarded by the home’s record keeping, policies and procedures. The health, safety and welfare of service users and staff are promoted and protected. The Commission considers Standards 33, 35 and 38 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 32, 33, and 35. The home is well run and residents’ rights and interests are protected. EVIDENCE: The atmosphere in the home is welcoming and relaxed and residents know they can affect how the home is run. The home has systems in place to obtain the views of residents and visitors about the services provided. There are good procedures in place to ensure that residents’ safety, welfare and financial interests are protected. STATUTORY REQUIREMENTS This section sets out the actions which must be taken so that the registered person/s meets the Care Standards Act 2000, Care Homes Regulations 2001 and the National Minimum Standards.
