Conditions of registration: The home is registered for a maximum of 24 service users to include up to four service users in the category of DE(E), up to four service users in the category of PD and up to 24 service users in the category of OP. The service should, at all times, employ a suitably qualified and experienced manager who is registered with the Commission for Social Care Inspection. Date of last inspection N/A Brief Description of the Service: Bankfield House is a large detached property standing in its own grounds in the Woodley area of Hyde. Its placement is secluded and private, offering residents peaceful surroundings. The home currently provides accommodation for 24 residents over the age of 65 years. There is a small conservatory for those residents wishing to smoke and two lounges and an additional conservatory which are non smoking. The dining room is used at meal times and throughout the day for visiting and activities. Bedroom accommodation offers single and double rooms. This is an overview of what the inspector found during the inspection. The inspection of Bankfield was announced and completed over two days, starting at 8am on the first day and 10am on the second, with a total of ten hours on the premises. This was the first inspection under the new ownership; the home had prepared well for the inspection completing and returning a pre-inspection questionnaire and providing other required documents and records. Prior to the inspection the inspector spoke with one relative regarding the services at the home and received comment cards from three residents and one relative. One comment card was received from a general practitioner who visits the home. The inspector spent time sitting and talking to residents and was able to share two mealtimes with them. The care of two residents was specifically looked at, with their individual care plans being inspected and observations being made of the care they received. Overall the inspection was positive with the new owners actively attempting to develop all aspects of the service for the benefit, comfort and pleasure of residents. Verbal feedback of the findings of the inspection was given to the registered providers during and at the end of the inspection. What the service does well:
The home continues to offer residents a consistent service. Residents stated they felt well cared for and supported by “good staff”. Residents’ clothing was clean and well presented with care and attention given to the co-ordination of clothing. The health needs of residents were maintained with arrangements in place to receive specialist support and equipment where required. All residents spoken to stated they felt well cared for and supported. The intended outcomes for Standards 1 – 6 are: 1. 2. 3. 4. 5. 6. Prospective service users have the information they need to make an informed choice about where to live. Each service user has a written contract/ statement of terms and conditions with the home. No service user moves into the home without having had his/her needs assessed and been assured that these will be met. Service users and their representatives know that the home they enter will meet their needs. Prospective service users and their relatives and friends have an opportunity to visit and assess the quality, facilities and suitability of the home. Service users assessed and referred solely for intermediate care are helped to maximise their independence and return home. The Commission considers Standards 3 and 6 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 2, 3, 4 & 5. Standard 6 is not applicable to Bankfield Residents have their care needs assessed and have the opportunity to visit the home, receive information and assess the service prior to making any decisions about their future. EVIDENCE: Inspection of three residents’ files confirmed that the registered manager visits the residents prior to them making any decisions about moving into the home. Files contained an assessment of need, contracts and letters of confirmation from the home that they could meet the needs of the resident. One resident explained that her son and daughter had acted on her behalf and visited the home to view the services on offer and view a room. Some records stated that residents and/or their relative had taken the opportunity to visit the home prior to making any decisions about the future. The intended outcomes for Standards 7 – 11 are: 7. 8. 9. 10. 11. The service user’s health, personal and social care needs are set out in an individual plan of care. Service users’ health care needs are fully met. Service users, where appropriate, are responsible for their own medication, and are protected by the home’s policies and procedures for dealing with medicines. Service users feel they are treated with respect and their right to privacy is upheld. Service users are assured that at the time of their death, staff will treat them and their family with care, sensitivity and respect. The Commission considers Standards 7, 8, 9 and 10 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 7, 8, 9 & 10 Residents had care plans in place and received appropriate support to maintain, as far as possible, good health. Medication records were maintained correctly. Some care practices compromised residents’ dignity and privacy. EVIDENCE: All residents’ comment cards stated they felt well cared for. All residents spoken to stated they were supported well and received their medicines each day and that doctors and district nurses visited them at the home. Observations of the district nurse support were that treatments were given in private. Medication records were correctly maintained. Care staff were observed weighing residents in the lounge area, eye creams were also administered at the dining table and other practices which should have been completed discreetly were completed in view of others.

Comments received from a medical practitioner stated that, in the main, the home communicates clearly and works together for the benefit of residents’ health. Comments were also made that the home demonstrates a clear understanding of residents’ health care needs and that it makes appropriate decisions when they can no longer manage the care needs of a resident. The medical practitioner had no complaints regarding the home and was satisfied with the care provided. Service users find the lifestyle experienced in the home matches their expectations and preferences, and satisfies their social, cultural, religious and recreational interests and needs. Service users maintain contact with family/ friends/ representatives and the local community as they wish. Service users are helped to exercise choice and control over their lives. Service users receive a wholesome appealing balanced diet in pleasing surroundings at times convenient to them. The Commission considers all of the above key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 12, 13, 14 & 15 Residents’ lifestyles meets their expectations. They continue to have contact with their families and friends and enjoy a well balanced diet. Residents are able to maintain control over their lives. EVIDENCE: Residents stated that, despite mobility difficulties, they had been able to remain as independent as they can. They stated they went to bed when they wanted to and got up when they wished. One resident had chosen to stay in bed the day previously, she stated she had received drinks, meals and support to enable her to do so in comfort. Residents were observed choosing where to have their meals and where to receive visitors. Menus displayed offered a variety of meals which, when served, appeared appetising. Residents were observed receiving guests and leaving the home to visit friends and family. Bankfield House F54 F04 bankfield house A s62991 v222974 010605 stage 4.doc Version 1.30 Page 12 The home has recently appointed an activities co-ordinator and, whilst the activities programme is fairly new, it appears to be enjoyed by residents. On the first day of the inspection residents were observed to be making cakes. It was observed that whilst the activities co-ordinator completed activities, care staff occupied themselves elsewhere. It was obvious that one person was not sufficient to support the numbers of residents in activities. A lengthy period of time was spent by residents waiting for the activity to be set up. One resident was overheard asking if it was “still on” and another asking the inspector “how long will it be before we start?” Residents stated that they had been to the theatre and had recently made flower arrangements. When asked, all residents were satisfied with the activities provided by the home. The owners stated that it is their intention to develop this part of the service to offer residents activities on a daily basis. The voluntary worker was observed giving out afternoon drinks alone. As a consequence, it was some considerable time before some residents received drinks. The inspector was informed by one resident and two visitors that they had not received a drink. JUDGEMENT – we looked at outcomes for standard(s) 16 & 18 The complaints procedure is known and used by residents and visitors. The home has safeguards in place for the protection of vulnerable adults. EVIDENCE: The home has received one complaint since the last inspection. The complaints procedure is displayed within the home. Residents’ comment cards stated that they have a person they can speak to if they are unhappy about any parts of their care. When spoken to, all residents were satisfied with the service and had no complaints to raise. Staff are trained in adult protection procedures and are aware of their responsibility to report any incidents or suspicions of abuse. The home has written policies and procedures to be followed for the protection of all residents. Comment cards stated that residents felt safe . The Commission considers Standards 19 and 26 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 19, 25 & 26 The home was clean, safe and well maintained. EVIDENCE: It is recognised that Bankfield House is in need of upgrading and, as stated within the summary, plans are already underway to redevelop the home and provided a higher standard of accommodation than is currently available. Not withstanding that, residents stated they liked the home and found it a comfortable place to live. They stated they were looking forward to the redecoration and provision of new chairs. One resident stated she had received new bedding and was aware that her room was to be redecorated in the future. Throughout the inspection the home was clean and free from offensive odours. The garden areas are now attended to by professional gardeners and work has commenced to make the grounds more accessible to residents. Outside seating and tables are in place and used by residents during fine weather.

Routine maintenance is provided and records identified that professional services are provided when specific work or repairs are required. Service users are supported and protected by the home’s recruitment policy and practices. Staff are trained and competent to do their jobs. The Commission considers Standards 27, 29, and 30 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 27, 28, 29 & 30 The home provided sufficient numbers of staff who receive training for the work they are employed for. The home does not follow safe recruitment and selection procedures. EVIDENCE: The rota confirmed that the numbers of staff on duty were able to meet the needs of residents and exceeds that desired by the local authority. Of the 16 care staff, 11 have completed training at NVQ level 2 or above. The home’s induction of new staff is sufficient, however it could not be confirmed that the home’s six week induction and foundation training which has to be completed within six moths of employment meets the standard set by the National Training Organisation. There are serious concerns regarding the home’s recruitment and selection procedures which are not strictly followed when recruiting new staff. On the day of the inspection a new member of staff was completing her initial induction. Inspection of her file identified that her application was incomplete, in that, there was no educational details or employment history. References had not been received, there was also no proof of identity, photo or CRB check. The failure to follow rigorous recruitment procedures potentially increases the risk of harm for residents. Service users live in a home which is run and managed by a person who is fit to be in charge, of good character and able to discharge his or her responsibilities fully. Service users’ rights and best interests are safeguarded by the home’s record keeping, policies and procedures. The health, safety and welfare of service users and staff are promoted and protected. The Commission considers Standards 33, 35 and 38 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 31, 32, 33, 36 & 38 The home is run in the interests of the residents. EVIDENCE: Health and safety records were completed appropriately. Accidents relating to residents are recorded and filed as required by the Data Protection Act 1998. Servicing records confirmed appropriate checks to equipment were made. Fire safety records recorded weekly checks and fire safety practices by staff. After the inspection and prior to completing the report, the inspector received a report from the Fire Officer detailing the outcome of the most recent fire safety inspection. Mrs Pyatt was the manager of Bankfield under the previous owner. She has the relevant qualifications and experience to manage a care home. Records confirmed that care staff receive formal supervision and annual appraisals. The inspector observed that some relatives have completed questionnaires, as have new staff, as part of the home’s quality assurance procedure.
