Conditions of registration: Date of last inspection 25th October 2005 Brief Description of the Service: Bankfield provides personal care and support for up to 37 older people. The house is a detached Victorian property located approximately a quarter of a mile outside the centre of Sowerby Bridge. There is a steep drive up to the house. At the front of the house there is a garden with panoramic views of the surrounding countryside. Bedrooms are at ground, first and second floor level. All parts of the house can be accessed by a passenger lift. Most bedrooms are single, only two are double. 23 rooms have en suite facilities; the remainder have a hand washbasin with toilets and bathrooms nearby. This is an overview of what the inspector found during the inspection. The Commission for Social Care Inspection has to carry out at least two inspections of care homes every year. The inspection year runs from April to March and this was the second inspection visit for 2005/2006. Copies of previous inspection reports are available at the home or on the Internet at www.csci.org.uk The last inspection of the home was unannounced and took place on 25 October 2005. There have been no further visits until this unannounced inspection. This was an unannounced inspection carried out by one inspector over a 4hour period. The main purpose of the inspection was to make sure that the home continues to provide a good standard of care for the people who live there. The methods used at this inspection included looking at medication and care records and talking to service users, staff and the manager. A tour of the building also took place. What the service does well:
Prospective service users are provided with useful information about the home to help them make a decision about moving in. Pre admission visits to the home are actively encouraged. Evidence shows that service users are appropriately assessed before they move in to make sure that their needs can be met. The home is well managed and staffed by a competent team of staff. People who work at the home are friendly and helpful. The registered provider and operations managers are approachable and keen to provide a good quality of care at the home. They can be contacted if anyone has any concerns about any aspect of provision. Measures are in place to make sure that staff are suitable to work with older people, checks are made with previous employers and with the Criminal Records Bureau. Staff receive a range of training opportunities, which includes awareness of adult protection issues.

Bedrooms are individually decorated and set out to meet each person’s needs and tastes. Everyone has lots of their own personal possessions around them in their bedrooms. Service users and staff say the food is very good with plenty of variety on the menu and good quality produce used. Visitors to the home are very welcome and the invitation to stay for a meal is always on offer. What has improved since the last inspection? Each service user has a written contract/ statement of terms and conditions with the home. No service user moves into the home without having had his/her needs assessed and been assured that these will be met. Service users and their representatives know that the home they enter will meet their needs. Prospective service users and their relatives and friends have an opportunity to visit and assess the quality, facilities and suitability of the home. Service users assessed and referred solely for intermediate care are helped to maximise their independence and return home. The Commission considers Standards 3 and 6 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 1,2,3,5. All prospective service users have their needs fully assessed prior to admission and visits to the home are arranged for them and their families. EVIDENCE: There is a good, well-presented service user guide, which provides information about Bankfield for prospective service users and their families. The information is written in plain English and is in large print. The layout of the guide and the use of graphics makes the information more manageable. Copies are available on request from Bankfield. Everyone is invited to visit the home to meet service users and staff before making a decision about moving in. Relatives are very welcome to call at the home to view the facilities. One relative called at the home unannounced, during the inspection to have a look round and to talk to staff. All prospective service users are assessed by the manager to make sure that their needs can be met at Bankfield.

The intended outcomes for Standards 7 – 11 are: 7. 8. 9. 10. 11. The service user’s health, personal and social care needs are set out in an individual plan of care. Service users’ health care needs are fully met. Service users, where appropriate, are responsible for their own medication, and are protected by the home’s policies and procedures for dealing with medicines. Service users feel they are treated with respect and their right to privacy is upheld. Service users are assured that at the time of their death, staff will treat them and their family with care, sensitivity and respect. The Commission considers Standards 7, 8, 9 and 10 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 9 Service users are protected by the homes policies and procedures for dealing with medicines. EVIDENCE: Medication records are well-maintained and appropriate policies and procedures are in place covering all aspects of medication administration. Records are in place for all medicines received, administered and disposed of to ensure that there is no mishandling. All medication is stored in locked cupboards in a locked medical room. All senior staff that administer medication have received training or are taking part in training on the safe handling of medication. The senior on duty confirmed that General Practitioners routinely carry out medication reviews in respect of all service users. Service users find the lifestyle experienced in the home matches their expectations and preferences, and satisfies their social, cultural, religious and recreational interests and needs. Service users maintain contact with family/ friends/ representatives and the local community as they wish. Service users are helped to exercise choice and control over their lives. Service users receive a wholesome appealing balanced diet in pleasing surroundings at times convenient to them. The Commission considers all of the above key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): Not assessed on this inspection. See last report. EVIDENCE: Comments were recorded in the last report about the variety and good quality of the food served at the home. Service users and staff made similar positive comments again on this inspection. Hospitality was offered to visitors who were at the home to see their relative. They had travelled from outside the area and were staying over in the locality and spending time at the home during the day. The visitors were provided with a meal and dining space was made available for them in one of the lounges that was not in use. Service users’ legal rights are protected. Service users are protected from abuse. The Commission considers Standards 16 and 18 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): Systems are in place at the home to ensure that service users are safeguarded from abuse and that complaints are dealt with effectively. EVIDENCE: The home has a procedure that clearly specifies how complaints can be made; it also includes information about timescales for the home to respond to complainants. Details are contained in the service user guide. The manager said there had been one complaint in the last twelve months. Discussion took place with the manager about keeping a log of complaints so that it is possible to see at a glance the date of complaint, who made the complaint, what it was about, what action was taken to resolve the complaint and the outcome. This is not possible with the existing records. The complaint on file at the home had been made by a relative but it was not possible to tell which service user the details related to, nor was it possible to see details of the outcome. All staff receive training about adult protection issues as part of their induction package when they start work at the home. In addition, all staff took part in abuse awareness training during 2005. The staff records indicate that appropriate checks are carried out on all new staff before they start work in the home. References are always taken up and checks are carried out with the Criminal Records Bureau and Protection of Vulnerable Adults register.

Service users live in safe, comfortable bedrooms with their own possessions around them. Service users live in safe, comfortable surroundings. The home is clean, pleasant and hygienic. The Commission considers Standards 19 and 26 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 19,21,22,23,24,25,26. The home is clean and well maintained throughout, providing a safe, comfortable environment for service users. EVIDENCE: All parts of the house are comfortably furnished and pleasantly decorated. There are 24 en suite bedrooms out of a total of 33 single rooms, and of the 2 double rooms 1 has en suite facilities. All bathrooms and toilets are close to bedrooms and the shared areas of the home. There is evidence that assisted toilets and bathing facilities have been installed to ensure the safe handling of service users. Bedrooms are arranged to suit individual needs. Each one is individually decorated and there is evidence that people have lots of their own possessions around them such as photographs, ornaments and plants. People spend varying amounts of time in their rooms depending on their preferred daily routine.

The manager talked about plans to create a “reminiscence room” in one of the small lounges. There are plans to decorate the room with old photographs of the surrounding area and to obtain other memorabilia, which will serve as focal points for discussion and reminiscence.
