What the care home does well

Residents and visitors spoken with praised the attitude and competence of all staff employed. Staff training and induction procedures were comprehensive and ensured staff were working with the best interests of residents in mind. Communication between managers, staff members and relatives/visitors was good and this again served to enhance quality of life for residents. All areas of the home seen were found to be extremely clean and provided a comfortable and homely environment. The inspector felt that the atmosphere in the home was relaxed and friendly. Residents and visitors spoken with praised the quality of the meals provided.

What has improved since the last inspection?

Since the last inspection care planning at the home has improved Care plans are more comprehensive and include the physical as well as the psychological care needs of residents. This has been achieved by ensuring that the expertise of the Registered General Nurses employed as well as the Registered Mental Nurses is utilised. At the time of this inspection radiator guards were being fitted to all radiators in order to improve safety for residents.

What the care home could do better:

The general manager discussed the homes intentions to raise standards further by improving staff training, development and recruitment.

Its registration categories include Mental Health and Dementia. The home has established links with the Alzheimers society and is staffed by registered mental health nurses and care staff. Situated in a rural area close to the village of Kirby Muxloe Leicestershire it is within a short distance of Junction 22 on the M1 The home is owned by Mr and Mrs L Warren. The property has been developed and offers accommodation on two floors, which are served by a passenger lift. The home has large mature gardens and a patio area and has pleasant views of the countryside. It has three comfortable lounges and the main lounge has a dining area. The home has twenty-one single bedrooms and four double bedrooms of which two are en-suite.Adequate parking facilities are available and the home is situated on a main road The home has a website at www.baronspark.co.uk Barons Park Nursing Home C51 C01 S1887 Barons Park V234431 111005 STAGE 4.doc Version 1.40 Page 5 SUMMARY This is an overview of what the inspector found during the inspection. This inspection was unannounced and took place over three hours and was facilitated by the home’s general manager. The inspector selected four residents and tracked the care they receive through review of their records, discussion with them (where possible), the care staff and observation of care practices. The inspector also spoke with staff members regarding training and support and with one visitor to the home. This was a positive inspection and was facilitated by the manager and staff in a helpful, friendly and professional manner. The CSCI sends out service provider pre inspection questionnaires, resident and relative questionnaires and health/social care professionals’ questionnaires throughout the inspection year. These are questionnaires to ascertain the satisfaction of service users, relatives / visitors and professionals as to the standard of care and service provided. At the time of this inspection eighteen relatives questionnaires and two health/social care professional questionnaires had been returned, all gave positive feedback regarding the quality of care and service provided. Comments made included – ‘Staff, visitor, family relationships are excellent and the food is good’ ‘My mother is well cared for and the home always has a pleasant atmosphere’. Six relatives/visitors felt staffing levels could be improved. Only specific standards were inspected during this inspection. Prospective service users have the information they need to make an informed choice about where to live. Each service user has a written contract/ statement of terms and conditions with the home. No service user moves into the home without having had his/her needs assessed and been assured that these will be met. Service users and their representatives know that the home they enter will meet their needs. Prospective service users and their relatives and friends have an opportunity to visit and assess the quality, facilities and suitability of the home. Service users assessed and referred solely for intermediate care are helped to maximise their independence and return home. The Commission considers Standards 3 and 6 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 3 Assessment procedures in place ensured that the needs of residents could be met. EVIDENCE: Assessment records for the four case tracked residents were inspected. Assessments were found to be thorough and included a mental health assessment. Where possible, ‘life profiles’ are included within resident’s care and assessment records. These enable staff to gain a greater understanding of residents care needs, expectations and preferences. The service user’s health, personal and social care needs are set out in an individual plan of care. Service users’ health care needs are fully met. Service users, where appropriate, are responsible for their own medication, and are protected by the home’s policies and procedures for dealing with medicines. Service users feel they are treated with respect and their right to privacy is upheld. Service users are assured that at the time of their death, staff will treat them and their family with care, sensitivity and respect. The Commission considers Standards 7, 8, 9 and 10 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 7, 9 and 10 Comprehensive care policies and procedures support staff to meet resident’s needs in a respectful and appropriate manner. EVIDENCE: Care plans for four case tracked residents were inspected and appeared to be meeting all assessed needs. Care plans included all relevant risk assessments. The relationship between staff, residents and visitors was praised within relative satisfaction questionnaires and by residents and visitors spoken to on the day of this inspection. The inspector observed interactions between residents’ staff and found them to be positive and respectful. The medication administration records for four case tracked residents were inspected. Medication records appeared to be in good order. Medication is administered by qualified nurses. Service users find the lifestyle experienced in the home matches their expectations and preferences, and satisfies their social, cultural, religious and recreational interests and needs. Service users maintain contact with family/ friends/ representatives and the local community as they wish. Service users are helped to exercise choice and control over their lives. Service users receive a wholesome appealing balanced diet in pleasing surroundings at times convenient to them. The Commission considers all of the above key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 12, 13 and 15 Residents are enabled to exercise choice and autonomy and are provided with a wholesome and nutritious diet. EVIDENCE: The lunchtime meal was served during this inspection. The meals being served appeared appealing and nutritious. The dining area appeared congenial and staff were seen assisting residents in an appropriate and sensitive manner. Menu records were inspected. A wholesome and nutritious menu was on offer and catering staff kept a detailed list of individual residents likes, dislikes and dietary needs. The kitchen area appeared very clean and well organised. Catering staff confirmed that sufficient resources were supplied and that there was always food available for staff to make snacks and drinks for residents outside of catering staff hours. Residents and visitors spoken with were extremely satisfied with the quality of meals provided. A programme of activities is planned each month; activities included cards, games dominoes and music. Entertainer’s come into the home and fetes and barbecues are held in the grounds. Religious services are held at the home. Service users and their relatives and friends are confident that their complaints will be listened to, taken seriously and acted upon. Service users’ legal rights are protected. Service users are protected from abuse. The Commission considers Standards 16 and 18 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 16 and 18 Complaints and adult protection policies were robust and afforded protection and minimised risk to all service users accommodated. EVIDENCE: The homes policies and procedures were in line with national and local guidelines. Staff spoken with were aware of the correct procedure to follow in the event of suspected abuse. The home has complaints procedures in place and residents and visitors spoken with confirmed that they would be happy to approach the management team or any of the staff if they had any concerns. The general manager and deputy manager were described as being particularly helpful. Regular residents and staff meeting are held. The home is clean, pleasant and hygienic. The Commission considers Standards 19 and 26 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 19 and 26 All areas of the home inspected appeared well maintained, comfortable and homely and provided a safe and comfortable environment for residents. EVIDENCE: All areas of the inspected appeared extremely clean and hygienic. Separate laundry facilities are provided. At the time of this inspection there were three domestic staff on duty every day and a fourth staff member undergoing induction training. Residents and visitors spoken with were very satisfied with the cleanliness at the home. Staff spoken with and training records inspected confirmed that fire safety training was provided. Service users needs are met by the numbers and skill mix of staff. Service users are in safe hands at all times. Service users are supported and protected by the home’s recruitment policy and practices. Staff are trained and competent to do their jobs. The Commission considers Standards 27, 29, and 30 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 27, 28 and 30 Resident’s needs are met by the numbers and skill mix of staff. Staff are provided with the training required to meet the needs of the residents accommodated. EVIDENCE: At the time of this inspection there were thirty-three residents living in the home. The duty roster for the week in which this inspection took place was inspected. There is at least one qualified nurse on duty at all times. There were six care assistants on duty each morning, five in the afternoon/evening and two at night. Separate catering and domestic staff were also provided. While it was identified within relative/visitors questionnaires that staffing levels could be improved, at the time of this inspection staffing numbers appeared to be meeting the needs of residents accommodated. A programme of National Vocational Training in care was in place. Staff training records were inspected. All staff receive induction and foundation training in line with national training standard specifications. Service users live in a home which is run and managed by a person who is fit to be in charge, of good character and able to discharge his or her responsibilities fully. Service users benefit from the ethos, leadership and management approach of the home. The home is run in the best interests of service users. Service users are safeguarded by the accounting and financial procedures of the home. Service users’ financial interests are safeguarded. Staff are appropriately supervised. Service users’ rights and best interests are safeguarded by the home’s record keeping, policies and procedures. The health, safety and welfare of service users and staff are promoted and protected. The Commission considers Standards 33, 35 and 38 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 32,35 and 38 Management and administration policies and procedures in place safeguarded the best interests and welfare of residents. EVIDENCE: Residents, staff and visitors spoken with felt the home’s management approach was open and inclusive. Regular staff and visitors meeting are held and opportunity is provided for residents, staff and visitors to air their views. The home’s pre inspection questionnaire confirmed that all required maintenance checks had been carried out. All staff receive relevant health and safety training. The general manager confirmed that he was responsible for all risk assessments and that these were carried out at least annually and included fire risk assessment. The home does not become involved in the financial affairs of residents or store any personal money. Any items needed by residents are purchased by the provider and then an invoice for that amount is issued.
