What the care home does well

The Registered Manger completes a comprehensive assessment, prior to the residents moving to the home, which ensures that their needs will be fully met. The registered provider provides policies and procedures ensure that residents and relatives concerns are addressed promptly. Relatives who spoke with theinspector felt the Registered Manager would address any concerns or complaints they had. Residents` relatives who spoke with the inspector said that they felt that the lifestyle at the home matched their expectations and satisfied their cultural, social and religious needs. The atmosphere at Barons Park was friendly and relaxed. Residents are encouraged to continue their personal interests or needs. A religious service and prayer is conducted on the 3rd Monday of the month as part of a planned activity. The church visits the home. The curate offers Holy Communion at this time to the Roman Catholic residents and a relevant minister for the different faiths offers communion to the other denominations.

What has improved since the last inspection?

Residents` bedroom door signs show the residents previous home address to encourage stimulation of the residents` memory and to encourage recognition of information from each individual residents past. Quality audits continue to be improved and feedback is placed on the notice board in the front entrance foyer. Barons Park continues to expand the range of information about itself including external newsletters.

What the care home could do better:

The outcomes for residents would be improved by ensuring that medication administered is appropriately signed for. Whilst building work is being conducted, extra heat should be provided to the two front lounges to ensure that residents are being kept sufficiently warm and comfortable during the days.

To be able to admit the person of category DE(55 ) named in variation application dated 16th July 2002. 9th January 2006 Date of last inspection Brief Description of the Service: Barons Park is a nursing home registered for thirty-three service users of both sexes over sixty-five years of age. It is registered for Mental Health and Dementia care. The home has established links with the Alzheimers society and is staffed by registered mental health nurses and care staff. Situated in a rural area close to the village of Kirby Muxloe Leicestershire it is within a short distance of Junction 22 on the M1 The home is owned by Mr and Mrs L Warren. The property has been developed and offers accommodation on two floors, which are served by a passenger lift. The home has large mature gardens and a patio area and has pleasant views of the countryside. It has three comfortable lounges and the main lounge has a dining area. The home has twenty-one single bedrooms and six double bedrooms of which two are ensuite. Adequate parking facilities are available and the home is situated on a main road. The home has a website at www.baronspark.co.uk Fees range from £600.00 to £700.00 per week The Statement of Purpose and Service User Guide (information about the service provided) is currently available. The focus of inspections undertaken by the Commission for Social Care Inspection (CSCI) is on outcomes for residents and their views of the service provided. The home provides care for up to thirty- three residents. On the day of inspection there were thirty-two residents. Barons Park Nursing home is specialist home for people who fall within the category of old age requiring care for dementia or a mental disorder. This home provides both Registered Mental Nurses (RMN) and Registered General Nurses (RGN) to ensure that all the residents’ health care needs are met. The inspection took six hours to complete. Preparation included examining inspection records and information obtained from the CSCI pre-inspection questionnaire to the Provider about Barons Park. This aided the inspection process by providing background information. Discussion was held with two residents. However, other residents were observed in their daily routine. The two residents were unable to communicate very much with the inspector and residents relatives spoke with the inspector at length. The primary method of inspection used was “case tracking”. This involved speaking with the residents who use the service provided, looking at three residents care plans and making observations. Six relatives spoke with the inspector and a number of other residents were observed during the day also. All the required key standards were inspected during this visit. Areas of concern raised by the last inspection report were discussed and had been addressed. There were two new requirements made at this inspection. The Registered Manager facilitated the inspection. Prospective service users have the information they need to make an informed choice about where to live. Each service user has a written contract/ statement of terms and conditions with the home. No service user moves into the home without having had his/her needs assessed and been assured that these will be met. Service users and their representatives know that the home they enter will meet their needs. Prospective service users and their relatives and friends have an opportunity to visit and assess the quality, facilities and suitability of the home. Service users assessed and referred solely for intermediate care are helped to maximise their independence and return home. The Commission considers Standards 3 and 6 the key standards to be inspected. JUDGEMENT – we looked at outcomes for the following standard(s): 3 and 6 Quality in this outcome area is good. This judgement has been made using available evidence including a visit to this service. Residents and their relatives are given information about the home in order to make an informed decision about whether the service is right for them. The personalised needs assessment ensures that residents’ diverse needs are identified before they move to the home. EVIDENCE: The Service User Guide and the Statement of Purpose are available in the hallway. A copy of the most recent inspection report was available in the hallway. The Registered Manager is responsible for conducting the care assessments. The pre-assessment forms were completed and a full assessment of need entered into the care plans, as seen by the inspector. A trial period of one month is given to residents to ensure that the residents needs can be fully met. Registered Mental Nurses primarily give care but Registered General Nurses are also employed and both mental and physical care is provided as required in accordance with the residents needs as assessed. None of the residents were able to describe the assessment process but family members who spoke with the inspector were able to explain that they felt fully involved in this process. The service user’s health, personal and social care needs are set out in an individual plan of care. Service users’ health care needs are fully met. Service users, where appropriate, are responsible for their own medication, and are protected by the home’s policies and procedures for dealing with medicines. Service users feel they are treated with respect and their right to privacy is upheld. Service users are assured that at the time of their death, staff will treat them and their family with care, sensitivity and respect. The Commission considers Standards 7, 8, 9 and 10 the key standards to be inspected. JUDGEMENT – we looked at outcomes for the following standard(s): 7, 8, 9 and 10 Quality in this outcome area is adequate. This judgement has been made using available evidence including a visit to this service. At Barons Court staff are successful in delivering appropriate care to individual residents. Residents’ physical and mental health care needs are met. However more careful recording of medications given is required. EVIDENCE: Three care plans were inspected. Care, social and mental needs were addressed. Care plans included a record of residents’ interests and during the inspection this was seen. This is recorded in the activities and social care section of this report. A key worker system is in operation at the home. The key worker ensures that the residents’ stay is made comfortable by befriending the resident, ensuring that the residents’ health and social care needs are fully met. Barons Park Nursing Home DS0000001887.V327948.R01.S.doc Version 5.2 Page 11 One relative spoke highly of the care given to their mother and the respect and dignity shown to their mother by the staff members. The relative explained that sometimes they would wake at night concerned that their mother is alright and they would ring the home in the middle of the night. The staff would always promise to look in on their mother and ring them back to let them know that their mother is well, even though they may have just completed a round or night inspection of all the residents. ‘My wife is always well dressed. Care is good here.’ ‘Mum is always clean and tidy.’ ‘Staff will write down any requests by relatives in regard to the care of their loved one.’ ‘My relative was able to settle in, the staff seem to have a knack at doing this’ ‘My relative is always well dressed she is washed and showered when required.’ ‘If I had any health concerns about my relative I would talk to the Matron or deputy Matron. If my relative has an accident with her dinner, her clothing is changed immediately.’ ‘As a relative I feel that I am fully aware of any treatment and care given to my relative at this home’. The Registered Manager was able to explain that the doctor held a well persons clinic at the home as well as visiting when a resident became ill. If they needed an eye test the optician came to the home. Health care specialists visited whenever they were needed. The three care plans seen also verified this. Medications of the three case tracked residents’ at the home were checked. Two were satisfactory but one resident’s medication was not entered on the medicine sheet, however it was mentioned in the daily care records. One of these medicines was a controlled drug and it is important that drugs are recorded on the drug sheets to avoid medication errors. (Controlled drugs fall under the Misuse of Drugs Act 1971 which aims at checking the unlawful use of the drugs liable to produce dependence or cause harm if misused). There were no self-medicating residents at the home. A requirement has been made for practice to be improved in this area. When staff entered they knocked and waited for a reply before entering the bedroom a relative told the inspector this was the usual practice at this home. This was just one example of the staff’s respect shown to the residents at the home. Service users find the lifestyle experienced in the home matches their expectations and preferences, and satisfies their social, cultural, religious and recreational interests and needs. Service users maintain contact with family/ friends/ representatives and the local community as they wish. Service users are helped to exercise choice and control over their lives. Service users receive a wholesome appealing balanced diet in pleasing surroundings at times convenient to them. The Commission considers all of the above key standards to be inspected. JUDGEMENT – we looked at outcomes for the following standard(s): 12, 13, 14 and 15 Quality in this outcome area is excellent. Activities within the home succeed in meeting the identified daily and social needs of residents and the outcome is positive for the residents. EVIDENCE: ‘I wanted to see a home that offered tea and sympathy in a homely caring environment, this home has it.’ ‘The carers are fun and very friendly’. ‘Staff are always friendly and make me feel very welcome.’ ‘My relative is unable to see now and is always walking around, I would like my relative to be involved with more religious services as she loves hymns’. ‘The food is good here.’ Special diets are provided and liquidised meals were seen being given to residents in both of the front lounges. Barons Park Nursing Home DS0000001887.V327948.R01.S.doc Version 5.2 Page 14 The Environmental Health Officer had recently visited Barons Park and the inspector was told that there were no problems identified during that inspection. Activites seen by the inspector appeared to suit the needs of the residents. Care staff were fully involved assisting the care coordinator who organised activities to ensure that each resident was doing something that they could do and enjoying it. Some residents looked at photgraphs and had to recognise themselves by pointing to their picture. One resident walked around the lounge with staff, other residents were involved with drawing and colouring pictures that they made. Two residents were just sitting with staff who simply held their hands so that they knew that someone was there with them. Photographs of group activities were displayed on a board in the foyer where some of the residents had been taken out for a meal to a local place of interest in this way, the residents remain in contact with their local community. JUDGEMENT – we looked at outcomes for the following standard(s): 16 and 18 Quality in this outcome area is good. This judgement has been made using available evidence including a visit to this service. There are policies and procedures for dealing with complaints and protection thereby giving protection to the residents. EVIDENCE: The Commission of Social Care Inspection has not received any complaints about Barons Park Nursing Home since the last inspection. Residents who spoke with the inspector were not able to explain what they would do if they had a problem however, relatives who spoke with the inspector were happy to express their concerns or complaints and knew that the Registered Manager would address them. There are policies in place for dealing with complaints and protection. The Registered Manager was able to show the inspector the staff training records. Staff receives training on the Protection of Vulnerable Adults (POVA). Staff who spoke with the inspector were able to demonstrate their understanding of the issues involved ensuring that residents are protected in this way. The complaints book was checked and satisfactory. The complaints procedure is displayed in the entrance to the building. Service users have sufficient and suitable lavatories and washing facilities. Service users have the specialist equipment they require to maximise their independence. Service users’ own rooms suit their needs. Service users live in safe, comfortable bedrooms with their own possessions around them. Service users live in safe, comfortable surroundings. The home is clean, pleasant and hygienic. The Commission considers Standards 19 and 26 the key standards to be inspected. JUDGEMENT – we looked at outcomes for the following standard(s): 19 and 26 Quality in this outcome area is adequate. This judgement has been made using available evidence including a visit to this service. Residents are provided with a safe and hygienic environment although during this period of building work extra heating will need to be supplied to areas affecting residents’ comfort. EVIDENCE: ‘My mother is sharing a bedroom temporarily until the building work is completed and I am fine with this.’ ‘This is a lovely home, it really is.’ The bedrooms of residents case tracked and public areas were safe and well maintained. Each bedroom was pleasant and clean some having with nice views. One resident who liked to look outside and walk around was given a room with pleasant views this was appropriate for their specific need. Barons Park Nursing Home DS0000001887.V327948.R01.S.doc Version 5.2 Page 17 A handy person is employed at Barons Park who ensures routine maintenance needs of the home are met and records are kept for this. Building work is currently taking place at Barons Park and the area being developed is kept separate to ensure resident safety. However, it was noted by the inspector that the two lounges to the front of the building were cold during the day and extra heating needs to be provided to ensure residents who use these lounge areas are kept warm. It was a cold day (snow) and no evidence of extra heating was seen. Both lounge doors were routinely open and not all residents had jumpers on to keep them warm. This was mentioned during feedback to the Registered Manager who explained that once the builders had gone home the rooms were able to retain their heat. This will be a requirement. Service users’ needs are met by the numbers and skill mix of staff. Service users are in safe hands at all times. Service users are supported and protected by the home’s recruitment policy and practices. Staff are trained and competent to do their jobs. The Commission consider all the above are key standards to be inspected. JUDGEMENT – we looked at outcomes for the following standard(s): 27, 28, 29 and 30 Quality in this outcome area is excellent. This judgement has been made using available evidence including a visit to this service. The Registered Provider has a recruitment procedure, which is fully implemented and protects the residents. EVIDENCE: Staff records were checked and clear evidence of a relevant training programme was seen. Specialist training is provided for the staff in Dementia care and by the Alzheimer’s society in Alzheimer’s awareness. Other training includes podiatry, (foot and nail care), wound care, infection control, medication updates training for staff and food hygiene plus all the basic training expected of a care home. Over half of the care staff are trained to National Vocational Qualification (NVQ) level 2 in care and above. In this way staff receives training for the assessed needs of residents. The skill mix of staff is sufficient to meet the needs of the residents. There was evidence of good recruitment practices. Staff records inspected were satisfactory and had relevant information in them. In this way the residents are supported and protected by well-trained staff competent to do their jobs. Service users are safeguarded by the accounting and financial procedures of the home. Service users’ financial interests are safeguarded. Staff are appropriately supervised. Service users’ rights and best interests are safeguarded by the home’s record keeping, policies and procedures. The health, safety and welfare of service users and staff are promoted and protected. This judgement has been made using available evidence including a visit to this service. The Registered Manager ensures that the home is run in the best interest of residents. EVIDENCE: The registered manager ensures that safe working practices are in place and records are provided at the home. The Registered Manager is an experienced nurse with management experience. The Registered Provider does not hold money for residents. The families or appointed persons are appointed to do this. The Registered Manager explained that Barons Park undertakes quality reviews with residents where possible, and their relatives.
