This inspection was carried out on 13th February 2006.

What the care home does well

The home is clean and comfortable and the layout provides a sociable atmosphere for service users. Care plans were thorough and relevant to the needs of service users. The food in the home was of a good standard and staff supported service users in a sensitive manner.

What has improved since the last inspection?

Since the previous inspection the home has improved the care plans so that they are now more relevant to the needs of people who have dementia.

What the care home could do better:

A requirement has been made concerning the way in which the home looks after service users` spending money and a recommendation has been made that specimen signatures of staff administering medication are kept on file.

It is registered to accommodate thirty service users over the age of sixty-five years who require nursing and those who have life limiting disease. It is situated in a residential area close to local amenities and is accessible by train and local bus services. It is a family residence that has been extended. Service users have access to a mature garden the rear of the home. Car parking space is available at the front of the home. This was the second of the two annual inspections for the service and concentrated on those key standards not assessed at the previous inspection as well as areas that did not meet the standard at the previous inspection. Consequently this inspection concentrated on only four standards. The Inspector toured the building, spoke with the Matron, Deputy Matron and two members of staff as well as three service users and one visitor. Prospective service users have the information they need to make an informed choice about where to live. Each service user has a written contract/ statement of terms and conditions with the home. No service user moves into the home without having had his/her needs assessed and been assured that these will be met. Service users and their representatives know that the home they enter will meet their needs. Prospective service users and their relatives and friends have an opportunity to visit and assess the quality, facilities and suitability of the home. Service users assessed and referred solely for intermediate care are helped to maximise their independence and return home. The service user’s health, personal and social care needs are set out in an individual plan of care. Service users’ health care needs are fully met. Service users, where appropriate, are responsible for their own medication, and are protected by the home’s policies and procedures for dealing with medicines. Service users feel they are treated with respect and their right to privacy is upheld. Service users are assured that at the time of their death, staff will treat them and their family with care, sensitivity and respect. The Commission considers Standards 7, 8, 9 and 10 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 7 and 9 Service users benefited from having clear and comprehensive care plans and were protected by the home’s medication policies and practices. EVIDENCE: The previous inspection had highlighted a need for care plans to include information on supporting the mental health needs of people with dementia. This had now been addressed and the care plans contained all the required information. The care plans were comprehensive and written clearly so that they were easy to follow. They also contained information from the initial assessment about each person’s life before they came into the home including their family history, employment, preferences and hobbies and interests. All plans were written in sufficient detail to describe to care staff exactly what was required. Service users had been involved in the writing of their own care plans and the plans were reviewed on a monthly basis. The home has policies and procedures in place for safely managing the administering of medication. Medication, including controlled drugs, was stored safely and all staff involved in administering medication had regularly updated training. All medication coming into and going out of the home was recorded. Records of medication administered were generally good except that, on some Bedhampton Nursing Home DS0000011476.V282783.R01.S.doc Version 5.1 Page 9 records, the signatures of some staff were difficult to distinguish. It is recommended that the home keep a list of specimen signatures within the medication file. Service users find the lifestyle experienced in the home matches their expectations and preferences, and satisfies their social, cultural, religious and recreational interests and needs. Service users maintain contact with family/ friends/ representatives and the local community as they wish. Service users are helped to exercise choice and control over their lives. Service users receive a wholesome appealing balanced diet in pleasing surroundings at times convenient to them. The Commission considers all of the above key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 15 Service users benefited from a nutritious, balanced diet suited to their needs. EVIDENCE: The dietary needs of all service users were assessed and recorded on admission to the home and specific diets, e.g. diabetic, were catered for. The home had menus in place with two choices at each mealtime. The menus provided a wide variety of food. Service users were able to choose their meals from the menu on a daily basis and an alternative would be offered where a service user did not like the choices from the menu. Most service users chose to eat breakfast in their rooms and to have lunch and supper in the lounge/dining room. The home is in the process of purchasing new furniture for the dining area and at present most service users eat from a small table in front of their chairs. There were also secluded parts of the lounge/dining room where people could eat if they chose to. The room provided a comfortable and sociable area for mealtimes. Service users’ meals were served to them on individual trays along with drinks, napkins and condiments and the meals were well presented. Meals were served in adequate portions and the food looked appealing. Fresh vegetables were used. Service users spoken with said they enjoyed the meals in the home. Staff were available to support people who needed help with their meals and this support was provided in a sensitive and unhurried manner. Service users live in a home which is run and managed by a person who is fit to be in charge, of good character and able to discharge his or her responsibilities fully. Service users benefit from the ethos, leadership and management approach of the home. The home is run in the best interests of service users. Service users are safeguarded by the accounting and financial procedures of the home. Service users’ financial interests are safeguarded. Staff are appropriately supervised. Service users’ rights and best interests are safeguarded by the home’s record keeping, policies and procedures. The health, safety and welfare of service users and staff are promoted and protected. The Commission considers Standards 31, 33, 35 and 38 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 35 Service users’ interests were not fully protected by the home’s system for accounting for their individual money. EVIDENCE: In line with its policy, the home has as little involvement as possible in looking after the money and finances of service users. They do manage small amounts of spending money for some service users, up to a value of one hundred pounds. This money is kept in a cash-box and is accessible only to the Matron and the Deputy Matron. Records were kept of transactions and these were signed by service users where possible and otherwise by two members of staff. At the time of the inspection there were eight envelopes in the cash-box for eight different service users. The Inspector compared the amounts in the envelopes and found that two lots of money did not correspond. One had twenty pence less than it should have had and the other had five pounds more Bedhampton Nursing Home DS0000011476.V282783.R01.S.doc Version 5.1 Page 16 than it should have had. The other six envelopes corresponded with the records. Money also fell out of envelopes in the cash-box when it was opened. A requirement has been made that the home reviews the system for managing these amounts of money and ensures that it is replaced with a more reliable system.
