This inspection was carried out on 12th May 2005.

The inspector made no statutory requirements on the home as a result of this inspection and there were no outstanding actions from the previous inspection report.

What the care home does well

The service provides individual support to suit the needs of the residents. Staff know what the residents` needs are and residents confirmed this. Resident`s individual likes and dislikes are well known to staff. Records contain appropriate information about residents` needs and their likes and dislikes. Residents spoke highly of the staff and the quality of care offered. Residents said that they were happy with the food served and that they were able to choose whether or not they had meals in the dining room or their own rooms. There is a relaxed atmosphere in the Home. The Manager and Deputy Manager provide a good level of support to staff. Records show formal supervision and staff meetings are arranged regularly. Staff stated that they are able to raise issues with the management. The Home has very attractive grounds that are easily accessible. The Home is well maintained, clean and hygienic.

What has improved since the last inspection?

An additional en-suite has been installed in one bedroom. There continues to be a commitment to enabling staff to access qualifying training.

What the care home could do better:

Care plans could be improved by including additional information about residents` social and emotional needs. There are no requirements or recommendations in this report.

Date of last inspection 04.11.04 Brief Description of the Service: Birch Hill is situated on the edge of Norham village, whichis approximately eight miles from Berwick upon Tweed. There are local bus services to Berwick where there is a main line railway station. Birch Hill is a detached house situated in its own attractive gardens with views of the village and the river Tweed. The Home is owned by BEN Motor and Allied Trades Benevolent Fund and is registered as a charity. Birch Hill provides comfortable, homely accommodation for up to twenty-six elderly people two of whom suffer some degree of dementia. The inspection lasted four hours and involved talking to residents, the Manager and staff; inspection of records and a tour of the building. Residents said that they were satisfied with the quality of care provided. They spoke highly of individual staff. A good rapport existed between residents and staff. Care plans seen during the inspection contained appropriate information about each person’s care needs and the support required. The Home is well furnished and appointed and residents are not required to share a room unless they have chosen to do this. There is a homely atmosphere in the Home. Staff have appropriate training provided to ensure that they are able to meet residents’ needs. Staff feel able to put forward their views about the running of the Home to the Manager or to the line Manager who visits regularly. Prospective service users have the information they need to make an informed choice about where to live. Each service user has a written contract/ statement of terms and conditions with the home. No service user moves into the home without having had his/her needs assessed and been assured that these will be met. Service users and their representatives know that the home they enter will meet their needs. Prospective service users and their relatives and friends have an opportunity to visit and assess the quality, facilities and suitability of the home. Service users assessed and referred solely for intermediate care are helped to maximise their independence and return home. The Commission considers Standards 3 and 6 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 2, 3, 4, 5, 6 Each resident has a written contract with the Home. Residents have their needs assessed before moving into the Home. Arrangements are in place to ensure that residents’ needs are met. Pre visits are arranged to the Home so that prospective residents and their family are aware of the facilities and service provided. Birch Hill does not provide intermediate care. EVIDENCE: The Manager confirmed that all residents receive a written contract stating the terms and conditions of residence. One resident was able to confirm that she had received a contract when she was admitted. Every resident has a full assessment of their needs before they are admitted. Records were available to confirm this. If the person has a Care Manager information about the assessment is sent to the Home prior to admission, where there is no Care Manager the Home’s staff carry out the assessment. The Manager and staff described how the person and their family are involved in this process. Relatives are also encouraged to visit the Home. Residents were able to confirm that they visited the Home before coming to live there. Information was available in one resident’s file to confirm this practice. The service user’s health, personal and social care needs are set out in an individual plan of care. Service users’ health care needs are fully met. Service users, where appropriate, are responsible for their own medication, and are protected by the home’s policies and procedures for dealing with medicines. Service users feel they are treated with respect and their right to privacy is upheld. Service users are assured that at the time of their death, staff will treat them and their family with care, sensitivity and respect. The Commission considers Standards 7, 8, 9 and 10 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 7, 8 Care plans providing details of each resident’s needs are in place. Staff promote and maintain residents’ health and ensures access to health care services to meet assessed needs. EVIDENCE: Three residents’ files were checked. These contained individual care plans based on the assessment carried out at the time of admission. Evidence was available of regular review of the care plan. Risk assessments were available in an appropriate form. Staff were able to confirm that they knew the content of care plans and that they are involved in drawing up and reviewing the plans. Three residents stated that staff were aware of their needs and that they were satisfied with the support provided. Evidence was available from records that individual health care needs are identified and met. Records showed that health care professionals are consulted and visit regularly and as needed. The Manager confirmed that there is a good level of support from local health care professionals. Service users find the lifestyle experienced in the home matches their expectations and preferences, and satisfies their social, cultural, religious and recreational interests and needs. Service users maintain contact with family/ friends/ representatives and the local community as they wish. Service users are helped to exercise choice and control over their lives. Service users receive a wholesome appealing balanced diet in pleasing surroundings at times convenient to them. The Commission considers all of the above key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 13, 14 Residents are encouraged to maintain contact with relatives, friends and the local community. Residents are helped to exercise choice and control over their lives. EVIDENCE: Residents stated that they are able to maintain contact with family and friends and that staff assist them to do this. Links with the local community are encouraged and residents regularly attend the church, local history society and the parish council meetings. Records and discussion with residents and staff confirm that residents’ routines are organised to suit individual choice. Residents said that they are able to choose where they take their meals and at what time. Service users and their relatives and friends are confident that their complaints will be listened to, taken seriously and acted upon. Service users’ legal rights are protected. Service users are protected from abuse. The Commission considers Standards 16 and 18 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 16 Residents feel confident about making a complaint to the staff and know that their concerns will be taken seriously. There is a simple, clear and accessible complaints procedure in place. EVIDENCE: Residents said that they could make a complaint to a member of staff or the Manager. They said that they were confident that this would be taken seriously. Staff knew that there were written policies and procedures relating to complaints. They were able to give appropriate examples of how they would assist a resident to make a complaint. Complaint records were available for inspection and these provided appropriate information about the investigation and outcome of complaints. No complaints have been made since the last inspection. The home is clean, pleasant and hygienic. The Commission considers Standards 19 and 26 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 26 The Home is clean, pleasant and hygienic. EVIDENCE: A tour of the building was carried out and the Home was clean and no unpleasant odours were noted. Evidence was available that appropriate cleaning routines are in place. The Manager reported that appropriate training is provided in infection control and one member of staff regularly attends sessions with the Infection Control link nurse. Written Guidance is in place for Health and Safety and Infection Control. Records and discussion with staff confirmed that appropriate training has been provided and that they were aware of written guidance. The laundry is fitted with appropriate equipment. A sluice is available. Water services have been checked and records confirm this. Radiators can be individually controlled. The central heating system is regularly serviced. Residents stated that they were satisfied with the standard of cleanliness in the Home. Service users needs are met by the numbers and skill mix of staff. Service users are in safe hands at all times. Service users are supported and protected by the home’s recruitment policy and practices. Staff are trained and competent to do their jobs. The Commission considers Standards 27, 29, and 30 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 27 Staffing at the time of this inspection was sufficient to meet the needs of the residents. EVIDENCE: The rotas showed that generally there are four care staff are on duty in the mornings and three in the afternoon, although there has been an increase to four in the evenings to meet identified needs. Two waking staff are on duty from 10pm to 8am. There is a senior member of staff on each shift. The Manager provided information that showed that fourteen staff have an appropriate qualification in care. This is to be commended. Residents stated that they feel there are enough staff to meet their needs. Service users benefit from the ethos, leadership and management approach of the home. The home is run in the best interests of service users. Service users are safeguarded by the accounting and financial procedures of the home. Service users’ financial interests are safeguarded. Staff are appropriately supervised. Service users’ rights and best interests are safeguarded by the home’s record keeping, policies and procedures. The health, safety and welfare of service users and staff are promoted and protected. The Commission considers Standards 33, 35 and 38 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 38 Health, safety and welfare of residents is promoted and protected. EVIDENCE: Records were available to confirm Moving and Handling, First Aid, Fire Safety, Food Hygiene and Infection Control training is provided at regular intervals. Staff stated that there are regular training events covering these areas. Records confirmed that there are arrangements in place for regular checks/servicing of the central heating system, water temperature, electrical equipment, fire system. Written evidence was not available to confirm that weekly fire alarm tests are carried out. Fire risk assessments were in place. Records of accidents are maintained appropriately. The Manager confirmed that Induction and Foundation training is provided for all new staff.
