This inspection was carried out on 18th October 2005.

CSCI has not published a star rating for this report, though using similar criteria we estimate that the report is Adequate. The way we rate inspection reports is consistent for all houses, though please be aware that this may be different from an official CSCI judgement.

The inspector found there to be outstanding requirements from the previous inspection report but made no statutory requirements on the home.

What the care home does well

There is a relaxed and comfortable atmosphere in this home. Staff are friendly and helpful and service users commented upon feeling well cared for and treated as individuals. The manager has worked hard to update the way in which information is stored and retrieved. Many more records are stored on computer than at the last inspection with more being scheduled for computer back up in future. Staff said they all got along well as a team and felt supported in their role. The manager is approachable and encourages staff and service users alike to make comment upon the delivery of the service good or otherwise. This has led to an atmosphere of enthusiasm and optimism, which was evident in the attitude of staff and apparent in the positive comments made by those receiving the service.

What has improved since the last inspection?

The administration and recording of medication has improved since the last inspection. Record keeping and the storage of information have improved and systems are being developed to improve the ease with which this information may be retrieved and collated. An activity organiser has been recently appointed whose sole remit is to consult with service users and arrange suitable activities. All commented that meals had improved and were very good.

What the care home could do better:

The newly appointed manager has plans for developing the quality assurance system, which is at a planning stage at present. Records for health and safety maintenance must all be available for inspection. Although the storage of this information had improved, it remained difficult to obtain access to all the certificates as some were kept centrally.

The number of service users in categories OP and DE(E) who may be admitted for respite care is limited to a maximum of four (4) at any one time. 3rd June 2005 Date of last inspection Brief Description of the Service: Granville court is situated in Bridlington town centre. It is a period house, in keeping with those around it. The home is in close proximity to local amenities including transport, shops, health care and leisure facilities. It is registered to provide care for a maximum of 25 older people, some of whom have dementia. The accomodation has twenty single bedrooms and three floors with access by a passenger lift to the upper floors. The service users have the choice of two lounges. A small courtyard style garden is located at the rear of the property, which has been made accessible to service users. Wheelchair access to the home is facilitiated via a permanent ramp to the main entrance. This unannounced inspection took place on October 18th 2005. It took eight hours to complete which includes time for preparation and for writing the report. Two service users and several staff were spoken to. All were complementary regarding the care offered at the home. A manager has been appointed since the last inspection and an application for registration with CSCI is awaited. What the service does well: What has improved since the last inspection? The service user’s health, personal and social care needs are set out in an individual plan of care. Service users’ health care needs are fully met. Service users, where appropriate, are responsible for their own medication, and are protected by the home’s policies and procedures for dealing with medicines. Service users feel they are treated with respect and their right to privacy is upheld. Service users are assured that at the time of their death, staff will treat them and their family with care, sensitivity and respect. The Commission considers Standards 7, 8, 9 and 10 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 9 and 10. Service users are assured they will be treated with respect and with regard to their dignity. EVIDENCE: All medication is now recorded suitably with a clear system for recording returns. Only those staff who have received medication training administer medication. Further training is planned. Staff induction includes training in how to treat service users. Individual rooms and bathrooms are lockable, with access to staff in emergencies. All health care examinations are carried out in service users own rooms. Most rooms are single. Visitors may see service users in communal areas or in private. Access to a telephone is made for those who request this. Calls may be taken in private. Service users find the lifestyle experienced in the home matches their expectations and preferences, and satisfies their social, cultural, religious and recreational interests and needs. Service users maintain contact with family/ friends/ representatives and the local community as they wish. Service users are helped to exercise choice and control over their lives. Service users receive a wholesome appealing balanced diet in pleasing surroundings at times convenient to them. The Commission considers all of the above key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 12,13,14 and 15. Service users are enabled to exercise choice and control over their lives. EVIDENCE: An activities coordinator has recently taken up post. She said she had begun to consult with service users regarding activities and had received suggestions regarding outings, artwork, audio books and memory games from service users and staff. One service user said that staff were always friendly and kind and that any wishes were taken into consideration, regarding social or recreational activities. Visitors are welcome at any reasonable time and several visitors called during the inspection. Those who wish to take a meal are invited to do so. Service users said they could get up or retire at their convenience. Staff said they were given time to accompany service users on short outings to the shops or for a walk on a one to one basis from time to time. Service users and staff alike reported very favourably on the quality and variety of meals offered by the recently appointed cook. She had devised new menus in consultation with service users. The home is clean, pleasant and hygienic. The Commission considers Standards 19 and 26 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 26 Service users live in a pleasant, clean environment. EVIDENCE: The home has two washing machines, one newly replaced. Although the home does not have a designated person in charge of laundry, no problems with the laundry were reported. The laundry is in a suitable place and a sluicing facility is available. Service users benefit from the ethos, leadership and management approach of the home. The home is run in the best interests of service users. Service users are safeguarded by the accounting and financial procedures of the home. Service users’ financial interests are safeguarded. Staff are appropriately supervised. Service users’ rights and best interests are safeguarded by the home’s record keeping, policies and procedures. The health, safety and welfare of service users and staff are promoted and protected. At present it is not possible to confirm that the home is run in the best interests of the service users as quality assurance is at a planning stage. EVIDENCE: The home has plans to carry out a thorough Quality Assurance programme with a quarterly report. Questionnaires for service users, visitors and others were seen, but these have yet to be fully implemented. One service user said he felt he was consulted adequately over the way in which the service was provided, both on an individual basis and at meetings. The home keeps personal allowances for a number of service users with records kept. The acting manager has re organised the storage of information following the sudden departure of the previous manager. Most of the maintenance certificates were seen, however, some of the certificates are held centrally and copies need to be available for inspection at each home.
