Conditions of registration: Service users to include up to 3 (LD) Date of last inspection 21/01/05 Brief Description of the Service: The home is a 3 bedded bungalow currently registered as a care home for Younger Adults with Learning Disabilities offering “Residential Personal Care.” The Bungalow is situated in Wheathills Road, Huyton close to local amenities. Community Integrated Care is the named company. The Registered Manager is Mr Ian Campbell and the Registered Provider is Dr D E Robertson. This is an overview of what the inspector found during the inspection. The inspection took place over 3.5 hours. It was an unannounced visit and was carried out as part of the regulatory requirement for care homes to be inspected at least twice a year. There has been no cause for any visits to the home since the last routine inspection in 21/1/05. A partial tour of he bungalow was conducted. Care records and other care home records were inspected. The Two Staff on duty had general discussions with the Inspector and the Inspector met all 3 of the Residents who had resided at the bungalow for many years What the service does well: What has improved since the last inspection?
There is a plan for the gradual and ongoing maintenance and upgrading of the home. Some work has been undertaken since the last inspection and all doors now have electronic door wedges which promotes further safety within the bungalow.

The intended outcomes for Standards 1 – 5 are: 1. 2. 3. 4. 5. Prospective service users have the information they need to make an informed choice about where to live. Prospective users’ individual aspirations and needs are assessed. Prospective service users’ know that the home that they will choose will meet their needs and aspirations. Prospective service users have an opportunity to visit and to “test drive” the home. Each service user has an individual written contract or statement of terms and conditions with the home. The Commission consider Standard 2 the key standard to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) Not measured at this inspection. EVIDENCE: As identified at the home’s previous inspections, the home has three long standing Service Users and there is no movement with the current status, so at present time there are no new admissions into the home. The intended outcomes for Standards 6 – 10 are: 6. 7. 8. 9. 10. Service users know their assessed and changing needs and personal goals are reflected in their individual Plan. Service users make decisions about their lives with assistance as needed. Service users are consulted on, and participate, in all aspects of life in the home. Service users are supported to take risks as part of an independent lifestyle. Service users know that information about them is handled appropriately, and that their confidences are kept The Commission considers Standards 6, 7 and 9 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 6.9 A review of personal support should take place so that the resident’s personal choices are not restricted. There continues to be progress made within care plans which include details for health, personal and social care needs of Residents. EVIDENCE: Individual plans of care are available and identify relevant aspects of health and personal care and plan accordingly and the staff have helped develop a style of plan called “essential lifestyle plans” that identifies a planned approach to individual choice, likes and dislikes. One plan was noted to be in need of updating due to a recent change to the Residents needs however it was acknowledged that the daily records gave a detailed description of all actions taken by Staff to improve the Residents health. It was noted that there had been no activity and support for one Resident for a week as the Manager was on holiday and Staff stated they were unable to manage the wheelchair and only the Manager was able to effectively support the Resident. This was discussed with the Manager following this inspection and identified a need for the company to review this situation as a matter of priority so that the Residents needs and choice should not be restricted especially as the manager was currently working between 2 homes and

The intended outcomes for Standards 11 - 17 are: 11. 12. 13. 14. 15. 16. 17. Service users have opportunities for personal development. Service users are able to take part in age, peer and culturally appropriate activities. Service users are part of the local community. Service users engage in appropriate leisure activities. Service users have appropriate personal, family and sexual relationships. Service users’ rights are respected and responsibilities recognised in their daily lives. Service users are offered a healthy diet and enjoy their meals and mealtimes. The Commission considers Standards 12, 13, 15, 16 and 17 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 12.13.15.17 Further review is needed by the company in supporting Residents with employment in the community and in transport. Family members are fully informed and involved in regular contact were requested. Procedures for meals are to be reviewed including updated training. EVIDENCE: Staff were observed to have a good rapport with Residents and were observed to assist Residents in various choices. The care plans, essential life plans and daily record book all emphasise the importance of promoting as much choice as the Residents are able to exercise however the issue of supporting one Resident in the community has recently been restricted as Staff explained they find they are unable to manoeuvre the wheelchair and Staff were unsure when one Resident would be going back to their job which they thought had temporarily stopped due to staffing problems. The home no longer have its own minibus and each Resident now has a mobility allowance to enable them to access either public transport or taxi’s. Staff are aware of each Residents likes and dislikes to food and prepare meals daily, assistance with Residents during meal times was discussed with the Manager following this inspection

Care records show all relevant persons important to each Resident and support is given by Staff to maintain regular contact or communication. The intended outcomes for Standards 18 - 21 are: 18. 19. 20. 21. Service users receive personal support in the way they prefer and require. Service users’ physical and emotional health needs are met. Service users retain, administer and control their own medication where appropriate, and are protected by the home’s policies and procedures for dealing with medicines. The ageing, illness and death of a service user are handled with respect and as the individual would wish. The Commission considers Standards 18, 19, and 20 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 19.20 Medication procedures were found to be in good order. Care records are detailed however one care plan needed to be updated this standard was met in part. EVIDENCE: Staff described how they have recently made contact with members of the multi disciplinary team including District Nurses. Residents are assisted in accessing these services in the community by attending appointments at local clinics. Care records identified access to these services, however one care plan needed to be updated and include all the referrals and actions taken by Staff to improve one Residents health. The home has its own medication cupboard and Staff manage all aspects of each Residents medication. The intended outcomes for Standards 22 – 23 are: 22. 23. Service users feel their views are listened to and acted on. Service users are protected from abuse, neglect and self-harm. The Commission considers Standards 22, and 23 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 23 Improvements have been made in this standard with just one outstanding issue to now be attended to regarding all monies to eventually be paid into each Individuals account. EVIDENCE: Although standard 23 has not been measured in full at this inspection the Manager has contacted CSCI following this inspection and stated verbally that the outstanding requirement regarding the Residents management of their money has been met. He explained that each Resident now has access to their own account and money is paid into this and all records held at head office are now fully accessible to them, the only outstanding issue is the payment of mobility money which the manager explains the company are arranging for this to be paid directly into each Residents account in the next month, the Company now need to give the Residents ands Manager a confirmed date of when this procedure will be put in place. Service users’ bedrooms promote their independence. Service users’ toilets and bathrooms provide sufficient privacy and meet their individual needs. Shared spaces complement and supplement service users’ individual rooms. Service users have the specialist equipment they require to maximise their independence. The home is clean and hygienic. The Commission considers Standards 24, and 30 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 24.30 This standard is met and the home offers a safe and comfortable environment. EVIDENCE: The Bungalow was found to be well maintained and decorated to a good standard, with all areas being clean and tidy. Staff explained that they were just waiting for the kitchen flooring to be replaced however they did not have a date and this was noted at the previous inspection. A confirmed date should now be given to Residents and Staff to replace the kitchen flooring to improve this area of the bungalow as its old and stained and looks worn to the rest of the environment. Service users benefit from well supported and supervised staff. The Commission considers Standards 35 the key standard to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 35 This standard was met in part around mandatory training and further developmental training was noted as a possible need to review and develop by the Company. EVIDENCE: Staff described the various training sessions they had attended recently and explained that they were happy with all of the courses they had attended and felt they were appropriate to the work they carry out. Two areas of possible updates to training identified in this inspection involved the use of wheelchairs and procedures for nutrition and support of people with swallowing difficulties. Service users benefit from competent and accountable management of the service. The Commission considers Standards 39, and 42 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 42 This standard was met with updated risk assessments in place. EVIDENCE: Risk assessments are carried out by the homes Manager and reviewed on a regular basis, the issue around doors being wedged open and the risks associated to this has been addressed with the installation of magnetic door releases put on most doors within the bungalow.. The Manager has spoken to the Inspector following this inspection and explained that he is currently managing two homes as a temporary measure, this should be reviewed and stopped b y the company as it was found to impinge on the support needed for one Resident who had not been out for a week while the Manager was on holiday. STATUTORY REQUIREMENTS This section sets out the actions which must be taken so that the registered person/s meets the Care Standards Act 2000, Care Homes Regulations 2001 and the National Minimum Standards. The Registered Provider(s) must comply with the given timescales. No. 1. Standard 23 Regulation 20 Requirement For the Responsible Person to provide evidence to the CSCI and the Manager and Residents, that outstanding monies kept in head office are arranged to be paid into each Residents individual account as a matter of priority. The Responsible Person must ensure that the staffing of the home meets the ongoing needs of the Service Users, and submit evidence to the CSCI describing the actions taken to meet this regulation and inform the Resident of actions to be taken to support them both in the community and in their place of work.
