Conditions of registration: To allow 3 beds for service users aged 55 years and over Date of last inspection 11th January 2005 Brief Description of the Service: Blenheim Court is a residential nursing home providing care and accommodation for 44 older people, including care for up to three people between the ages of 55 and 65. Blenheim Court is situated in the North of Sheffield, close to Sheffield Lane top. The home is within a short driving distance to local amenities such as shops, and churches; and local shops and pubs can be reached within a short walk from the home. The home is a two-storey building with lift access to the upper level. The home is accessible for wheelchair users. The home has both single and double accommodation.The home is split into two wings consisting of the original building and an extension. The homes gardens are accessible to wheelchair users and there is a patio with seating area. This is an overview of what the inspector found during the inspection. This was an unannounced inspection carried out from 09:35 to 13:35. Julie Wright,deputy manager was present during the inspection. Nine residents, five relatives and five staff were spoken with. A sample of records were examined and a partial inspection of the building was carried out. The inspector wishes to thank the staff, relatives and residents for their time, friendliness and co-operation throughout the inspection process. Resident contracts need to contain more details on the fees charged. The quality and presentation of the food needs some improvement. The lounge carpets require additional cleaning or replacing. Staffing levels must be maintined and increased supervision of residents in two lounges is needed. The frequency of practice fire drills must be increased. The intended outcomes for Standards 1 – 6 are: 1. 2. 3. 4. 5. 6. Prospective service users have the information they need to make an informed choice about where to live. Each service user has a written contract/ statement of terms and conditions with the home. No service user moves into the home without having had his/her needs assessed and been assured that these will be met. Service users and their representatives know that the home they enter will meet their needs. Prospective service users and their relatives and friends have an opportunity to visit and assess the quality, facilities and suitability of the home. Service users assessed and referred solely for intermediate care are helped to maximise their independence and return home. The Commission considers Standards 3 and 6 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 1,2 and3. Standard 6 is not applicable to this home. The Statement of Purpose and Service User Guide provided sufficient information. Residents’ contracts did not contain a detailed breakdown of fees charged by the home. Residents’ needs had been assessed. EVIDENCE: The homes Statement of Purpose and Service User Guide were informative and up to date which would enable prospective residents to make an informed decision about admission to the home. Copies of the statement of purpose were available in the reception area of the home. Residents had been issued with a contract. The information in the contracts did not clearly inform residents about the fees charged by the home. Two resident files were checked and each contained a copy of their full needs assessments. The information from the full needs assessment had been incorporated into the resident care plans. This will enable staff to plan and meet the residents’ needs.

The intended outcomes for Standards 7 – 11 are: 7. 8. 9. 10. 11. The service user’s health, personal and social care needs are set out in an individual plan of care. Service users’ health care needs are fully met. Service users, where appropriate, are responsible for their own medication, and are protected by the home’s policies and procedures for dealing with medicines. Service users feel they are treated with respect and their right to privacy is upheld. Service users are assured that at the time of their death, staff will treat them and their family with care, sensitivity and respect. The Commission considers Standards 7, 8, 9 and 10 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 7,8 and 10. Care plans detailed a significant amount of information relating to health, personal and social needs. A range of health care professionals visited the home to assist in maintaining the health care needs of residents. Residents themselves said that the care they were receiving was very good and that the staff were friendly, helpful and polite. Relatives said that the care delivered by staff was excellent. Each set out individual needs and the action required by staff to ensure those needs were met. Discussion with residents and relatives identified that a range of health professionals visited the home to assist in maintaining the residents health care needs. Residents or their relatives were involved in drawing up the plans. Residents said that they were happy and that the staff were very caring and “marvellous”. Relatives said that they thought the staff of the home were” wonderful” and that the care delivered by them was “excellent”. Residents were well dressed in clean clothes and had received a good standard of personal care. Residents and relatives said that staff were friendly, helpful and polite. Staff were observed to be assisting residents in a positive and friendly manner, doors were closed where staff were helping with personal care. Service users find the lifestyle experienced in the home matches their expectations and preferences, and satisfies their social, cultural, religious and recreational interests and needs. Service users maintain contact with family/ friends/ representatives and the local community as they wish. Service users are helped to exercise choice and control over their lives. Service users receive a wholesome appealing balanced diet in pleasing surroundings at times convenient to them. The Commission considers all of the above key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 12,13,14 and 15. On the whole the meal served at the home on the day of inspection was not of a good quality. Residents had a choice of lifestyle within the home and they were able to maintain contact with family and friends. Blenheim Court Nursing Home J55 S21770 Blenheim Crt V236473 11.07.05 UI Stage 4.doc Version 1.40 Page 12 EVIDENCE: Throughout the day friends and family were seen visiting freely and being offered hospitality, which creates a home that people want to visit. A friendly and welcoming feel was evident in Blenheim Court. Residents and relatives spoke positively about the amount of activities available both within the home and on trips outside the home. Residents said that they had really enjoyed a barge trip last week. Residents said they chose when they got up and went to bed. Lunch was served in a pleasant relaxed manner with staff sat with some residents assisting them to eat. Six residents said that they had not enjoyed their lunch, as the texture of the food was dry. One resident was adding copious amounts of relish to their meal because they said the meal was too dry. The portions of food did appear very large and there was a fairly large amount of waste left on several of the residents’ plates. The residents did say that generally the food served was of a good quality but that the odd meal was not very nice. The appearance of the pureed diets was poor and did not look appetising as the meat and vegetables were pureed together and not individually pureed and served in three separate portions on a plate. Complaints procedures are in place to enable residents and relatives to feel confident that any concerns they voice will be listened to. EVIDENCE: Complaints procedures were displayed around the home. Relatives and residents said that if they had any concerns that they would feel comfortable in talking to the staff or the manager. The Commission considers Standards 19 and 26 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 19,20,21,22,24,25 and 26. The environment within the home was generally well maintained and clean providing a comfortable, safe environment for residents. Blenheim Court Nursing Home J55 S21770 Blenheim Crt V236473 11.07.05 UI Stage 4.doc Version 1.40 Page 15 EVIDENCE: The grounds around the home were very welcoming and residents and their relatives were sat outside on furniture in the well-tended gardens. Since the last inspection refurbishment of the home has continued. The lounges and corridors have been redecorated which has markedly improved the aesthetics of the home. Some lounge carpets in the home are marked and need either thorough cleaning or replacing. However the environment within the home was on the whole, well maintained providing a comfortable, safe environment for residents. Three bedrooms were checked in detail and many others seen, all were comfortable and homely. Residents said their beds were comfortable. Bed linen checked was clean and in a good condition. The home was clean, with no unpleasant odours noticeable. Relatives and residents said that the home was always kept clean. The home was warm in all areas. Staff said that there were enough hoists available to ensure that residents could be safely moved. Service users are supported and protected by the home’s recruitment policy and practices. Staff are trained and competent to do their jobs. The Commission considers Standards 27, 29, and 30 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 27. Staff were not employed in sufficient numbers to meet the needs of residents in accordance with agreed staffing levels. EVIDENCE: Staffing arrangements and numbers meant that at times residents’ needs may not be met. There was a shortfall of two care assistants, due to staff sickness, which meant that agreed staffing levels were not met. Residents were not being supervised in two lounges in the home for periods of up to ten minutes. There was little stimulation for the residents in these lounges during the morning period whilst staff were attending to the other residents care needs. Residents did say that generally there was always a member of staff available when they needed them. Staff said staffing levels were adequate providing that there was no staff sickness. Service users live in a home which is run and managed by a person who is fit to be in charge, of good character and able to discharge his or her responsibilities fully. Service users benefit from the ethos, leadership and management approach of the home. The home is run in the best interests of service users. Service users are safeguarded by the accounting and financial procedures of the home. Service users’ financial interests are safeguarded. Staff are appropriately supervised. Service users’ rights and best interests are safeguarded by the home’s record keeping, policies and procedures. The health, safety and welfare of service users and staff are promoted and protected. The Commission considers Standards 33, 35 and 38 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 31,32,33 and 38. There was a positive style of management in the home and staff moral was good. Further staff fire and food hygiene training is needed to protect the safety of the residents and staff. In the main however, the homes policies and procedures promoted the health, safety and welfare of residents and staff. Blenheim Court Nursing Home J55 S21770 Blenheim Crt V236473 11.07.05 UI Stage 4.doc Version 1.40 Page 18 EVIDENCE: Relatives and residents said that they met regularly with the manager and spoke positively about her approachability and helpfulness. Staff said that they enjoyed working at the home and regularly attended staff meetings. Records were securely stored around the home, which protected the residents’ best interests and confidentiality. Staff said they had received recent fire safety training .A sample of records showed that staff were receiving this and other statutory training. The safety of the residents and staff could not be fully maintained however because: • A practice fire drill had not been completed at the home since December 2004. • The deputy manager said that not all the catering staff had received food hygiene training. At the time of inspection fire exits were clear and window restraints were in situ at first floor windows checked to prevent falls. Hazardous products were safely stored in the home. The hot water temperature in one bathroom measured a safe temperature of 43 degrees centigrade. This will promote the safety and welfare of the service users.
