Conditions of registration: 1. Named Individuals The home may accommodate individuals as set out in a letter to the registered person dated 8 April 2003, which establishes the basis on which the individuals needs will be met by the home. Where necessary the homes Statement of Purpose shall reflect any changes in service provision required for this arrangement. This condition may not apply to anyone else other than the named individuals who fall outside the registered categories. Two staff members must be nominated to be key workers and associate key workers for those 3 named individuals in the category of LD (E). They must receive appropriate training specific to learning disabilities unless they have relevant experience and have previously received appropriate training. 9th November 2005 2. Date of last inspection Brief Description of the Service: Brockwell Court Care home is a purpose built home in the centre of Consett. It is within easy reach of the shops, library and town centre amenities. The home provides twenty-four hour nursing and personal care for older persons. There is a separate unit for people who have dementia. The home offers single room accommodation which all have en-suite facilities. There are sufficient toilet and bathroom areas located throughout the home, some with specialist adaptations for people who are less mobile. Several spacious lounge and dining areas are also available. The fees charged at the time of this inspection were between £365 and £579 per week. This is an overview of what the inspector found during the inspection. This unannounced inspection of Brockwell Court took place on the 14th & 15th June 2006. Records were examined and a tour of the building took place. Time was spent talking to service users, staff and relatives. The manager supplied some information on a pre inspection questionnaire. Ten service users and three relatives returned surveys to the Commission for Social Care Inspection (CSCI.) Information about these is reflected in the report. The inspection focussed on key standard outcomes for service users. What the service does well:
Service users and relatives gave positive comments about the care provided at Brockwell Court. Service users have access to health care and other professionals. There are safe systems in place for the administration of medication. Family and friends can visit the home at any time. Routines within the home are flexible and service users are encouraged to make choices. Residents say that their privacy is maintained and that they are respected. The company have policies and procedures in place to support staff working in the home. Health and Safety systems within the home protect service users, staff and visitors. The intended outcomes for Standards 1 – 6 are: 1. 2. 3. 4. 5. 6. Prospective service users have the information they need to make an informed choice about where to live. Each service user has a written contract/ statement of terms and conditions with the home. No service user moves into the home without having had his/her needs assessed and been assured that these will be met. Service users and their representatives know that the home they enter will meet their needs. Prospective service users and their relatives and friends have an opportunity to visit and assess the quality, facilities and suitability of the home. Service users assessed and referred solely for intermediate care are helped to maximise their independence and return home. The Commission considers Standards 3 and 6 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 2&3 Quality in this outcome area is adequate. This judgement has been made using available evidence including a visit to this service. Whilst admissions to the home are well managed some service users have not received a contract to tell them what is included in their fees. The home does not provide intermediate care and therefore assessment of Standard 6 is not applicable. EVIDENCE: Ten people returned questionnaires. Five people said that they had not received a contract from the home and did not know what was included in their fees.

The manager said that that she visits the prospective service user before admission to the home. The service user and their relatives are involved in this process. One family said that they had looked around the home before their relative went to live there. They said that there was enough information available for them to decide whether or not their relative would like to live in the home. The intended outcomes for Standards 7 – 11 are: 7. 8. 9. 10. 11. The service user’s health, personal and social care needs are set out in an individual plan of care. Service users’ health care needs are fully met. Service users, where appropriate, are responsible for their own medication, and are protected by the home’s policies and procedures for dealing with medicines. Service users feel they are treated with respect and their right to privacy is upheld. Service users are assured that at the time of their death, staff will treat them and their family with care, sensitivity and respect. The Commission considers Standards 7, 8, 9 and 10 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 7, 8, 9 & 10 Quality in this outcome area is adequate. This judgement has been made using available evidence including a visit to this service. Some care plans require more information to ensure that all care needs are appropriately met. Medication within the home is well managed promoting good health. EVIDENCE: The manager said that all of the service users have care plans. Four were looked at during the inspection. One service user did not have care plans for all of the needs identified in the admission assessment. Two of the service users spoken to said that they were aware of their plan of care but there was no evidence to confirm that they had been consulted. Where possible care plans should be signed to confirm that the service user has been involved in the decision about their care.

These include district nurses, doctors, and care managers. The inspector spoke to four people who said that they usually receive the support and care they need. One service user said, “The staff are really good, they got the doctor to come and look at my painful knee”. Medication systems were looked at during this inspection. The home uses a monitored dosage system. An audit of medication was carried out and found to be in order. Temperature recordings for the drug fridge in the upstairs treatment room were in excess of 8°(c). This was brought to the attention of the manager who agreed to attend to this immediately. She said that a fridge on another unit could be used until a new one arrived. Service users and relatives said that the staff are polite, friendly and treat people with respect. Service users find the lifestyle experienced in the home matches their expectations and preferences, and satisfies their social, cultural, religious and recreational interests and needs. Service users maintain contact with family/ friends/ representatives and the local community as they wish. Service users are helped to exercise choice and control over their lives. Service users receive a wholesome appealing balanced diet in pleasing surroundings at times convenient to them. The Commission considers all of the above key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 12, 13, 14 & 15 Quality in this outcome area is good. This judgement has been made using available evidence including a visit to this service. The activities are varied and provide recreation for most people living in the home. Family and friends can visit the home at any time and are made to feel welcome. The meals are of a good standard. Menus are varied and service users are given a choice. EVIDENCE: The home employs an activities co-ordinator. Several service users spoken to commented on activities. One said, “The activities are really good. I went out shopping yesterday”. A relative said that one of the reasons her mother chose the home was because the activities were good. One service user said, “The activities are too childish. I don’t bother anyway due to being deaf”. The manager told the inspector that the activities organiser is speaking to people on an individual basis to try and create a programme that suits individual needs and wishes. One relative said, “You can come and go as you please”. The dining area is spacious and comfortable. During lunch the atmosphere was relaxed and unhurried. Staff who were helping service users with their food did this in a courteous and discreet manner. Service users said that they like the food and that they get a choice. One said “Nice food, there is a choice but mine is limited due to medical problems. The cook always asks me how I like my food cooked”. Another said, “The food is good, portions are just right”. Service users’ legal rights are protected. Service users are protected from abuse. JUDGEMENT – we looked at outcomes for the following standard(s): 16 & 18 Quality in this outcome area is good. This judgement has been made using available evidence including a visit to this service. Service users can be assured that complaints are dealt with properly and that systems are in place to keep them safe. EVIDENCE: Information is available for service users and visitors to the home on how to make a complaint. Service users and families views are obtained through regular contact and an ‘open door policy’. Residents meetings are held to discuss any concerns or potential difficulties. Service users and relatives said that they feel confident in discussing any issues with the manager The policies and procedures regarding protection of service users are regularly reviewed and updated. These provide information and guidance to staff. A copy of Durham and Darlington Adult Protection Committees policy and procedures on adult protection matters is held in the home. Service users live in safe, comfortable bedrooms with their own possessions around them. Service users live in safe, comfortable surroundings. The home is clean, pleasant and hygienic. The Commission considers Standards 19 and 26 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 19 & 26 Quality in this outcome area is good. This judgement has been made using available evidence including a visit to this service. The environment is clean, comfortable and safe for service users. EVIDENCE: The inspector looked around the home and found it to be light and airy. Service users said that they could take their own possessions into the home to make their rooms more pleasant and homely. One service user said, “I love my bedroom and really like my new carpet”. During the tour of the building the inspector saw that the flooring in some of the bathrooms and dining areas was a little shabby. The manager said that she plans to have these replaced.

There were no unpleasant smells apparent on the day of inspection. All of the people who returned questionnaires said that the home is always clean and fresh. Service users are supported and protected by the home’s recruitment policy and practices. Staff are trained and competent to do their jobs. The Commission consider all the above are key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 27, 28, 29 & 30 Quality in this outcome area is adequate. This judgement has been made using available evidence including a visit to this service. Staff are recruited properly and in adequate numbers to meet the needs of the people who live in the home. EVIDENCE: The staffing rotas were examined during the inspection. During the morning one qualified nurse and ten care assistants are available throughout the home. Where service users’ needs increase, staffing levels are adjusted to provide an additional carer. The carers are reduced to nine during the afternoon and evening. During the night there is one qualified nurse and six care assistants available. Most of the service users spoken to said that staff are always available when they need them. One service user who prefers to spend most of the time in her bedroom said, “The staff are lovely. They are always around and pop in and out regularly to see if I am o.k.” Staff felt that these staffing levels were adequate but when there is sickness they sometimes have to work short.
Brockwell Court Care Home DS0000000703.V297787.R01.S.doc Version 5.2 Page 18 Training has recently taken place in fire safety, health and safety, infection control and Protection of Vulnerable Adults. Certificates to confirm this were seen in staff files. Due to staff changes within the home only 17 of staff are currently trained to NVQ Level 2. However several staff are currently enrolled on the course. When these have completed it there will be over 50 of staff with this qualification. Four staff files were audited. One was for a recently recruited member of staff. All contained evidence that the required checks are carried out before employment to protect service users. This includes two references, an enhanced criminal record bureau check (CRB) and a medical reference. Induction and training records are also available. Service users’ rights and best interests are safeguarded by the home’s record keeping, policies and procedures. The health, safety and welfare of service users and staff are promoted and protected. Service users can be assured that the home is well managed and they are given the opportunity to comment on how the home is run. Policies and procedures are in place to safeguard their health, safety and wellbeing. She has almost completed the Registered Managers Award. One service user said, “The manager is lovely. She helps me all of the time”. A member of staff said, “I am well supported and always know where I can contact the manager”. Meetings are held every Friday afternoon. Service users and families are welcome to attend. This gives people an opportunity to make their views about the home known. The area manager carries out a quality assurance and monitoring visit on a monthly basis. This covers all aspects of care delivery and environmental issues. Copies of these were available in the home. The administrator is responsible for the record keeping with regard to service user finances. The company audits these on a monthly basis to ensure that residents are protected. The manager confirmed that the home carries out regular Health & Safety checks. The inspector checked some of the records. Those viewed were up to date.
