Conditions of registration: Date of last inspection 31st January 2006 Brief Description of the Service: Bromfield House is a large detached property providing accommodation on two floors. The home is a registered residential service, providing care and support for up to 10 Adults over the age of 65, 8 rooms have en-suite facilities. The home is owned by Mr Paul Jackman and Mrs Teresa Oakey is the Registered Manager. The home employs a team of 11 care staff and 1 domestic worker. The home is located on the Isle of Sheppey, in the small town of Minster on Sea. It has local amenities close by. Bromfield House has comprehensive policies and procedures, and a transparent culture, which underpin the rights, fulfilment, dignity, privacy, independence and choice of the service users who live there. The home has a passenger lift to assist with any mobility difficulties of the Residents. The home has a gravelled parking area at the front of the house that can accommodate up to 10 cars. This is an overview of what the inspector found during the inspection. The site visit carried out on the 26th April 2007 completed the homes first Key Inspection under Inspecting for Better Lives (IBL). The evidence gathered for the Key Inspection consisted of looking at past inspection reports, Service History, a completed Pre Inspection Questionnaire, a site visit that included looking around the home, viewing documentation and speaking to people living and working at the home including the Manager. The evidence gathered has led to the outcome judgement that Bromfield is a good home with people living there confirming that ‘they are well cared for’ ‘the food is lovely’ ‘the staff are younger now which is nice’. The Manager is aware that although the paperwork being used is good and meets the requirements there is room for some adjustments that would improve the quality even more.

The intended outcomes for Standards 1 – 6 are: 1. 2. 3. 4. 5. 6. Prospective service users have the information they need to make an informed choice about where to live. Each service user has a written contract/ statement of terms and conditions with the home. No service user moves into the home without having had his/her needs assessed and been assured that these will be met. Service users and their representatives know that the home they enter will meet their needs. Prospective service users and their relatives and friends have an opportunity to visit and assess the quality, facilities and suitability of the home. Service users assessed and referred solely for intermediate care are helped to maximise their independence and return home. The Commission considers Standards 3 and 6 the key standards to be inspected. JUDGEMENT – we looked at outcomes for the following standard(s): 1,3,5,6 Quality in this outcome area is good, this judgement has been made using available evidence including a visit to this service. Potential new residents are assessed to ensure their needs can be met and are offered the opportunity to visit the home. EVIDENCE: Details of the last admission were looked at and documentation was seen to verify the following events took place. The prospective referrals daughter visited the home and was given the homes Statement of Purpose and Service User Guide, the manager had received the purchasing authorities assessment. The manager had also carried out an assessment of the prospective referral at Medway Hospital to ensure that their needs could be met at the home. Agreement was reached for a four week trial placement to take place before a permanent placement was agreed. The home does not accept referrals for Intermediate care only.

The intended outcomes for Standards 7 – 11 are: 7. 8. 9. 10. 11. The service user’s health, personal and social care needs are set out in an individual plan of care. Service users’ health care needs are fully met. Service users, where appropriate, are responsible for their own medication, and are protected by the home’s policies and procedures for dealing with medicines. Service users feel they are treated with respect and their right to privacy is upheld. Service users are assured that at the time of their death, staff will treat them and their family with care, sensitivity and respect. The Commission considers Standards 7, 8, 9 and 10 the key standards to be inspected. JUDGEMENT – we looked at outcomes for the following standard(s): 7,8,9,10,11 Quality in this outcome area is good. This judgement has been made using available evidence including a visit to this service. People using the service are treated with respect and have their needs set out in a care plan with their health care needs met. EVIDENCE: People living at the home have their needs set out in an individual care plan, two of the plans were looked at and found to provide detail of action that needs to be taken to ensure that all aspects of the health, personal and social care needs, where possible the input from family and relatives was included. The plans had been reviewed and evaluated on a monthly basis. One of the care plans viewed had a procedure for staff to follow regarding the results of blood tests outside of the set limits relating to blood sugar levels. All of the people living at the home are registered with a Doctor, Dentist and Optician and there are six weekly visits from a chiropodist. The home has a record of wishes on all individuals regarding death and illness.

The intended outcomes for Standards 12 - 15 are: 12. 13. 14. 15. Service users find the lifestyle experienced in the home matches their expectations and preferences, and satisfies their social, cultural, religious and recreational interests and needs. Service users maintain contact with family/ friends/ representatives and the local community as they wish. Service users are helped to exercise choice and control over their lives. Service users receive a wholesome appealing balanced diet in pleasing surroundings at times convenient to them. The Commission considers all of the above key standards to be inspected. JUDGEMENT – we looked at outcomes for the following standard(s): 12,13,14,15 Quality in this outcome area is good. This judgement has been made using available evidence including a visit to this service. People living at the home have contact with friends and family and are encouraged to participate in activities and outings. The food provided is wholesome and nutritious. EVIDENCE: The Manager is looking into a new service being provided in the area relating to the hiring of transport, a mini bus, for half or full days so that more trips out can be arranged, it is hoped that family and/or friends will also accompany them on these outings. The Manager is aware that the almost half of the people living in the home are in their nineties, two are 100 years old this year, so trips and outings although offered are not always taken up, it is hoped that by including family and friends this may increase. The home arranges for a singer of old time favourites to come into the home at regular intervals, this event goes down well with people living at the home who enjoy joining in and singing.

Comments from people living in the home included ‘I can do what I want but sometimes require help and this is always given’. JUDGEMENT – we looked at outcomes for the following standard(s): 16,18 Quality in this outcome area is good. This judgement has been made using available evidence including a visit to this service. People living at the home are protected from abuse. EVIDENCE: The complaints policy and procedure and forms are all available by the front door next to the signing in book. Neither the home or CSCI have received any concerns or complaints since the last inspection. One of the policies all staff read and sign at their induction is the Protection of Vulnerable Adults, this confirms that they have read and understood it. Staff who have not attended training in Vulnerable Adults have been booked on a course in the near future. All Staff have attended training in Dementia, 1st Aid, Food Hygiene and Moving and Handling. Service users live in safe, comfortable bedrooms with their own possessions around them. Service users live in safe, comfortable surroundings. The home is clean, pleasant and hygienic. The Commission considers Standards 19 and 26 the key standards to be inspected. JUDGEMENT – we looked at outcomes for the following standard(s): 19,21,23,24,25,26 Quality in this outcome area is good. This judgement has been made using available evidence including a visit to this service. People live in a safe, clean and well-maintained home that has ample personal and communal facilities to meet Individuals needs. EVIDENCE: All of the work has now been completed in upgrading the upstairs rooms to ensuite. People living in the home said that they ‘were very happy there’ ‘it was lovely’, bedrooms were not viewed on this occasion but at the last inspection were well furnished and there were personal belongings on display in individuals rooms. The home was clean, well furnished and in good decorative order, there was ample seating in the communal area that was comfortable and in pleasant

There was also several seating areas in the garden that people could use in the warmer weather. The Manager carried out regular checks of the home for Health and Safety. Service users are supported and protected by the home’s recruitment policy and practices. Staff are trained and competent to do their jobs. The Commission consider all the above are key standards to be inspected. JUDGEMENT – we looked at outcomes for the following standard(s): 27,28,29,30 Quality in this outcome area is good. This judgement has been made using available evidence including a visit to this service. The people living at the home are supported and protected by a staff team that are competent to do their jobs and by the homes recruitment practices. EVIDENCE: The home is fully staffed with two going through the induction process and will be working supernumerary until induction is complete. Two staff files were viewed and both contained all of the documentation set out in Schedule 2 of the Care Home Regulations 2001. The manager telephones references to confirm authenticity and completes interview notes that are kept on file. Staff are encouraged to do a voluntary shift (supernumerary) before accepting employment, they do not commence work until both POVA and CRBs are returned. The home employs nine staff, of these five have an NVQ level 2 or higher and four are about to start their NVQ level 2. Staff training has taken place on Vulnerable Adults on 13/4/07 for 3 staff also First Aid, Food Hygiene, Medication and Moving and Handling since the last inspection. Bromfield House Residential Care Home Ltd DS0000023922.V330236.R01.S.doc Version 5.2 Page 17 On looking through the training matrix, Dementia training was attended by two staff in November 2006 and four staff attended in 2004, it would benefit the home if staff attended to update and refresh their skills along with staff who have had no training in this area. A person living at the home said that ‘it’s nice that younger staff are being employed’ and ‘staff are very helpful and caring’. Service users live in a home which is run and managed by a person who is fit to be in charge, of good character and able to discharge his or her responsibilities fully. Service users benefit from the ethos, leadership and management approach of the home. The home is run in the best interests of service users. Service users are safeguarded by the accounting and financial procedures of the home. Service users’ financial interests are safeguarded. Staff are appropriately supervised. Service users’ rights and best interests are safeguarded by the home’s record keeping, policies and procedures. The health, safety and welfare of service users and staff are promoted and protected. The Commission considers Standards 31, 33, 35 and 38 the key standards to be inspected. JUDGEMENT – we looked at outcomes for the following standard(s): 31,33,35,38 Quality in this outcome area is good. This judgement has been made using available evidence including a visit to this service. The Manager is able to discharge their duties fully and in the best interests of the people living there. The health, safety and welfare of people living and working at the home are promoted and protected. EVIDENCE: The manager has worked at the home since 1998 and been the Registered Manager for over three years. They have put in place a good system of recording and evaluation of the care provided. People living at the home spoken to said that ‘it’s a lovely home’ ‘I am well cared for’.

The finances of the people living at the home were discussed and two people are responsible for and have their own money. The Manager is responsible for the personal allowances that other individuals receive, all expenditures are covered with receipts apart from the cost of the hairdresser, the Manager would put in place a system where a receipt was obtained. The Manager carries out regular weekly fire alarm tests and health and safety checks around the home, these were recorded and signed and dated.
