This inspection was carried out on 31st January 2006.

The inspector made no statutory requirements on the home as a result of this inspection and there were no outstanding actions from the previous inspection report.

What the care home does well

The home has a good program of activities for Residents to participate in if they choose to, this includes darts, bingo, dominoes, reminiscing, rings and snakes and ladders. The home has a nice relaxed atmosphere with a caring staff team that has a good hand over and communication system that ensures that they are updated and aware of any changes when they come on duty.

What has improved since the last inspection?

Staff have attended Fire prevention training and some work has been carried out to complete the variation on putting in an en-suite bathrooms, this has taken longer than intended and been difficult to do as the home has full occupancy and the work is being carried out with as little disruption to the Residents as possible. The home has completed work in the garden and now has a fishpond, new seating area and a ramp has been made to allow access for wheelchairs.

What the care home could do better:

Following the Quality Management Review, the way the menu is devised is being changed, at present Residents are asked what they would like to eat on the day, the review returned 1 comment that asked for specific foods to be available more often, this has been addressed.

The home is a registered residential service, providing care and support for up to 10 Adults over the age of 65, 7 rooms have en-suite facilities. The home is owned by Mr Paul Jackman and Mrs Teresa Oakey is the Registered Manager. The home employs a team of 11 care staff and 1 domestic worker. The home is located on the Isle of Sheppey, in the small town of Minster on Sea. It has local amenities close by. Bromfield House has comprehensive policies and procedures, and a transparent culture, which underpin the rights, fulfilment, dignity, privacy, independence and choice of the service users who live there. The home has a passenger lift to assist with any mobility difficulties of the Residents. The home has a gravelled parking area at the front of the house that can accommodate up to 10 cars. Graham Cummings carried out the Announced Inspection on January 31st 2006. The inspection consisted of discussions with the Manager and Provider, 2 staff, 3 Residents, looking at 2 staff and 2 Residents files and viewing the home, prior to the inspection the Inspector received a completed Pre Inspection Questionnaire and 7 Service User and 8 Relative/Visitors comment cards. The Inspector received only positive comments from the information gathered from all parties, a Resident spoken to told the Inspector that ‘I am very happy here’ ‘the food is nice but I can never eat it all, there’s always too much’ ‘the staff are very nice and kind’. Comments from relatives and visitors included ‘I have always been happy with the care that Bromfield House has given’ ‘it is a well run home, staff are very friendly and nothing is too much trouble’ ‘my mother is very well cared for and looked after with care, kindness and consideration’. The paperwork seen was of a good quality with both staff and residents files containing all of the required information. There were 2 recommendations from the last inspection, the redecorating in the bedroom where the sink was removed has been completed, the 2nd recommendation has been addressed with 2 staff having attended fire prevention training and another 2 booked to attend in March 2006. The home has recently completed a Management Quality Review, the Manager was pleased with the response but was disappointed that there were very few comments on ways to change or improve the service. The Inspector left the home with no concerns for the health, safety or welfare of the Residents. What the service does well: What has improved since the last inspection? Prospective service users have the information they need to make an informed choice about where to live. Each service user has a written contract/ statement of terms and conditions with the home. No service user moves into the home without having had his/her needs assessed and been assured that these will be met. Service users and their representatives know that the home they enter will meet their needs. Prospective service users and their relatives and friends have an opportunity to visit and assess the quality, facilities and suitability of the home. Service users assessed and referred solely for intermediate care are helped to maximise their independence and return home. The Commission considers Standards 3 and 6 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 1,2,3,5,6 Prospective Residents have the information required to make an informed decision about moving to the home. Residents have a written contract with the home. Residents do not move into the home without having an assessment of needs. Prospective Residents and family are offered the opportunity to visit the home prior to making a decision to move. The home does not cater for Intermediate care. EVIDENCE: The home has a Statement of Purpose and Service User Guide that both reflect the way the home functions. On receipt of a telephone call enquiring about a possible placement, the Manager sends out copies of the Statement of Purpose and Service User Guide and arranges to make a visit to the prospective resident to assess if the home can meet the individuals needs, the next stage is to invite the individual and family to the home to ensure that they like it, if all parties are happy then a date for admission is agreed. The home does not cater for intermediate care. The service user’s health, personal and social care needs are set out in an individual plan of care. Service users’ health care needs are fully met. Service users, where appropriate, are responsible for their own medication, and are protected by the home’s policies and procedures for dealing with medicines. Service users feel they are treated with respect and their right to privacy is upheld. Service users are assured that at the time of their death, staff will treat them and their family with care, sensitivity and respect. The Commission considers Standards 7, 8, 9 and 10 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 7,8,9,10,11 Residents health, personal and social care needs are set out in an individual plan. Residents health care needs are fully met. Residents do not self medicate. Residents are treated with respect. Residents requests are noted on file about their wishes upon death. EVIDENCE: The Inspector looked at 2 of the Residents files and found them to contain good clear information regarding the care required and to be provided. The care plans had been evaluated recently to include any changing needs of the individuals. The files contained risk assessments and individual likes and dislikes. All of the Residents are registered with a local Doctor, Optician and Dentist. No Residents in the home self medicate at present, the home does have policies and procedures in place should the need arise. The Inspector when speaking to Residents was told that the ‘staff were excellent’ ‘staff treat you very well’ ‘staff are kind and helpful’. The Manager has devised a sensitive letter regarding Residents wishes at the time of death, this is sent with the initial documentation of Statement of Purpose and Service User Guide. Service users find the lifestyle experienced in the home matches their expectations and preferences, and satisfies their social, cultural, religious and recreational interests and needs. Service users maintain contact with family/ friends/ representatives and the local community as they wish. Service users are helped to exercise choice and control over their lives. Service users receive a wholesome appealing balanced diet in pleasing surroundings at times convenient to them. The Commission considers all of the above key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 13,14,15 Residents maintain contact with family and friends as they wish. Residents are encouraged to exercise choice and control over their lives. Residents receive a nutritious and wholesome diet. EVIDENCE: Residents family and friends can call at anytime, wherever possible the home tries to avoid mealtime visits but this is not a problem should anyone turn up. Residents have access to a hairdresser who comes into the home every week and can choose whether to have their hair done. Residents are encouraged to choose what clothes they would like to wear and what they would like to eat on a daily basis, however following the Management Quality Review the Manager is going to make some minor changes. The food according to 1 Resident ‘is nice but I can never eat it all, there’s always too much’. Service users and their relatives and friends are confident that their complaints will be listened to, taken seriously and acted upon. Service users’ legal rights are protected. Service users are protected from abuse. The Commission considers Standards 16 and 18 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 16,18 Residents and relatives complaints are listened to and taken seriously. Residents are protected from abuse. EVIDENCE: The home has displayed the Complaints Procedure near the front door by the signing in book, 7 of the 8 comment card responses indicated that they were aware of the homes complaints procedure. Neither the home or the Inspector have received any complaints. Most of the staff have attended an adult protection training course and 2 staff are booked to attend one in the near future. Service users have sufficient and suitable lavatories and washing facilities. Service users have the specialist equipment they require to maximise their independence. Service users’ own rooms suit their needs. Service users live in safe, comfortable bedrooms with their own possessions around them. Service users live in safe, comfortable surroundings. The home is clean, pleasant and hygienic. The Commission considers Standards 19 and 26 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 19,20,21,23,24,25,26 Residents live in a safe well-maintained environment that has comfortable indoor and external communal facilities. Residents individual rooms suit their needs and are furnished with personal possessions. Residents live in safe comfortable surroundings that are clean, pleasant and hygienic. EVIDENCE: The Inspector spoke to Residents and toured the home and viewed some bedrooms, the rooms were well decorated and furnished with personal belongings and were clean and tidy with no offensive odours. The recommendation in the last inspection report that required decorating following the removal of a sink has been completed. The home has now got 7 bedrooms with en-suite facilities and the 8th room has been started, this is taking longer than anticipated as the room is occupied and there are only certain times that the room can been accessed. Service users’ needs are met by the numbers and skill mix of staff. Service users are in safe hands at all times. Service users are supported and protected by the home’s recruitment policy and practices. Staff are trained and competent to do their jobs. The Commission consider all the above are key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 27,29,30 Residents needs are met by the numbers and skill mix of the staff. The homes recruitment process protects residents. Staff are trained and competent to do their jobs. EVIDENCE: The home employs a total of 12 staff, 4 Wake night, 7 Care and 1 cleaner, the Manager works 9 – 5 Monday to Friday and is supernumerary to the care staff, they are available to work evenings, weekends and wake nights if required. The Inspector went through 2 staff files and found them to contain all of the required documentation. The Inspector saw the training program containing the courses staff have attended and those planned for the coming year. The Inspector spoke to 2 staff who confirmed that the running and management of the home was extremely good. Service users live in a home which is run and managed by a person who is fit to be in charge, of good character and able to discharge his or her responsibilities fully. Service users benefit from the ethos, leadership and management approach of the home. The home is run in the best interests of service users. Service users are safeguarded by the accounting and financial procedures of the home. Service users’ financial interests are safeguarded. Staff are appropriately supervised. Service users’ rights and best interests are safeguarded by the home’s record keeping, policies and procedures. The health, safety and welfare of service users and staff are promoted and protected. The Commission considers Standards 31, 33, 35 and 38 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 31,32,33,34,35,36,37,38 The Manager has a clear vision for the home, which she has effectively communicated to the Service Users, staff and stakeholders. The home is run in the best interests of the residents. Residents finances are safeguarded by the homes financial procedures. Staff are appropriately supervised. Residents rights and interests are safeguarded by the record keeping and policies and procedures. The health, safety and welfare of Residents are promoted and protected. EVIDENCE: The Manager is supported well by the Provider in providing clear leadership throughout the home with all staff demonstrating an awareness of their roles and responsibilities. The Inspector looked at 2 of the Residents files and it was clear to the Inspector on looking through them, speaking to Residents and staff and touring the home was run in the best interests of the Residents. The Manager went through the financial procedures and practices with the Inspector, this showed that the residents finances were safeguarded and Bromfield House Residential Care Home Ltd DS0000023922.V270335.R01.S.doc Version 5.1 Page 15 protected. The Inspector viewed the records and found them to be clear and well written. The Inspector had no concerns regarding the health, safety and welfare of the Residents.
