Conditions of registration: Date of last inspection 5th September 2005 Brief Description of the Service: Brook House is a private residential home providing personal care to 32 older people. It has been registered since 1984. The home is owned and managed by Mrs Kay Cosens. The home is in a quiet rural area, not far from the village of Morda and Trefonen and only a few miles away from the town of Oswestry. Brook House stands in its own attractive and well-maintained grounds with outlooks over gardens and surrounding fields. A patio area offering seating is available at the rear of the home. A number of raised flowerbeds with a variety of plants and flowers with varying fragrances can be enjoyed by all service users, families and friends. This area is particularly beneficial to those service users with a sensory impairment. The home has gone through major refurbishments and extension. The improvements to the home have provided more individual space, new assisted baths, showers and en-suite facilities. Accommodation is provided on ground and first floors, with a passenger lift providing access between first floor bedrooms and communal areas on the ground floor. The accommodation is well maintained, furnished and equipped to meet the needs of the service users. A range of communal lounges, dining rooms, the conservatory and smaller seating areas around the home provide choice for service users as to where they spend their time. This is an overview of what the inspector found during the inspection. The inspection was announced and started at 10.45am and lasted 3 and a half hours. The inspection included discussion with service users, relatives and staff. Observation of daily routines, examination of service user records and a home tour took place as part of the inspection. This inspection reviewed key standards only as the home is currently considered to be performing well and thus warrants the application of a reduced methodology. This report should be read in conjunction with the unannounced inspection report of 5th September 2005. What the service does well: What has improved since the last inspection?
The home continues to follow a development plan taking into account it’s stated aims and objectives. Each service user has a written contract/ statement of terms and conditions with the home. No service user moves into the home without having had his/her needs assessed and been assured that these will be met. Service users and their representatives know that the home they enter will meet their needs. Prospective service users and their relatives and friends have an opportunity to visit and assess the quality, facilities and suitability of the home. Service users assessed and referred solely for intermediate care are helped to maximise their independence and return home. The Commission considers Standards 3 and 6 the key standards to be inspected at least once during a 12 month period. The intended outcomes for Standards 7 – 11 are: 7. 8. 9. 10. 11. The service user’s health, personal and social care needs are set out in an individual plan of care. Service users’ health care needs are fully met. Service users, where appropriate, are responsible for their own medication, and are protected by the home’s policies and procedures for dealing with medicines. Service users feel they are treated with respect and their right to privacy is upheld. Service users are assured that at the time of their death, staff will treat them and their family with care, sensitivity and respect. The Commission considers Standards 7, 8, 9 and 10 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 9 Service users are protected by the homes polices and procedures for dealing with medication, and by designated staff who are trained to administer medication. EVIDENCE: It was stated on the pre inspection questionnaire completed by the manager as part of the pre inspection research that the home has a policy and procedure for receipt, recording, storage, handling, administration and disposal of medication. Records relating to the administration of medication were checked and seen to be satisfactory at the time of the visit. Service users find the lifestyle experienced in the home matches their expectations and preferences, and satisfies their social, cultural, religious and recreational interests and needs. Service users maintain contact with family/ friends/ representatives and the local community as they wish. Service users are helped to exercise choice and control over their lives. Service users receive a wholesome appealing balanced diet in pleasing surroundings at times convenient to them. The Commission considers all of the above key standards to be inspected at least once during a 12 month period. Service users’ legal rights are protected. Service users are protected from abuse. The Commission considers Standards 16 and 18 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 18 Service users are protected from abuse by the home’s policies and procedures. EVIDENCE: The pre inspection questionnaire completed by the manager declared the home has in place an adult protection policy. The document was not examined at this inspection. There is also a copy of the Multi Agency Adult Protection policy in place. Discussion with staff indicated a clear understanding of the action that should be taken should any form of abuse be suspected. The Commission considers Standards 19 and 26 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 25 The home is well maintained and provides a comfortable environment for service users to live in. EVIDENCE: Feedback from service users was positive in relation to their satisfaction with the environment they live in. Relatives/visitors who completed comment cards as part of the pre inspection research offered the following comments in relation to the homes environment. “The home is kept clean and bright and is a pleasure to visit”. “The home and garden provide an outstanding environment”. Service users are supported and protected by the home’s recruitment policy and practices. Staff are trained and competent to do their jobs. The Commission consider all the above are key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 27 & 30 The home is staffed to a level that ensures service user needs are met at all times. Standard 30 was not assessed as there had been no new staff appointed since the last inspection EVIDENCE: Staff seen and discussion with the senior carer confirmed staffing levels are appropriate to the number and needs of the service users living at the home. A duty rota is maintained, which confirmed observations made during the inspection and discussions held with staff. Service users live in a home which is run and managed by a person who is fit to be in charge, of good character and able to discharge his or her responsibilities fully. Service users benefit from the ethos, leadership and management approach of the home. The home is run in the best interests of service users. Service users are safeguarded by the accounting and financial procedures of the home. Service users’ financial interests are safeguarded. Staff are appropriately supervised. Service users’ rights and best interests are safeguarded by the home’s record keeping, policies and procedures. The health, safety and welfare of service users and staff are promoted and protected. The Commission considers Standards 31, 33, 35 and 38 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 31, 33 & 38 The home is managed by an experienced individual who carries out her responsibilities fully. There is an effective quality assurance system in place. Health, safety and welfare of service users and staff are promoted fully by safe working systems in place. EVIDENCE: Mrs Cosens has been the manager/proprietor since 1984. Through discussion it is apparent she has a sound knowledge of the needs of older people and is committed to providing a high standard of care to service users. Mrs Cosens is
Brook House DS0000020704.V268222.R01.S.doc Version 5.1 Page 15 supported by a senior team with individual delegated duties, to ensure the home is run effectively and efficiently. The home has an effective quality assurance system in place. An annual survey is sent out to service users. The results are collated and responded to by Mrs Cosens. A copy of the analysis and action taken is forwarded to CSCI each year. The last survey was received at CSCI in November 2005. Feedback from Health and Social Care professional’s relatives and service users was positive in relation to the quality of care at Brook House. Comments included, “Brook House seems to be a very well run establishment where the staff take a real interest in the residents”. “We are very satisfied with the care Mum receives”. “Excellent service”. “The staff are very helpful and ensure a very high level of care and comfort for the residents”. “Mrs Cosens and her staff work very hard too provide a caring and safe home for the residents, staff are very vigilant to the well being of our relative”. “My mother is always very positive about the care she receives. I have always found the manager and staff caring and helpful”. It was stated on the pre inspection questionnaire completed by the manager that there are a number of policies and procedures relating to the safe working practices within the home. These were not examined at this inspection. Two members of staff are due to attend fire risk assessment training in April 2006. This will qualify the staff to undertake fire risk assessment of the building. Matters pertaining to fire safety and environmental health were found to be satisfactory at the last inspection carried out by Shropshire Fire and Rescue service and Environmental Health Department. The home is awaiting the gas certificate, it was agreed that a copy of this will be forwarded to CSCI once it is in the homes possession.
