Conditions of registration: Date of last inspection 7th March 2005 Brief Description of the Service: Brook House is a private residential home providing personal care to 32 older people. It has been registered since 1984. The home is owned and managed by Mrs Kay Cosens.The home is in a quiet rural area, not far from the village of Morda and Trefonen and only a few miles away from the town of Oswestry.Brook House stands in it’s own attractive and well-maintained grounds with outlooks over gardens and surrounding fields. A patio area offering seating is available at the rear of the home. A number of raised flowerbeds with a variety of plants and flowers with varying fragrances can be enjoyed by all service users, families and friends. This area is particularly beneficial to those service users with a sensory impairment.The home has gone through major refurbishments and extension. The improvements to the home have provided more individual space, new assisted baths, showers and en-suite facilities.Accommodation is provided on ground and first floors, with a passenger lift providing access between first floor bedrooms and communal areas on the ground floor.The accommodation is well maintained, furnished and equipped to meet the needs of the service users.A range of communal lounges, dining rooms, the conservatory and smaller seating areas around the home provide choice for service users as to where they spend their time. This is an overview of what the inspector found during the inspection. The inspection was unannounced and started at 9.30am and lasted 3.5 hours. 31 places were occupied. The inspection included discussion with service users and staff. Observation of daily routines, examination of service user records and a home tour took place as part of the inspection. The intended outcomes for Standards 1 – 6 are: 1. 2. 3. 4. 5. 6. Prospective service users have the information they need to make an informed choice about where to live. Each service user has a written contract/ statement of terms and conditions with the home. No service user moves into the home without having had his/her needs assessed and been assured that these will be met. Service users and their representatives know that the home they enter will meet their needs. Prospective service users and their relatives and friends have an opportunity to visit and assess the quality, facilities and suitability of the home. Service users assessed and referred solely for intermediate care are helped to maximise their independence and return home. The Commission considers Standards 3 and 6 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 3 All service users are assessed before moving into Brook House. EVIDENCE: The three most recent service users who moved into the home were all assessed by the manager prior to moving in. Evidence of the homes assessments were seen on all three files. The intended outcomes for Standards 7 – 11 are: 7. 8. 9. 10. 11. The service user’s health, personal and social care needs are set out in an individual plan of care. Service users’ health care needs are fully met. Service users, where appropriate, are responsible for their own medication, and are protected by the home’s policies and procedures for dealing with medicines. Service users feel they are treated with respect and their right to privacy is upheld. Service users are assured that at the time of their death, staff will treat them and their family with care, sensitivity and respect. The Commission considers Standards 7, 8, 9 and 10 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 7, 8 &10 There is a good clear care planning process in place. The staff are sensitive to the needs of each service user and meet these in a professional manner. Service users right to be treated with respect and dignity is maintained by the staff group. EVIDENCE: All service users have a plan of care, these were well documented and detailed individuals needs and clear instructions to staff on how identified needs should be met in line with service user preferences. Information contained in the care plan demonstrated that individuals had been consulted in the care planning process, however confirmation by means of the service user or their representative signature should be evidenced upon completion of each plan. In the third case no evidence of a review could be located. After discussion with the member of staff responsible for reviews it was clarified that even those service users considered to be temporary must have their care plan reviewed each month. Written evidence, observations of health care professional visits during the inspection and discussions with service users informed the inspector that service users’ health care needs are fully met. Service user relatives and friends were seen during the inspection visiting their relative in private. Individual’s receiving care from staff and visiting health care professionals during the inspection were seen to be given this in the privacy of their own rooms. Many service users spoken to shared with the inspector that staff treated them extremely well with regard to respecting them as individuals and their right to privacy and maintaining their dignity. Service users find the lifestyle experienced in the home matches their expectations and preferences, and satisfies their social, cultural, religious and recreational interests and needs. Service users maintain contact with family/ friends/ representatives and the local community as they wish. Service users are helped to exercise choice and control over their lives. Service users receive a wholesome appealing balanced diet in pleasing surroundings at times convenient to them. The Commission considers all of the above key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 12, 13, 14 & 15 Service users expectations of the home and their lifestyles within the setting appear to be sensitively considered by staff. Service users are able to maintain contact with their family and friends as they wish. Service users are enabled to exercise choice and control over activities of daily living so far as practicable. Service users are offered a varied, well balanced diet, which takes into account individual likes and dislikes along with seasonal changes. EVIDENCE: Service users spoken to described a variety of activities taking place within the home on a regular basis. A list of activities arranged for September was on display on the hall notice board. Some service users spoken to were happy to share with the inspector that they regularly see their family and friends. Visitors are not restricted and welcomed

Service users told the inspector that where possible they are able to have choice and control over their lives. One service user told the inspector that they had only just risen at mid morning, a choice that is exercised frequently. Service users spoken to were very complimentary about the quality of food. The home has exceeded in this standard on previous occasions. The quality of food, variety of choice and presentation is always of an excellent standard. Service users’ legal rights are protected. Service users are protected from abuse. The Commission considers Standards 16 and 18 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 16 & 17 There is a clear complaints procedure in place. Service users legal rights are protected. EVIDENCE: It was stated by the manager that there have been no complaints received since the last inspection. Records were shown to the inspector to confirm this. There have been no complaints received at the Commission for Social Care Inspection. Service users legal rights are protected and individuals are enabled to exercise them as they wish. Discussion with the manager demonstrated that the home consider this issue important to individuals. A recent example of the use of advocacy involvement was shared with the inspector. The electoral register is maintained and service users told the inspector they used their postal votes to participate. One service user was taken to vote by the homes manager. Service users live in safe, comfortable bedrooms with their own possessions around them. Service users live in safe, comfortable surroundings. The home is clean, pleasant and hygienic. The Commission considers Standards 19 and 26 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 19 & 26 The standard of the environment is good providing service users with a safe well-maintained environment to live in. The standard of cleanliness was good in areas seen on the home tour. EVIDENCE: The location and layout of the home is suited well to it’s purpose. The home is well maintained and service users were complimentary with regard to the accommodation. A tour of the home during the inspection found the accommodation to be clean and comfortable. JUDGEMENT – we looked at outcomes for standard(s) 29 The homes recruitment processes are robust and all necessary recruitment checks are carried out before staff commence working at the home. EVIDENCE: The files of the three newest recruits to the home were examined and found to contain all the relevant pre employment checks.
