This inspection was carried out on 14th April 2005.

CSCI has not published a star rating for this report, though using similar criteria we estimate that the report is Adequate. The way we rate inspection reports is consistent for all houses, though please be aware that this may be different from an official CSCI judgement.

The inspector found there to be outstanding requirements from the previous inspection report but made no statutory requirements on the home.

What the care home does well

The home carries out a detailed pre-admission assessment. Resident`s needs are then identified and the home records how it can care for that resident. This will be discussed with residents and family. Medication is stored safely in the home and staff administer medication or residents can choose to look after their own medicines. Visits to the home by family and friends are encouraged as is trips to the local community. The home works hard to protect residents from abuse and staff spoken to were able to talk through what to do if abuse was seen. The home is able to look after resident`s personal money and keeps records of spending undertaken on resident`s behalf. The manger likes residents to give opinions on how the home is run and will act on to improve matters if needed.

What has improved since the last inspection?

The home now carries out risk assessments and manual handling assessments for all residents. The medication practice has improved with improved ways of ordering, receiving and recording of medication now in place. Family members are encouraged to sign residents care plans and are also asked to be involved in this process.

What the care home could do better:

The home needs to ensure that hot surfaces such as radiators are risk assessed and action taken so as to prevent burns to the residents. Staff files must contain the correct information relating to the staff member.

Some of the people cared for may also have a mental disorder or have dementia. The home is located in a residential area close to local amenities. Accomodation is provided over two floors and has a stair lift to gain access between floors. there are four shared rooms and sixteen single rooms. There is an extensive rear garden that is well maintained and secure. The inspection took place over 5.5 hours and was carried out as the first of two annual inspections. The manager of the home was present for the majority of the inspection. A tour of the premises took place and staff and care records were inspected. Two of the members of staff on duty, four of the service users and two visitors were spoken to. Prospective service users have the information they need to make an informed choice about where to live. Each service user has a written contract/ statement of terms and conditions with the home. No service user moves into the home without having had his/her needs assessed and been assured that these will be met. Service users and their representatives know that the home they enter will meet their needs. Prospective service users and their relatives and friends have an opportunity to visit and assess the quality, facilities and suitability of the home. Service users assessed and referred solely for intermediate care are helped to maximise their independence and return home. The Commission considers Standards 3 and 6 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 3 The home has a detailed pre-admission assessment that clearly indicates whether or not the home can meet the needs of the prospective resident. EVIDENCE: The pre-admission assessment is based on activities of daily living. The topics are divided into topics and then further sub divided to fully explore peoples needs. There is a space in each section for the assessor to comment on each section. One of the residents spoken to was able to describe the assessment That took place before being admitted. Staff spoken to were able to give account of the needs of the residents that the inspector had read about. Individual records are kept for each of the residents and the last four admissions had copies of their pre-admission assessments on file. The service user’s health, personal and social care needs are set out in an individual plan of care. Service users’ health care needs are fully met. Service users, where appropriate, are responsible for their own medication, and are protected by the home’s policies and procedures for dealing with medicines. Service users feel they are treated with respect and their right to privacy is upheld. Service users are assured that at the time of their death, staff will treat them and their family with care, sensitivity and respect. The Commission considers Standards 7, 8, 9 and 10 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 7 and 9 Care plans are in place ensuring the health, personal and social care needs of residents have been identified and met. Medication practices safe guard residents from risk or harm. EVIDENCE: Individual health, personal and social care needs are identified and planned for. Plans are based upon activities of daily living. Daily entries are made these are basic and consists of a few sentances per entry on a daily basis with the exception when an significant event occurs then a more detailed entry is documented. Risk assessments are also kept on file. Cross referencing indicates that no event that concerns a resident should be missed. Evaluation takes place on a monthly basis. Discussion with staff clearly showed that care plans were read and being followed. The home has a medication policy that staff follow. Medication is safely stored in a locked cupboard in the kitchen. It was noted that the home does not have a controlled drug record book. One needs to be obtained before controlled drugs are brought into the home. Service users find the lifestyle experienced in the home matches their expectations and preferences, and satisfies their social, cultural, religious and recreational interests and needs. Service users maintain contact with family/ friends/ representatives and the local community as they wish. Service users are helped to exercise choice and control over their lives. Service users receive a wholesome appealing balanced diet in pleasing surroundings at times convenient to them. The Commission considers all of the above key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 13 and 14 Service users are encouraged to keep contact with family and friends and the local community. Residents choices are respected. EVIDENCE: In the residents care plans a record is made on the daily notes of visitors. The majority of residents receive visitors frequently. One visitor spoken to comment how welcome the staff and management make you feel and are encouraged to visit whenever they like. It was observed that when family members go on holiday alternative contacts were in the care plan. Zimmer frames were placed to enable residents to move freely around the home. residents were seen to be given choice of where to eat their meals. Their preferences are recorded on their files. Staff spoken to were aware of residents likes and dislikes. Service users and their relatives and friends are confident that their complaints will be listened to, taken seriously and acted upon. Service users’ legal rights are protected. Service users are protected from abuse. The Commission considers Standards 16 and 18 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 18 The home aims to safe guard residents from abuse. EVIDENCE: A procedure is in place for responding to allegations of abuse. Staff spoken to were aware of how to respond to any allegations and who to report it to. The home follows the Hampshire guidelines. One of the residents spoken to said they would report any abuse straight to the manager. Service users live in a safe, well-maintained environment. Service users have access to safe and comfortable indoor and outdoor communal facilities. Service users have sufficient and suitable lavatories and washing facilities. Service users have the specialist equipment they require to maximise their independence. Service users’ own rooms suit their needs. Service users live in safe, comfortable bedrooms with their own possessions around them. Service users live in safe, comfortable surroundings. The home is clean, pleasant and hygienic. The Commission considers Standards 19 and 26 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 21, 25 and 26 Bathing and toilet facilities meet the needs of the residents. Safety needs to be improved in the home with regards to radiators. The home on the whole was clean and comfortable. EVIDENCE: Although the home has currently three bathrooms only one of these bathrooms is in use and that is the bathroom with the hoist. None of the residents can use the bath independently and therefore require the assistance of two staff to get in and out of the bath. One of the bathrooms is currently used as a storage room. Staff spoken to feel it would be better to have a bath on each floor. The residents spoken to received baths when they requested or at least once a week. There is adequate provision of WC’s around the home. There is a lot of uncovered radiators in the home one room had a bed next to a radiator with no headboard to protect from burning the resident. The position of the bed was moved and the manager agreed to cover the radiator as a matter of priority. No risk assessments were available for the uncovered radiators but the manager stated they have been completed. The home is pleasantly decorated in a good state of repair. The maintenance man has a book that details the work to be carried out in the home. Service users needs are met by the numbers and skill mix of staff. Service users are in safe hands at all times. Service users are supported and protected by the home’s recruitment policy and practices. Staff are trained and competent to do their jobs. The Commission considers Standards 27, 29, and 30 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 29 The homes recruitment practices does not safe guard the safety of residents. EVIDENCE: The staff files of four members of staff indicated that the required documentation regarding identification, references and CRB checks were not being kept on file in all cases. Staff have been started before CRB check being returned. This practice must stop. One file did not contain the information regarding that staff members right to work in this country. Two references must be obtained one being from the last employer or school. The manager stated that the checks had been carried out just not recorded. This is an outstanding requirement from the last inspection. Service users live in a home which is run and managed by a person who is fit to be in charge, of good character and able to discharge his or her responsibilities fully. Service users benefit from the ethos, leadership and management approach of the home. The home is run in the best interests of service users. Service users are safeguarded by the accounting and financial procedures of the home. Service users’ financial interests are safeguarded. Staff are appropriately supervised. Service users’ rights and best interests are safeguarded by the home’s record keeping, policies and procedures. The health, safety and welfare of service users and staff are promoted and protected. The Commission considers Standards 33, 35 and 38 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 33 and 35 The home has a strong management team that ensure the residents receive consistent good quality of care. Residents personal money is managed in their interests. EVIDENCE: This is a family run business with one of the providers being the home manager. She has recently completed her NVQ managers award. The home carries out anonymous resident and family surveys. The information is then collated and points acted upon by the management. Staff spoken to feel it would be good to have residents meetings. A family member commented that they felt happy to approach the management team to discuss any issue regarding the quality of care and is encouraged to do so by the manager. The home stores residents money individually and is kept secure. Receipts and records of transactions were seen and the totals balanced with money.
