This inspection was carried out on 16th August 2005.

CSCI has not published a star rating for this report, though using similar criteria we estimate that the report is Good. The way we rate inspection reports is consistent for all houses, though please be aware that this may be different from an official CSCI judgement.

The inspector made no statutory requirements on the home as a result of this inspection and there were no outstanding actions from the previous inspection report.

What the care home does well

The residents said that they liked the food and the staff. Residents see the staff as kind and caring, and staff have a good team spirit. Several of the staff have been at the home for many years, and have good relationships with residents. The proprietors are committed to staff training and to improving the home. All members of staff are encouraged to learn. Classes in communication skills for overseas staff are provided so that staff are better able to understand the needs of the residents. The proprietors are involved in the day to day running of the home, and provide good leadership for all the staff.

What has improved since the last inspection?

50% of the care staff now hold NVQ qualifications in care. Two members of staff are qualified first aid trainers. Care planning systems are better, and wound care has improved. New equipment is available at the home, including a new hoist, new furniture and linen, and all the beds at the home are now adjustable.

What the care home could do better:

The registered manager should make sure that all care plans contain appropriate detail to enable the carers to deliver the care needs. Clean laundry in the laundry room should be stored in an appropriate container. The induction programme should be developed to ensure that managers know that new staff achieve the desired competence levels by the end of the induction period.

Date of last inspection 22 September 2004 Brief Description of the Service: Brooklands 1 is a large town house within five minutes from Banbury town centre, with good access to the main shopping area, churches, bus and rail stations, library, hospital and doctors’ surgeries. The accommodation is spread over three floors, served by a passenger lift. There is a wheelchair lift for ease of access from the rear of the building and car park. Accommodation is provided in 9 single rooms and 7 double rooms. There is a combined lounge/dining area on the ground floor. There are small garden areas at the front of the building and four small patio garden areas. The proprietors plan to extend and renovate the building in the near future. This announced inspection was carried out over one day, when the inspector saw all parts of the home, and spoke to residents about their experience of life in the home. The inspector also spoke to staff and managers, and saw records and documents relating to residents and the management of the home. The inspector had previously received the CSCI pre-inspection questionnaire, which had been completed by the home, giving details of all aspects of the home. The inspector also received CSCI comment card replies from 12 close relatives, 2 GPs and 1 care manager. This report also reflects the findings at the home on the day of inspection. The proprietors are planning to expand the home in the near future, to improve the environment for the residents. Prospective service users have the information they need to make an informed choice about where to live. Each service user has a written contract/ statement of terms and conditions with the home. No service user moves into the home without having had his/her needs assessed and been assured that these will be met. Service users and their representatives know that the home they enter will meet their needs. Prospective service users and their relatives and friends have an opportunity to visit and assess the quality, facilities and suitability of the home. Service users assessed and referred solely for intermediate care are helped to maximise their independence and return home. The Commission considers Standards 3 and 6 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 3. Prospective residents have their needs assessed, and are only admitted when their needs can be met at the home. EVIDENCE: Prospective residents have their needs assessed by a care manager and a senior manager from the home. The inspector saw residents’ records showing that information is also available through the discharge summary if the individual is admitted from hospital. Each resident has a care plan developed from the assessment and from observations following admission to the home. The service user’s health, personal and social care needs are set out in an individual plan of care. Service users’ health care needs are fully met. Service users, where appropriate, are responsible for their own medication, and are protected by the home’s policies and procedures for dealing with medicines. Service users feel they are treated with respect and their right to privacy is upheld. Service users are assured that at the time of their death, staff will treat them and their family with care, sensitivity and respect. The Commission considers Standards 7, 8, 9 and 10 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 7, 8 and 9. The care planning systems are good, though more attention to detail is needed. Health care needs and medication issues are well managed. EVIDENCE: The inspector randomly examined three residents’ care plans and discussed their individual care with the residents. Personal and health care needs were set out in the care plans, and risk assessments had been undertaken to determine what actions were needed to minimise risks to residents’ health. Two of the care plans had detailed actions recorded to help staff to meet the care needs. One care plan needed to be developed to include specific detail regarding catheter care and the plan needed to be signed by the resident or the resident’s representative. The majority of respondents to the CSCI comment cards said they were satisfied with the care received at the home. The inspector noted that most individuals were able to keep their family GP when entering the home and the GP visited as necessary. The registered manager discussed how she referred individuals to other health care professionals such as the optician, chiropodist, dentist and audiologist as necessary, and sought advice from health professionals such as the tissue viability nurse on an individual basis. Better monitoring of wounds takes place so that care can be reviewed quicker if necessary. The registered nurses are responsible for all aspects of medicine management at the home. The inspector examined the medicine records, including the controlled drugs, and was satisfied that appropriate records are kept and that medicine is correctly administered. Service users find the lifestyle experienced in the home matches their expectations and preferences, and satisfies their social, cultural, religious and recreational interests and needs. Service users maintain contact with family/ friends/ representatives and the local community as they wish. Service users are helped to exercise choice and control over their lives. Service users receive a wholesome appealing balanced diet in pleasing surroundings at times convenient to them. The Commission considers all of the above key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 15. Residents are satisfied with the food provided. EVIDENCE: The inspector discussed food issues with several residents. All were satisfied with the food, some saying it was very good. Others commented that fresh vegetables were provided regularly and that the portions were good. A cooked breakfast can be ordered and one resident said that the choice at suppertime was good. There are four menus, rotating over the month, and although it is not clear on the menu that vegetarian options are available, the inspector understood that vegetarian meals can be provided. Therapeutic diets are provided, and food is presented appropriately, according to need. The planned expansion of the home intends to improve the dining facilities. Service users and their relatives and friends are confident that their complaints will be listened to, taken seriously and acted upon. Service users’ legal rights are protected. Service users are protected from abuse. The Commission considers Standards 16 and 18 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 16, 17 and 18. Complaints are well managed, and residents’ legal rights are protected. The home’s adult protection procedures are robust. EVIDENCE: The complaints procedure is easy to use, and is included in the Service User Guide supplied to all residents. The majority of residents know how to make a complaint and records are kept of any complaint received. Residents are able to consult with their legal advisers and are helped to vote at elections if they wish. The home’s adult protection procedures have been tested on two occasions, and staff are aware of the Oxfordshire Codes of Practice for the Protection of Vulnerable Adults. Service users have sufficient and suitable lavatories and washing facilities. Service users have the specialist equipment they require to maximise their independence. Service users’ own rooms suit their needs. Service users live in safe, comfortable bedrooms with their own possessions around them. Service users live in safe, comfortable surroundings. The home is clean, pleasant and hygienic. The Commission considers Standards 19 and 26 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 19 and 26. The home is safe, hygienic and well maintained. EVIDENCE: The home is located near the centre of Banbury, and is safe and well maintained. A record is kept of repairs and staff can report new issues on an ongoing basis. The fire service and environmental health reports are acted on in a timely way. Although there is no garden area the courtyard areas have raised flowerbeds with seasonal flowers, and provide an area for residents to get access to fresh air and sunshine. The home’s laundry is situated off a corridor and is small. Hand washing facilities are provided and the washing machine has appropriate washing cycles. The clean laundry needs to be stored in an appropriate container before being returned to the residents. The home has policies and procedures for the control of infection. Service users needs are met by the numbers and skill mix of staff. Service users are in safe hands at all times. Service users are supported and protected by the home’s recruitment policy and practices. Staff are trained and competent to do their jobs. The Commission considers Standards 27, 29, and 30 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 27, 28. 29 and 30. Staffing levels are appropriate for the needs of the residents, and the proprietors are committed to staff training. The home’s recruitment procedures are thorough. EVIDENCE: The home has a staff duty rota showing that appropriate numbers of staff are on duty at all times, and that a registered nurse is in charge of the home at all times. Extra staff are on duty at peak times of activity. The home has an induction programme, which needs some development to ensure that managers know that new staff achieve the desired competence levels by the end of the induction period. 50 of the home’s staff hold NVQ qualifications, and new staff are now required to start the NVQ training programme after the induction and probationary period as part of their employment contract. Mandatory staff training such as moving and handling takes place regularly, and update training such as wound management is provided for appropriate staff. Two members of staff are first aid trainers and plan to implement a training plan for staff soon. The inspector checked 3 staff recruitment records and found that the recruitment procedure is thorough, and that all the information required about staff is in place. Service users’ rights and best interests are safeguarded by the home’s record keeping, policies and procedures. The health, safety and welfare of service users and staff are promoted and protected. Formal supervision for all care staff has recently started. EVIDENCE: The inspector saw information about staff supervision showing that the home’s formal recorded staff supervision has recently been implemented, allowing care staff to meet with a supervisor at least six times a year.
