This inspection was carried out on 31st December 2005.

CSCI has not published a star rating for this report, though using similar criteria we estimate that the report is Good. The way we rate inspection reports is consistent for all houses, though please be aware that this may be different from an official CSCI judgement.

The inspector made no statutory requirements on the home as a result of this inspection and there were no outstanding actions from the previous inspection report.

What the care home does well

The Home continues to provide the residents with comfortable well decorated and furnished accommodation. Service users at the last inspection were unanimous in their praise for the staff and expressed their satisfaction with the care provided. Although the home lost eight staff to a new home recently opened, the existing staff and new manager have ensured the quality of care has not suffered. Activities and food are still high on the list of praise by the residents.

What has improved since the last inspection?

The registered manager is addressing issues in the home that have been outstanding for some time and progress is being made. Official staff supervision has commenced and staff morale has improved. Activities available to the residents continue to grow.

What the care home could do better:

The home must still adjust their staff induction programme to meet the National Training Standards. Development of the homes quality assurance system is necessary to measure whether the home is meeting its aims and objectives. Regulation 26 reporting must be consistent.

Date of last inspection Brief Description of the Service: Brooklands is a large detached establishment set within its own well maintained grounds in the Eastwood area of Leigh on Sea. The home is close in proximity to all community amenities and services. Brooklands is decorated, furnished and maintained to a good standard throughout. The home has carparking facilities to the front and rear of the property. Brooklands is a care home with nursing providing 29 single bedrooms and 8 shared bedrooms. The home provides nursing care for 44 Older People and one Physical Disability service user. Residents are accommodated on two floors that are serviced by a passenger shaft lift. This was a routine, unannounced inspection which took place over one day with one inspector in the home. There was a tour of the premises and grounds and an inspection of records and documentation. Time was spent observing the care of the residents. The proprietor, manager and one member of staff were spoken with, there were no relatives visiting on the day of inspection. Sixteen National Minimum Standards were inspected on this occasion, twelve were met and there were three requirements and one recommendation detailed in the full report. Discussion of the inspection findings took place with the proprietor and manager at the end and throughout the inspection and guidance was given. Prospective service users have the information they need to make an informed choice about where to live. Each service user has a written contract/ statement of terms and conditions with the home. No service user moves into the home without having had his/her needs assessed and been assured that these will be met. Service users and their representatives know that the home they enter will meet their needs. Prospective service users and their relatives and friends have an opportunity to visit and assess the quality, facilities and suitability of the home. Service users assessed and referred solely for intermediate care are helped to maximise their independence and return home. The Commission considers Standards 3 and 6 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 2,6 Appropriate verbal and written information is provided to prospective residents and their families to enable informed choices. The home provides a caring environment where visitors are made welcome. EVIDENCE: The home has amended the service users contract to meet this standard. Each service user is issued with a contract. together with terms and conditions of residency. This was evidenced for the homes most recent admission. The home does not provide intermediate care. The service user’s health, personal and social care needs are set out in an individual plan of care. Service users’ health care needs are fully met. Service users, where appropriate, are responsible for their own medication, and are protected by the home’s policies and procedures for dealing with medicines. Service users feel they are treated with respect and their right to privacy is upheld. Service users are assured that at the time of their death, staff will treat them and their family with care, sensitivity and respect. The Commission considers Standards 7, 8, 9 and 10 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 11 Good arrangements are in place to ensure that the ageing, illness and death of service users needs are identified and met. EVIDENCE: It was noted at this and previous inspections that service users wishes regarding terminal care and arrangements after death were recorded in their care plans. The manager stated that flowers are sent. Service users find the lifestyle experienced in the home matches their expectations and preferences, and satisfies their social, cultural, religious and recreational interests and needs. Service users maintain contact with family/ friends/ representatives and the local community as they wish. Service users are helped to exercise choice and control over their lives. Service users receive a wholesome appealing balanced diet in pleasing surroundings at times convenient to them. The Commission considers all of the above key standards to be inspected at least once during a 12 month period. Service users and their relatives and friends are confident that their complaints will be listened to, taken seriously and acted upon. Service users’ legal rights are protected. Service users are protected from abuse. The Commission considers Standards 16 and 18 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 17,18 The home has safeguards in place to ensure that service users legal rights are protected. Appropriate policies, practices and training are in place in respect of complaints and protection of vulnerable adults. EVIDENCE: Information on advocacy services is contained in the service users guide and displayed in the home. All service users are included in the voters register, those wishing to vote were said to usually vote the postal vote. Appropriate policies and procedures are in place for the Protection of Vulnerable Adults. Most staff have now received Protection of Vulnerable Adults training. Service users live in a safe, well-maintained environment. Service users have access to safe and comfortable indoor and outdoor communal facilities. Service users have sufficient and suitable lavatories and washing facilities. Service users have the specialist equipment they require to maximise their independence. Service users’ own rooms suit their needs. Service users live in safe, comfortable bedrooms with their own possessions around them. Service users live in safe, comfortable surroundings. The home is clean, pleasant and hygienic. The Commission considers Standards 19 and 26 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 19,22 The home continues to provide a high standard of accommodation for the residents. EVIDENCE: Although there is still no plan of renewal of fabric and decoration, the proprietor informed that bedrooms are redecorated with every change of occupancy and if anything goes wrong he repairs or replaces it immediately. All bedrooms seen were well furnished and decorated and personalised to each individual service users taste. A plan of renewal of fabric and decoration was discussed with the proprietor on the day of inspection. The home has sufficient aids and adaptations and has been assessed by an occupational therapist and was found to be suitable for service users. Service users’ needs are met by the numbers and skill mix of staff. Service users are in safe hands at all times. Service users are supported and protected by the home’s recruitment policy and practices. Staff are trained and competent to do their jobs. The Commission consider all the above are key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 28,29,30 The procedures for the recruitment and training of staff have safeguards in place to offer protection to people living in the home. The home has an effective and competent staff team. Organisation of staff training requires further development. EVIDENCE: Four staff records were examined during a visit to the home on the 8th February 2005. These were found to contain all the information required to meet this standard. The same was evident at this inspection with the recruitment records sampled. Staff induction programme is still not to TOPSS specification. A wide range of training is available to staff, however, a programme needs to be developed to ensure all staff receive training and refresher courses at the appropriate times. This was discussed with the manager who stated that she is prioritising the investigation of this. Service users live in a home which is run and managed by a person who is fit to be in charge, of good character and able to discharge his or her responsibilities fully. Service users benefit from the ethos, leadership and management approach of the home. The home is run in the best interests of service users. Service users are safeguarded by the accounting and financial procedures of the home. Service users’ financial interests are safeguarded. Staff are appropriately supervised. Service users’ rights and best interests are safeguarded by the home’s record keeping, policies and procedures. The health, safety and welfare of service users and staff are promoted and protected. The Commission considers Standards 31, 33, 35 and 38 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 31,33,34,35,36,37 There is leadership, guidance and direction to staff and the home has in place practices that promote and safeguard the health, safety and welfare of the people using the service. Attention is still required with regard to legislative monitoring and audit within the home. EVIDENCE: The current manager is currently undertaking her Registered managers Award in order to meet to meet NVQ level 4 in management and care requirements. At the previous inspection a member of staff who had worked in the home for a number of years felt that the new manager was “the best thing that had happened to the home”. Staff said “she was easy, approachable and they had confidence in her.” The home does not have an official quality assurance scheme, however, although Regulation 26 visits are being carried out by the proprietor, these have not been consistent and the proprietor was reminded regarding this. Discussions took place regarding further action required to meet this standard. The home has appropriate insurance policies. A current employer liability certificate was displayed. It is judged that procedures are in place to ensure appropriate management of the business and there is no evidence to suggest that the home is not financially viable. The home looks after small amounts of residents personal money for day to day expenses e.g. Hairdresser, chiropody etc. A random sample of money and records checked at the last inspection were seen to balance. Staff supervision has commenced but is still incomplete. The acting manager discussed with the inspector delegating care staff supervision to the Registered Nurses in the near future. All staff spoken with were very knowledgeable of service users needs and all have received recognised training over the years. However, there is a need to provide an organised programme to ensure all staff receive training and refresher courses at the appropriate time. Records seen throughout the inspection were generally well maintained, accessible and securely stored. Some records have been commented on throughout the report. The Registered Provider must 12/02/06 ensure that effective quality assurance and quality monitoring systems, based on seeking the views of service users are in place to measure the success in meeting the aims and objectives and the Statement of Purpose of the home.
