This inspection was carried out on 31st May 2005.

The inspector made no statutory requirements on the home as a result of this inspection and there were no outstanding actions from the previous inspection report.

What the care home does well

The Home continues to provide the residents with comfortable well decorated and furnished accommodation. Service users were unanimous in their praise for the staff and expressed their satisfaction with the care provided. Although the home lost eight staff to a new home recently opened, the existing staff have ensured the quality of care has not suffered. Activities and food were high on the list of praise by the residents.

What has improved since the last inspection?

The acting manager is gradually addressing issues in the home that have been outstanding for some time. Official staff supervision has commenced and staff morale improved. Activities available to the residents continue to grow.

What the care home could do better:

The home must adjust their staff induction programme to meet the National Training Standards. Development of the homes quality assurance system is necessary to measure whether the home is meeting its aims and objectives.

Date of last inspection 10th March 2005 Brief Description of the Service: Brooklands is a large detached establishment set within it’s own well maintained grounds in the Eastwood area of Leigh on Sea. The home is close in proximity to all community amenities and services. Brooklands is decorated, furnished and maintained to a good standard throughout. The home has car-parking facilities to the front and rear of the property. Brooklands is a care home with nursing providing 29 single bedrooms and 8 shared bedrooms. The home provides nursing care for 44 Older People and one Physical Disability service user. Residents are accommodated on two floors that are serviced by a passenger shaft lift. This was a routine unannounced inspection which took place during the afternoon and evening of the 31st May 2005. and lasted for 7.5 hours. Opportunity was taken for a quick tour of the premises and inspection of records and documentation. Discussions were held with the proprietor, manager and 5 staff. Most of the residents were seen during the inspection and several were spoken to. Two visiting relatives gave their views on the home. Prospective service users have the information they need to make an informed choice about where to live. Each service user has a written contract/ statement of terms and conditions with the home. No service user moves into the home without having had his/her needs assessed and been assured that these will be met. Service users and their representatives know that the home they enter will meet their needs. Prospective service users and their relatives and friends have an opportunity to visit and assess the quality, facilities and suitability of the home. Service users assessed and referred solely for intermediate care are helped to maximise their independence and return home. The Commission considers Standards 3 and 6 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 1,3,4,5 The home operates a thorough pre-admission assessment process, which ensures the home can meet individual needs. Appropriate verbal and written information is given to prospective residents and their families and preadmission visits encouraged to enable informed choices. EVIDENCE: The home has an appropriate statement of purpose and service users guide which has recently been updated. Care records evidenced a full assessment had been undertaken by the manager or senior staff. Full social worker assessments were available for those residents sponsored by local authorities. Although eight care staff have recently left the home for a new recently opened home, the home still maintains a core group of experienced staff, both as nurses and carers, who were knowledgeable of the residents health and care needs. All prospective residents and their families are invited to visit the home before making a decision. The service user’s health, personal and social care needs are set out in an individual plan of care. Service users’ health care needs are fully met. Service users, where appropriate, are responsible for their own medication, and are protected by the home’s policies and procedures for dealing with medicines. Service users feel they are treated with respect and their right to privacy is upheld. Service users are assured that at the time of their death, staff will treat them and their family with care, sensitivity and respect. The Commission considers Standards 7, 8, 9 and 10 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 7-10 Residents’ needs are being met by the home and records evidenced this. The home has a stable care group of staff who have a good understanding of residents’ health and care needs. Staff treat service users in a sensitive and respectful manner. EVIDENCE: Examination of a random sample of residents care plans evidenced an admission care plan and a comprehensive care plan developed during the trial period. Care plans seen were to a good standard, clearly identifying residents needs and giving concise instructions to staff on how those needs should be met. There was evidence of service user and family involvement. No resident has been assessed as able to manage their own medication. All medication is administered by nursing staff. Records were completed appropriately. Records showed good monitoring of health needs and appropriate referrals to medical professionals. Residents were complimentary with regards to the way their personal and health care needs were being met and felt that staff treated them in an appropriate way, respecting their privacy and dignity. Residents wishes are recorded in their care plans. Service users find the lifestyle experienced in the home matches their expectations and preferences, and satisfies their social, cultural, religious and recreational interests and needs. Service users maintain contact with family/ friends/ representatives and the local community as they wish. Service users are helped to exercise choice and control over their lives. Service users receive a wholesome appealing balanced diet in pleasing surroundings at times convenient to them. The Commission considers all of the above key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 12-15 Social activities and meals are both well managed, creative and provide daily variation for the residents. Visiting arrangements were open and relaxed. The home supplied sufficient quantity and quality of food and provided a well balanced diet that met individual needs. EVIDENCE: The home has an enthusiastic activities organiser who arranges a wide range of activities for the residents both in groups and individually including activities which take the residents outside the home. She said she hopes to develop a mobile shop for residents to purchase small items including toiletries and sweets. On the day of the inspection a residents meeting took place where residents were able to express their opinion on the home’s menus, quality of food, activities and on the general running of the home. Residents were generally satisfied with the level of activities and said they can say what they like at the residents meetings. Visiting relatives said they were always made welcome and can visit when they liked. One relative commented that he was very impressed with the home. The home’s menus were seen to contain a wide range of varied and nourishing meals. Residents commented that the quality and choices of food provided is to a high standard. Residents requiring assistance with eating were helped in a sensitive and unhurried manner. Individual nutrition records are maintained. The main kitchen was large, well equipped and spotlessly clean and tidy. Service users and their relatives and friends are confident that their complaints will be listened to, taken seriously and acted upon. Service users’ legal rights are protected. Service users are protected from abuse. The Commission considers Standards 16 and 18 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 16 and 18 Appropriate policies, practices and training are in place in respect of complaints and protection of vulnerable adults. EVIDENCE: The home has an appropriate complaints policy and procedure. No complaints had been received since the previous inspection. Appropriate policies and procedures are in place for the Protection of Vulnerable Adults. Some staff have received Protection of Vulnerable Adults training. All other staff have bee booked on courses throughout 2005. The home is clean, pleasant and hygienic. The Commission considers Standards 19 and 26 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) The home continues to provide a high standard of accommodation for the residents. EVIDENCE: Although there is no plan of renewal of fabric and decoration, the proprietor informed that bedrooms are redecorated with every change of occupancy and if anything goes wrong he repairs or replaces it immediately. All bedrooms seen were well furnished and decorated and personalised to each individual service users taste. The home was clean and tidy throughout and is a credit to the domestic staff. The home benefits from a large well equipped laundry. Service users needs are met by the numbers and skill mix of staff. Service users are in safe hands at all times. Service users are supported and protected by the home’s recruitment policy and practices. Staff are trained and competent to do their jobs. The Commission considers Standards 27, 29, and 30 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 27 & 30 Staffing levels are being maintained with existing staff working additional shifts and extra hours. However, there appears to be a better team spirit amongst the staff. Organisation of staff training requires further development. EVIDENCE: Staff rotas indicated staffing levels are being maintained. However with 8 care staff leaving to join a newly built home and recruitment of staff being difficult, existing staff are having to work additional shifts and on some occasions long shifts. Some staff spoken with felt they were better off without the staff that left and said there was a better atmosphere in the home and existing care staff and nursing staff are now working as a real team. The proprietor said he is recruiting more adaptation nurses from abroad. Staff recruitment policies and practices were not inspected in depth at this inspection as the standard was met at the previous inspection. Staff induction programme is not to TOPSS specification. A wide range of training is available to staff, however, a programme needs to be developed to ensure all staff receive training and refresher courses at the appropriate times Staff are now receiving supervision on a regular basis. Service users said,” the staff are lovely and caring although at times they are very busy”. Service users live in a home which is run and managed by a person who is fit to be in charge, of good character and able to discharge his or her responsibilities fully. Service users benefit from the ethos, leadership and management approach of the home. The home is run in the best interests of service users. Service users are safeguarded by the accounting and financial procedures of the home. Service users’ financial interests are safeguarded. Staff are appropriately supervised. Service users’ rights and best interests are safeguarded by the home’s record keeping, policies and procedures. The health, safety and welfare of service users and staff are promoted and protected. The Commission considers Standards 33, 35 and 38 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 31,32,33,35,36, 38 The home has shown improvement in the management and staff morale since the new acting manager was appointed. She is aware that there is still much to be done and is working towards raising standards. EVIDENCE: A member of staff who had worked in the home for a number of years felt that the new manager was “the best thing that had happened to the home”. Staff said “she was easy, approachable and they had confidence in her.” The acting manager has applied for registration and is awaiting a “Fit Person Interview”. She is seeking training to meet the Registered Managers’ award. The home does not have an official quality assurance scheme, however Regulation 26 visits are being carried out by the proprietor, satisfaction Brooklands I56 I06 S15524 Brooklands V222820 310505 Stage 4.doc Version 1.20 Page 16 questionnaires have been received from all the service users and questionnaires are being prepared for relatives and visiting professionals. The home looks after small amounts of residents personal money for day to day expenses eg. Hairdresser, chiropody etc. A random sample of money and records checked were seen to balance. Staff supervision has commenced and all staff except four have received their first supervision by either the manager or Senior Nurse. The acting manager explained that she will be delegating care staff supervision to the Registered Nurses in the near future. All staff spoken with were very knowledgeable of service users needs and all have received recognised training over the years. However, there is a need to provide an organised programme to ensure all staff receive training and refresher courses at the appropriate time. The acting manager was aware of her duties under health and safety. The home’s maintenance person carries out regular safety checks. Safety certificates were available for services and equipment.
