It is located in the Old Swan district of Liverpool. Local amenities such as shops and churches can be found within a short walking distance of the care home. The care home is situated within several miles of Liverpool city centre and enjoys good transport links via bus to the centre. The care home has been purpose built for the provision of care. Accommodation is provided on two floors and a passenger lift provides full access to all areas of the home. There is wheelchair access to the care home and adequate parking at the front of the premises. There is a pleasant private garden to the rear of the building. This is an overview of what the inspector found during the inspection. This inspection was conducted over a period of six hours at a time when service users and their visitors could be spoken to and the main meal of the day observed. During this inspection, records relating to the identified needs of service users together with the records of the actual care given. All were found to be comprehensive and included full risk assessments and risk management strategies. Staff records were inspected to ensure that all staff were appropriately checked prior to them commencing work at the home and that relevant training had been given. The staff files were found to be clear and up to date and included all information necessary. The home is decorated and furnished to a high standard and provides a warm and pleasant environment. The intended outcomes for Standards 1 – 6 are: 1. 2. 3. 4. 5. 6. Prospective service users have the information they need to make an informed choice about where to live. Each service user has a written contract/ statement of terms and conditions with the home. No service user moves into the home without having had his/her needs assessed and been assured that these will be met. Service users and their representatives know that the home they enter will meet their needs. Prospective service users and their relatives and friends have an opportunity to visit and assess the quality, facilities and suitability of the home. Service users assessed and referred solely for intermediate care are helped to maximise their independence and return home. The Commission considers Standards 3 and 6 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 3 Pre-admission assessments are undertaken on prospective service users by qualified nurses to ensure that the home can meet their individual needs and preferences and therefore provide the appropriate level of care. EVIDENCE: Pre-admission assessments are undertaken on all prospective service users to ensure that the home can meet their individual needs and preferences. The format for the assessments has been reviewed and amended to ensure that all information is gathered and recorded. These assessments are undertaken by the manager or one of the qualified nurses. The information gathered enables the home to prepare a plan of care and to give the opportunity for any necessary equipment to be provided prior to admission. Information is gathered from the service user, their family and other healthcare professionals involved in their care. The assessment information is held securely in the main office when the plan of care has been prepared as confidential information may be recorded on the form.

The intended outcomes for Standards 7 – 11 are: 7. 8. 9. 10. 11. The service user’s health, personal and social care needs are set out in an individual plan of care. Service users’ health care needs are fully met. Service users, where appropriate, are responsible for their own medication, and are protected by the home’s policies and procedures for dealing with medicines. Service users feel they are treated with respect and their right to privacy is upheld. Service users are assured that at the time of their death, staff will treat them and their family with care, sensitivity and respect. The Commission considers Standards 7, 8, 9 and 10 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 7, 8, 9, 10 Comprehensive and up to date care plans are in place to give staff full information to enable them to provide the appropriate level of care required by individual each service user. EVIDENCE: Comprehensive care plans are prepared for all service users. The plans are informative and provide both nurses and care staff with full information regarding the service users care needs. Risk assessments have been undertaken on all service users to ensure that any risks are identified and appropriate risk management strategies put in place to reduce or remove those risks. All care plans inspected were found to contain the signature of the service user or their representative to provide evidence that the plans have been discussed with them and their agreement sought. All service users are registered with a doctor of their choosing. The records also provide evidence of visits to and by other healthcare professionals and

The Tissue Viability Nurse is contacted for advice for any service user who has pressure sores as necessary. None of the service users accommodated at the home has a pressure sore. The home has a policy and a procedure to be followed for the administration of medications. Records relating to the administration of medications were found to be in order and completed as required. All medications are stored securely and the storage areas were clean and organised. Appropriate arrangements have been made for the disposal of unwanted medications and safety measures have been put in place. Personal care is given to service users in their bedroom or in bathrooms as appropriate. The majority of service users are accommodated in single bedrooms and staff were seen to knock on doors prior to entering rooms. The home provides seven shared rooms, however, these are only used for shared occupancy at the specific request of service users. Service users may meet with their visitors in the privacy of their bedroom or in one of the communal areas as they wish. The home has also prepared risk assessments on the safety locks that have been fitted to fire doors, the electric wheelchair used by one service user and on dogs that visit the home to further ensure the protection of service users. Service users find the lifestyle experienced in the home matches their expectations and preferences, and satisfies their social, cultural, religious and recreational interests and needs. Service users maintain contact with family/ friends/ representatives and the local community as they wish. Service users are helped to exercise choice and control over their lives. Service users receive a wholesome appealing balanced diet in pleasing surroundings at times convenient to them. The Commission considers all of the above key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 12, 13, 14, 15 Dietary needs of service users are extremely well catered for with a balanced and varied selection of food available that meets service users tastes and choices. EVIDENCE: Service users files give details of individual preferences including the times that service users choose to go to bed or rise and where they prefer to take their meals. A comprehensive programme of activities is prepared and details are displayed in the foyer. Activities include both group events and one to one time with service users and records are held of the specific activities that each service user participates in. Activities include dominoes, bingo, reminiscence, music, entertainers and trips to local shops. Service users recently enjoyed a folk music group. Observation of the interaction between visitors and staff provided evidence that they were welcomed to the home and given information and updates on their relatives on a regular basis. The home follows a four weekly menu and a choice is available at all meals. The menus have recently been reviewed and service users have been involved in the changes. They have had the opportunity to express their preferences and these have been incorporated into the choices offered. The menus provide
Brooklands Nursing and Residential Home DS0000025092.V285496.R01.S.doc Version 5.1 Page 12 evidence that a wholesome and nutritious diet is served to service users and the meal being served on the day of the inspection appeared hot and appetising. Service users spoke highly of the meals served. Special diets can be provided on the advice of the dietician or on request and a choice of specialist meals is now offered. The home has now introduced a ‘Friday night special’ which includes offering curry’s or hot dogs as an option. The cook speaks with all new admissions to the home to gather information regarding individual food preferences and to discuss the dietary options available. Service users are given the opportunity to choose the meal of their preference and a record is held of the meals taken by each service user. Service users’ legal rights are protected. Service users are protected from abuse. The Commission considers Standards 16 and 18 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 16, 18 Staff have a good knowledge and understanding of Adult Protection issues which protects service users from abuse. EVIDENCE: The home has a detailed policy and procedure to be followed in the event of a complaint and information on this, and how to make a complaint is detailed in the Service User Guide which is in each bedroom. This information is also displayed on the notice board in the hallway of the home. Service users spoken to said that they had never needed to make a complaint but would feel confident that any issues would be addressed. Training has been given to staff on the different types of abuse and of the action to be taken if it were suspected. The manager was able to demonstrate that she was aware of the policy, the procedure to be followed and the people to contact. Service users live in safe, comfortable bedrooms with their own possessions around them. Service users live in safe, comfortable surroundings. The home is clean, pleasant and hygienic. The Commission considers Standards 19 and 26 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 19, 26 The owner and manager continue to improve the appearance of this home creating a comfortable and safe environment for those living there and visiting. EVIDENCE: The home is located in a predominantly residential area, close to shops and other amenities. The home is purpose built and provides 35 single bedrooms and 7 shared rooms. Eighteen of the rooms are provided with en-suite facilities. The shared rooms are used for single occupancy unless the service user requests to share, and at present, a married couple occupy one of the rooms. At the time of the inspection, all other rooms were for single occupancy. A programme of redecoration and improvements continues to take place with bedrooms being redecorated as they become vacant. The home is extremely clean and hygienic throughout and there are no unpleasant odours.

All bedrooms inspected were found to be bright and are decorated and furnished to a high standard. The lounges are bright and provide service users with a pleasant area to sit and socialise. The use of the small smoking lounge is currently being reviewed in line with the changing needs of the service users. Service users are supported and protected by the home’s recruitment policy and practices. Staff are trained and competent to do their jobs. The Commission consider all the above are key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): Staff morale is high resulting in an enthusiastic workforce that works positively with service users to improve their whole quality of life. EVIDENCE: The home employs and deploys staff in sufficient numbers to meet the needs of the service users. The home provides qualified nurses at all times who are supported by care staff. In addition to this, the home also employs ancillary staff to attend to domestic, laundry and maintenance duties. A domestic supervisor is responsible for overseeing all domestic tasks. The records provide evidence that the home has a robust recruitment policy and procedure and that this is followed for all new staff. All prospective staff are required to complete an application form prior to interview. Two relevant references are taken together with Criminal Record Bureau and Protection of Vulnerable Adults registers. Copies of all necessary documentation, together with evidence of training qualifications are held in the staff files. All staff undertake a programme of induction training when they commence work at the home. On going training is undertaken by all staff and a programme of training was seen. Staff spoken to during the inspection confirmed that they continued to develop their knowledge and understanding through on-going training. Staff were

Service users live in a home which is run and managed by a person who is fit to be in charge, of good character and able to discharge his or her responsibilities fully. Service users benefit from the ethos, leadership and management approach of the home. The home is run in the best interests of service users. Service users are safeguarded by the accounting and financial procedures of the home. Service users’ financial interests are safeguarded. Staff are appropriately supervised. Service users’ rights and best interests are safeguarded by the home’s record keeping, policies and procedures. The health, safety and welfare of service users and staff are promoted and protected. The Commission considers Standards 31, 33, 35 and 38 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 31, 33, 35, 38 EVIDENCE: The manager is a qualified nurse with extensive experience in the care of elderly people. She is an experienced manager who has recently completed an NVQ 4 in management and is currently working towards a Degree in Health and Social Care Practice. Staff meetings are held on a regular basis and provide the opportunity for staff to exchange information and discuss any issue within a group environment. The owner of the home, who is also a registered nurse, works in the home on a daily basis. Service users family or advocates attend to service users personal finances. The home holds small amounts of money, up to a maximum of £10, on the request of families and detailed records are held of all purchases made by service users.

Tests are made on all equipment in the home including portable appliances, lift and hoists. Certificates of tests were found to be in place and up to date.
