This inspection was carried out on 4th August 2005.

The inspector found there to be outstanding requirements from the previous inspection report. These are things the inspector asked to be changed, but found they had not done. The inspector also made 1 statutory requirements (actions the home must comply with) as a result of this inspection.

What the care home does well

There is a stable staff group who have worked at the home for a good length of time. The staff has a clear understanding of their roles and responsibilities and have the required skills to assist service users who live at the home. Service users communicate using Makaton signing and staff have developed training packages to ensure they can understand and communicate with service users. Clear skills and development programmes help staff to provide appropriate support to service users to develop independent living skills, risk assessments ensures that service users are safe and protected while participating in leisure pursuits.

What has improved since the last inspection?

The registered provider continues to improve the environment by decorating areas as required and service users are encouraged to participate in choosing colour schemes for their own bedrooms. Training opportunities for staff continues, and the deputy manager arranges distant learning courses for staff as they become available.

What the care home could do better:

Staff records and discussions with the deputy manager demonstrated that they have not received any training in managing challenging behaviour, this must be organised by the registered manager to ensure that staff have the necessary intervention skills.

Staffing levels are sufficient to ensure service users can be supported to pursue lifeskills and leisure activities. The property is a modern four bedroom detached bungalow in a residential part of Rotherham. It has three single bedrooms, a large diner, a domestic kitchen, bathing facilities and a staff sleeping in room. There are lawns to the front and side of the house. The area is enclosed and has an outdoor seating area for the service users. This unannounced inspection was conducted over 3.5 hours where a partial tour of the building was undertaken. The inspector examined three service users care plans and supporting documentation. Two service users were spoken to during the visit. The deputy manager assisted with the process while examining records, policies and procedures. JUDGEMENT – we looked at outcomes for standard(s) 2 Service users are not admitted into the home without a full needs assessment taking place by the registered manager, to ensure their needs can be met. Procedures ensure that visits take place prior to any arranged stays at the home. EVIDENCE: Discussion with deputy manager demonstrates that prospective service user are given the opportunity to visit the home on a number of occasions prior to making any decision about their stay. All service users have lived at the home for a number of years, three service users files examined demonstrated that assessments have taken place to ensure staff are able to meet complex care needs. Service users know their assessed and changing needs and personal goals are reflected in their individual Plan. Service users make decisions about their lives with assistance as needed. Service users are consulted on, and participate, in all aspects of life in the home. Service users are supported to take risks as part of an independent lifestyle. Service users know that information about them is handled appropriately, and that their confidences are kept The Commission considers Standards 6, 7 and 9 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 6, 7, 8, 9. The home clearly promotes philosophies to enable residents to meet their full potential, with clear care plan instructions and comprehensive risk assessments to maximise their safety and protection. EVIDENCE: The registered manager operates ‘Person Centred Plans’ for two of the service users who attend training centres. These are formally reviewed every four months. The reviews may take place either at the home or at the day centre and are used to coordinate activities at the centre and aims and dreams within the home setting. There is also a comprehensive individual plan for each service user, that is user friendly. They are well written with a daily entry made recording any significant events. These plans describe in detail the intervention required to meet their needs. One service user is able to manage his/her own finances and the manager acts as appointee for the other two service users. Documentation supporting the financial arrangements was examined and was found to be accurate. Service users have opportunities for personal development. Service users are able to take part in age, peer and culturally appropriate activities. Service users are part of the local community. Service users engage in appropriate leisure activities. Service users have appropriate personal, family and sexual relationships. Service users’ rights are respected and responsibilities recognised in their daily lives. Service users are offered a healthy diet and enjoy their meals and mealtimes. The Commission considers Standards 12, 13, 15, 16 and 17 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 12, 13, 14, 15, 16, 17. Where appropriate service users are encouraged and supported to develop independent living skills. Social and leisure activities are provided at the home for service users, to maintain and develop independent living skills, supported by an informed staff group. Service users are helped to maintain links with family members, with weekend and evening visits, as agreed in their care plans. Dietary needs of service user are well catered for with a varied selection of food available that meets service users tastes and choices. EVIDENCE: Service users are encouraged and supported to maintain links with the local community and two regularly attend a local group at ‘the meeting place’ organised by Mencap. Two service users continue to attend local training centres during the week, and the home has information about the activities for each day they attend. Service users are encouraged and supported to undertake routine tasks around the home including preparing vegetables and washing the pots. Service users are also encouraged and to tidy their own bedrooms and change their own beds. Service users are encouraged and supported to maintain links with family members and spend time visiting either with support from staff or independently. One service users has recently spent time at his/her parents to attend a family wedding. Mealtimes are organised around the routines of the service users and the main meal is provided at teatime when it is expected that all service users will be at home. Packed lunches are provided for those service users who are out for the day and service users are encouraged and supported to make their own lunch. Service users receive personal support in the way they prefer and require. Service users’ physical and emotional health needs are met. Service users retain, administer and control their own medication where appropriate, and are protected by the home’s policies and procedures for dealing with medicines. The ageing, illness and death of a service user are handled with respect and as the individual would wish. The Commission considers Standards 18, 19, and 20 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 18, 19, 20. Staff provides sensitive personal support to ensure privacy dignity and independence is maintained for all service users. Medication policies and procedures are well managed and staff have the necessary skills to administer the medication to service users, ensuring the safety and protection of service users. EVIDENCE: Service users were referred to by their first name and this was with the approval of service users. Staff have the necessary knowledge and skills to ensure appropriate intervention with regard to personal tasks are undertaken. There is a small staff group who have equal responsibility for each service user. The deputy manager is able to make the necessary arrangements for dealing with health care issues, including doctors and hospital appointments. An audit of medication stocks and records was examined and were found to be correct. All staff has received accredited medication training provided by a local college, to ensure they have the necessary skills and knowledge to undertake this task. Service users feel their views are listened to and acted on. Service users are protected from abuse, neglect and self-harm. The Commission considers Standards 22, and 23 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 22, 23. Service users are provided with information to enable them to raise concerns about the home and their care. Adult Abuse Policies and procedures and training of staff on abuse ensure the protection of service users from abuse EVIDENCE: The registered manager has a Complaints procedure, which is displayed on the wall in the entrance area of the home so that everyone is aware of it and states how complaints will be dealt with. There have been no complaints, which have required investigation, and this was confirmed by examining the procedure file. The policy is promoted to service users through discussion at care plan reviews and Advocacy Services are available to assist service users with any concerns. The registered manager has a copy of the Local Authorities Adult Protection procedures. They also have an Abuse Policy, which has been prepared by the owners and is incorporated into the homes policy and procedure manual. The deputy manager reiterates the procedures at staff meetings and supervision and staff training files also confirms that they have attended training in adult protection. The registered manager ensures that all the necessary checks are carried out on new staff for the protection of vulnerable adults. Service users have the specialist equipment they require to maximise their independence. The home is clean and hygienic. The Commission considers Standards 24, and 30 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 24, 30. The home provides a well-maintained safe environment suitable for service users. The home provides a clean and hygienic environment to maintain the health and safety of service users. EVIDENCE: A partial tour of the building found it clean and free from odours. The home provides comfortable communal areas with a lounges/dining area and a domestic kitchen. The grounds were tidy, and accessible to the service users; appropriate seating is available for service users. The home is close to all local amenities, and service users are able to walk to Rotherham town centre. Individual bedrooms are personalised to service users own interests and hobbies and are furnished appropriately including music centres and televisions. JUDGEMENT – we looked at outcomes for standard(s) 32, 33, 34, 35. Staff have the necessary skills and knowledge to meet the needs of service users. There is an effective staff team with regular meetings and supervision ensures staff are kept informed about the services provided. Recruitment policies are followed ensuring the safety and protection of service users. EVIDENCE: Staff are organised to ensure there are sufficient to meet the needs of service users who spend their time at the home during the day. Staff have the necessary skills and have achieved awards in care. Staff have worked at the home for a good length of time and have good relationships with the service users. The staff is able to communicate with service users by using Makaton signing. Staff access distance learning course to ensure refresher training is undertaken. Discussion with the deputy manager indicated that staff have not received any formal training to deal with challenging behaviour, therefore staff must attend a behaviour management course to ensure they have the necessary skills to deal appropriately with service users. The managers have responsibility for the recruitment of staff and there is currently one vacancy, and the person appointed is awaiting the required clearance from the CRB check and references. JUDGEMENT – we looked at outcomes for standard(s) 37, 42. The home is well managed to ensure the safety and protection of the service users. Staff and service users follow health and safety procedures and records provide evidence of servicing of essential equipment. EVIDENCE: The registered manager, who is also the owner of the home, has considerable experience and knowledge of younger adults with learning difficulties. The registered manager is working towards the registered managers award and the deputy manager has attained the award along with NVQ Level 4 in management. The deputy manager has indicated that she is considering making an application to be the registered manager at Broom haven, as she is mainly in charge of day-to-day operations at the home. The registered manager has the required Health and Safety policies and procedures and maintenance and service records examined were up to date and current to the services provided.
